A Medicare premium bill lands in your mailbox when your premium isn’t being pulled automatically from a Social Security or Railroad Retirement Board payment. The bill itself is Form CMS-500, and it can include your Part B premium, a Part A premium if you owe one, an income-related surcharge, a late enrollment penalty, or some combination. Why Medicare Is Billing You Directly
Most people never see a CMS-500 because their premium is withheld from a monthly Social Security or Railroad Retirement Board check before it hits their bank account. Direct billing happens when that deduction isn’t possible.2Medicare. Medicare Premium Bill (CMS-500)
The usual reason: you signed up for Part B at 65 but haven’t claimed Social Security yet, either because you’re still working or because you’re waiting to boost your eventual benefit. No check means no deduction, so Medicare bills you.
You can also end up on direct billing if your Social Security payment is too small to cover the full premium. In that case, the government takes what it can and bills you for the shortfall, or bills you for the entire premium. These bills generally cover three months of upcoming coverage at a time, though some people are billed monthly.2Medicare. Medicare Premium Bill (CMS-500)
What the Bill Is Actually Charging You For
The CMS-500 lists each charge on its own line, along with the coverage period it pays for and a due date, typically the 25th of the month.2Medicare. Medicare Premium Bill (CMS-500) If your total looks off, start by matching each line to one of these categories.
Part B Premium
The standard Part B premium in 2026 is $202.90 a month, up from $185.00 in 2025.1Centers for Medicare & Medicaid Services. 2026 Medicare Parts A and B Premiums and Deductibles This is what most enrollees pay. If your Part B line is higher than that, the extra is almost always an income surcharge, a late enrollment penalty, or both.
Part A Premium
Most people pay $0 for Part A because they or a spouse paid Medicare taxes for at least 10 years (40 quarters). If you fall short of that, Part A costs money:1Centers for Medicare & Medicaid Services. 2026 Medicare Parts A and B Premiums and Deductibles
- 30 to 39 quarters of Medicare-covered employment: $311 per month in 2026
- Fewer than 30 quarters: $565 per month in 2026
This charge catches people who spent much of their career abroad or in jobs that didn’t pay into Medicare.
IRMAA Surcharge
If you’re a higher-income beneficiary, your Part B line will carry an income-related monthly adjustment amount (IRMAA). The Part D IRMAA shows up as a separate line on the same bill. Both are explained in the next section.
Late Enrollment Penalty
A permanent surcharge added if you delayed signing up for Part B or Part D without qualifying coverage in between. Also explained below.
If the Bill Is Higher Than You Expected
An Income-Related Surcharge (IRMAA)
IRMAA is the single most common reason a Medicare bill comes in higher than people planned for. The Social Security Administration applies it based on the modified adjusted gross income on your tax return from two years earlier. That two-year lag trips people up. A one-time income event in 2024, like selling a house, cashing out a retirement account, or converting a traditional IRA to a Roth, can trigger a 2026 surcharge even though your current income is modest.

- $109,000 or less single / $218,000 or less joint: no surcharge, $202.90 total
- $109,001–$137,000 / $218,001–$274,000: $81.20 surcharge, $284.10 total
- $137,001–$171,000 / $274,001–$342,000: $202.90 surcharge, $405.80 total
- $171,001–$205,000 / $342,001–$410,000: $324.60 surcharge, $527.50 total
- $205,001–$499,999 / $410,001–$749,999: $446.30 surcharge, $649.20 total
- $500,000 or more / $750,000 or more: $487.00 surcharge, $689.90 total
Married people who file separately face a compressed scale. Part D carries its own IRMAA at the same income thresholds, ranging from $14.50 to $91.00 a month in 2026, added on top of whatever your drug plan charges.4Medicare.gov. 2026 Medicare Costs If you have Part D, you’ll see that amount as a separate line on your CMS-500.
A Late Enrollment Penalty
The Part B late enrollment penalty is 10% of the standard premium for every full 12-month period you were eligible for Part B but didn’t enroll, and it’s permanent. Delay two full years, pay 20% on top of $202.90 for the rest of your life on Medicare.5Medicare.gov. Avoid Late Enrollment Penalties You generally avoid it if you had qualifying employer coverage during the delay or you’re enrolled in a Medicare Savings Program.
The Part D penalty applies if you went 63 or more consecutive days without creditable prescription drug coverage after your initial enrollment period closed. It’s 1% of the national base beneficiary premium ($38.99 in 2026) times the number of months you went uncovered, added permanently to your monthly Part D premium. Fourteen months without creditable coverage, for example, works out to about $5.46 a month, recalculated each year as the base premium changes.5Medicare.gov. Avoid Late Enrollment Penalties
How to Pay the Bill

- Pay online through your Medicare.gov account.
- Enroll in Medicare Easy Pay for automatic bank deductions. Activation takes 6 to 8 weeks, so keep paying another way until the first automatic withdrawal confirms.6Medicare.gov. Medicare Easy Pay
- Use your bank’s online bill pay service.
- Mail a check, money order, or credit or debit card information with the coupon attached to your bill.
If you’re going to be billed directly for a while, Medicare Easy Pay is the lowest-risk option because it removes any chance of a missed deadline.
What Happens If You Don’t Pay
A missed payment doesn’t end your coverage immediately, but Medicare moves on a defined timeline. When your bill is past due by at least one billing period (generally 90 days from the initial bill), you’ll receive a delinquent notice showing both the current and past-due amounts and a final due date. Losing Part B this way is a serious problem. You typically can’t re-enroll until the next General Enrollment Period, which runs January through March, with coverage starting the month after you sign up.7Medicare.gov. When Does Medicare Coverage Start The resulting gap can stretch for months, and you may be hit with a 10% late enrollment penalty for each full year without coverage, permanently added to future premiums.5Medicare.gov. If you were terminated after an emergency or unusual circumstance kept you from paying, you can request reinstatement for “good cause.” The request has to be made within 60 days of the disenrollment date, and you must be able to pay everything owed within three months. The standard is narrow: it covers things like hospitalization or a natural disaster, not simply forgetting or being short on cash.8Centers for Medicare & Medicaid Services. Asking for a Lower IRMAA
If your bill carries an IRMAA that doesn’t match your current reality, you can ask SSA to recalculate.
After a Life-Changing Event

- Work stoppage or reduction (retirement, going part-time, job loss)
- Death of a spouse
- Divorce or annulment
- Marriage
- Loss of income-producing property
- Loss of pension income
- Employer settlement payment
You’ll submit evidence of both the event itself (retirement letter, death certificate, divorce decree) and your current, lower income (recent tax return or an estimate for the current year).9Social Security Administration. Wrong Tax Data or an Amended Return
If SSA used incorrect income data, or you filed an amended return that changed the year they relied on, call SSA at 1-800-772-1213 to request a correction. Submitting the Request
File Form SSA-44 online through your my Social Security account, fax or mail the paper form with your supporting documents to your local Social Security office, or call to start the process by phone.10Social Security Administration. SSA mails a written decision, and if approved, the lower premium takes effect with any overpayment credited back.
Help If You Can’t Afford the Premium
If the bill is just more than your budget allows, two sets of programs can help, and both are widely underused.
Medicare Savings Programs are state-run and can cover Part B premiums, and in some cases Part A premiums and cost-sharing. The 2026 federal income limits start at $1,350 per month for an individual and $1,824 for a couple at the Qualified Medicare Beneficiary level, with higher limits for the Specified Low-Income Medicare Beneficiary and Qualifying Individual programs. Federal resource limits are $9,950 for an individual and $14,910 for a couple, though some states set higher limits of their own.11Medicare.gov. Extra Help, also called the Low-Income Subsidy, assists with Part D premiums, deductibles, and copayments. Under the Inflation Reduction Act, eligibility now reaches up to 150% of the federal poverty level if you meet the resource test. For 2026, resources can’t exceed $16,590 for an individual or $33,100 for a married couple.12Centers for Medicare & Medicaid Services.
