Which State Has the Best Medicare Benefits for Seniors?

There is no single answer to which state has the best Medicare benefits, because Original Medicare is a federal program that pays for the same hospital and medical services in every zip code. What actually differs by state is the layer around Medicare: Medigap enrollment rules, state financial assistance programs, private plan competition, and a few consumer protections written into state law. On those measures, New York, Massachusetts, and Connecticut consistently offer the strongest package for seniors, with California, Oregon, and Maine close behind for specific advantages.

Why “Best State” Is the Wrong Question

Part A (inpatient hospital, skilled nursing, hospice, home health) and Part B (doctor visits, outpatient care, preventive screenings, durable medical equipment) work identically everywhere.1Medicare.gov. Parts of Medicare Premiums, deductibles, and coinsurance are set federally and apply nationwide.2Centers for Medicare & Medicaid Services. 2026 Medicare Parts A and B Premiums and Deductibles A retiree in rural Wyoming and one in downtown Boston face the same Part A deductible and the same Part B premium.

So when people ask which state is “best,” what they’re really comparing are four things a state can influence: how easy it is to buy or switch a Medigap policy, how generous the state’s financial assistance is for low-income beneficiaries, how competitive the private Medicare Advantage market is locally, and whether the state runs its own prescription drug assistance program. A handful of states do noticeably better on more than one of those.

The States That Stand Out

New York

New York offers the strongest Medigap protections in the country. Residents can enroll in or switch Medigap plans year-round, with no medical underwriting and no age-based pricing.3NAIC. Medigap Open Enrollment Rights State Summary Chart That protection matters most for people who develop a chronic condition after 65 and want to change plans without being denied or charged more.

New York also eliminates the asset test for its Medicare Savings Programs, so a homeowner with retirement savings can still qualify for help paying Part B premiums if income is low enough.4Medicare.gov. Medicare Savings Programs It bars providers from billing Part B excess charges, and it runs the Elderly Pharmaceutical Insurance Coverage program, one of the more established state pharmacy assistance programs in the country.5eCFR. Massachusetts

Massachusetts operates under a federal waiver with its own standardized Medigap plan types (Core, Supplement 1A, and Supplement 1) instead of the standard A-through-N lettering. Residents can enroll, switch, or drop a Medigap plan any month of the year, and premiums are community-rated so they don’t rise with age.6Massachusetts.gov. 2026 Massachusetts Consumer Guide to Medicare

The state has also eliminated the asset test for its Medicare Savings Programs and prohibits Part B excess charges. Its Prescription Advantage program supplements Part D drug coverage for residents who qualify.

Connecticut

Connecticut provides year-round Medigap open enrollment with community-rated premiums, so seniors keep the same pricing structure into their 80s that they had at 65.3NAIC. Honorable Mentions

California and Oregon both use a “birthday rule” that gives Medigap holders an annual window to switch plans without underwriting. In California, you have 60 days after your birthday to apply for a plan with equal or lesser benefits.7California Department of Insurance. Maine goes further in one respect: residents can switch to a Medigap plan with the same or lesser benefits at any time, as long as they haven’t had a gap in supplemental coverage of more than 90 days.9Maine.gov. Why Medigap Rules Are the Biggest Difference

Medigap policies pay costs Original Medicare leaves behind, including deductibles, copayments, and coinsurance. The federal government standardizes the plans themselves (Plan G is the same coverage everywhere), but states control pricing, enrollment windows, and consumer protections.10Centers for Medicare & Medicaid Services. Medigap – Medicare Supplement Health Insurance

Federal law gives you one six-month open enrollment window starting when you turn 65 and enroll in Part B. After that, insurers in most states can deny you coverage or charge more based on your health. That’s the trap the year-round states solve. If you buy a policy at 65 and your health declines, you may find yourself stuck in the wrong plan in most of the country. In New York, Massachusetts, or Connecticut, you can move.

Pricing structure matters too. Insurers use three methods to set Medigap premiums:

  • Community-rated: Everyone in the area pays the same premium regardless of age.
  • Issue-age-rated: Your premium is set by your age when you buy the policy and stays tied to that bracket.
  • Attained-age-rated: Your premium rises automatically as you get older. These plans often start cheapest and become the most expensive over time.

States that require or default to community rating give beneficiaries the most predictable costs over a 20- or 30-year retirement.

One more state-level protection worth knowing: Part B excess charges. When a doctor accepts Medicare but not the Medicare-approved payment amount as payment in full, they can bill you up to 15% above that amount. About eight states, including Connecticut, Massachusetts, New York, Ohio, and Pennsylvania, prohibit these charges outright. In those states, the choice between Medigap Plan G (which covers excess charges) and cheaper plans that don’t becomes less consequential.

Financial Assistance: The Asset Test Makes a Real Difference

Medicare Savings Programs help low-income beneficiaries pay premiums, deductibles, and coinsurance. Every state has to offer them, but the federal government only sets a floor. The most consequential state choice is whether to count savings and other assets when deciding who qualifies.

Under federal rules, having more than $9,950 in countable resources (or $14,910 for a married couple) disqualifies you even if your income is low.4Medicare.gov. Medicare Savings Programs That threshold blocks many retirees with modest savings from getting help. More than a dozen states have thrown out the asset test entirely for Medicare Savings Programs: New York, Connecticut, California, Massachusetts, Oregon, Maine, Alabama, Arizona, Delaware, Louisiana, Mississippi, New Mexico, Vermont, Washington, and the District of Columbia. In those places, only your monthly income counts.

A few states also push income thresholds above the federal floor. The federal Qualified Medicare Beneficiary limit sits at roughly 100% of the federal poverty level; the District of Columbia extends QMB eligibility to 300% of the federal poverty level, which is the widest expansion in the country.

The 2026 federal income limits for the two most-used programs are $1,350 per month (QMB) and $1,616 per month (SLMB) for an individual.4Medicare.gov. Medicare Savings Programs

Medicare Advantage: County Matters More Than State

If you’re leaning toward a Medicare Advantage plan instead of Original Medicare plus Medigap, the “best state” framing breaks down further. Medicare Advantage plans are offered by private insurers, and each plan sets its own service area by county. Densely populated urban areas attract more carriers, more competition, lower premiums, and richer extras like dental, vision, hearing aids, fitness memberships, and grocery allowances.11Centers for Medicare & Medicaid Services. Understanding Medicare Advantage Plans Rural counties, including many in otherwise “good” states, see fewer plan options and higher out-of-pocket maximums.

State-level averages for in-network out-of-pocket maximums range from roughly $3,900 to $8,450 against a federal ceiling of $9,250 for 2026. HMO plans dominate in western states and major metros; PPO plans have grown quickly in the South and Midwest. Two neighboring counties in the same state can offer meaningfully different plan menus, so the answer to “what’s available” comes from a zip code lookup, not a state ranking.

State Prescription Drug Assistance

Some states run their own pharmaceutical assistance programs that coordinate with Part D, paying costs the federal program leaves behind like copayments and coverage gaps.5eCFR. Subpart J – Coordination of Part D Plans With Other Prescription Drug Coverage New York’s Elderly Pharmaceutical Insurance Coverage, Massachusetts’s Prescription Advantage, and Wisconsin’s SeniorCare are established examples. Not every state has one, and eligibility rules vary where they exist.

This layer matters less than it once did because the federal $2,000 annual cap on out-of-pocket Part D spending, which took effect in 2025, now applies in every state.12Centers for Medicare & Medicaid Services. HHS Announces 15 Additional Drugs Selected for Medicare Drug Price Negotiations Still, a state pharmacy program can cut premiums and copayments before you hit that ceiling, which is real money for someone on several maintenance medications.

How to Check What’s Best Where You Actually Live

The Medicare Plan Finder at medicare.gov/plan-compare takes your zip code and returns every Medicare Advantage, Part D, and Medigap plan sold in your county, with premiums, benefits, star ratings, and provider networks. That’s the closest thing to a personal ranking you can get.

For one-on-one help, every state runs a State Health Insurance Assistance Program (SHIP) staffed by trained counselors who provide free advice on plan comparisons, financial assistance applications, and appeals.13Administration for Community Living.
