You can send Form SSA-44 to the Social Security Administration in four ways: upload it online through your my Social Security account, fax it to your local Social Security office, mail it to that office, or drop it off in person. All four routes reach the same processing staff, so pick whichever is easiest for you.1Social Security Administration. Request to Lower an Income-Related Monthly Adjustment Amount (IRMAA)
The form asks Social Security to lower the Income-Related Monthly Adjustment Amount added to your Medicare Part B and Part D premiums after a qualifying life-changing event, such as retirement, the death of a spouse, divorce, or the loss of pension income. Whichever channel you use, send the completed form together with the documents that prove the event happened.
Submit Online Through My Social Security
Sign in to your my Social Security account at ssa.gov and use the document upload feature to send your completed SSA-44 along with your supporting evidence. This is typically the fastest option because it skips mail transit and creates an immediate electronic record of what you sent and when.2Social Security Administration. Social Security Forms
Save a copy of the confirmation screen and the files you uploaded. If a question comes up later about what Social Security received, that copy is your record.
Fax or Mail to Your Local Office
Faxing and mailing both go to the Social Security field office that serves your current home address. Each office manages records for beneficiaries in its geographic area, so you need to send the form to the one that covers where you live now.
Find the address and fax number using the office locator at ssa.gov/locator. Enter your ZIP code, and the results will show the street address, mailing address, and fax number for your office.3Social Security Administration. Field Office Locator
If you mail the form, consider using certified mail with a return receipt. The stamped receipt gives you proof that the office received your documents, which matters if there is ever a dispute over whether or when the form arrived.4Social Security Administration. Submitting a Request for Reconsideration
Faxing avoids postal transit entirely and gives you a transmission confirmation from your fax machine or service. Keep that confirmation with your copy of the form.
Deliver the Form in Person
You can hand-deliver SSA-44 to your local Social Security office. The advantage is immediate confirmation: a representative can tell you the documents were received and entered into the system while you are standing there. Use the same office locator to find the street address and hours.
Whichever method you use, keep a personal copy of everything you submit.
What to Send With the Form
Social Security requires original documents or certified copies to prove your life-changing event, not plain photocopies.5Social Security Administration. Form SSA-44 – Medicare Income-Related Monthly Adjustment Amount – Life-Changing Event The specific evidence depends on the event: a certified death certificate for a spouse’s death, a certified divorce or annulment decree, a letter from a pension fund administrator, or a signed employer statement for a work stoppage. If a work stoppage document isn’t available, your signed statement on the form itself is accepted.
Along with the event documentation, you can include a copy of your federal tax return or a tax return transcript to support the income figures you reported.
What Happens After You Send It
Staff at your local office review the form, verify the life-changing event, and cross-check your reported income against IRS records. You will receive a written determination by mail explaining the decision, your new premium amounts, and the effective dates.
If you paid a higher IRMAA before the adjustment took effect, Social Security refunds the excess retroactively.6Social Security Administration. Beneficiary Questions an IRMAA Determination or Decision
If you reported an income estimate rather than a filed return, contact Social Security once you file so the agency can reconcile the figures. Skipping that step can trigger retroactive charges or refunds later, after the IRS match runs.5Social Security Administration. If Your Request Is Denied
You can appeal within 60 days of receiving the denial notice. File Form SSA-561-U2 (Request for Reconsideration) by uploading it online, mailing it to your local office, or calling Social Security at 1-800-772-1213 to start the process.7Social Security Administration. Request Reconsideration
If reconsideration is denied, further appeal levels are available: a hearing before the Office of Medicare Hearings and Appeals, then the Medicare Appeals Council, and finally federal district court. Each level carries its own 60-day filing deadline from the date of the previous denial.
