Finding Your Local SSA Field Office Address
Use the SSA’s office locator at ssa.gov/locator.2Social Security Administration. Field Office Locator Enter your zip code and the tool returns the name, mailing address, and phone number of the field office that handles your area. That address is where the form goes.
Sending the form to an IRS processing center is one of the most common mistakes, and it causes real delays. The IRS has no ability to adjust your Social Security payments. Because the form carries your Social Security number and other personal information, certified mail with a return receipt is a sensible choice. You get a delivery confirmation for your records. You can also hand-deliver the form to your local office during business hours; most offices have a service counter or a secure drop box for document submission.
Skip the Envelope: Online and Phone Options
You may not need to mail anything at all. The SSA lets you start, stop, or change federal tax withholding online at ssa.gov/manage-benefits/request-withhold-taxes.3Social Security Administration. Request to Withhold Taxes You sign in to (or create) a personal my Social Security account. The same withholding rate choices are available online as on the paper form.
If you would rather speak to someone, call the SSA at 1-800-772-1213 to request withholding changes by phone.1Internal Revenue Service. Form W-4V (Rev. Filling Out the Form So It Isn’t Rejected

Lines 1 through 3 ask for your full legal name, Social Security number, and home address. Line 4 is the claim number, and this trips people up because it is not always the same as your Social Security number. Your claim number includes a letter or letter-number suffix identifying the type of benefit you receive. A wage earner’s claim number might look like 123-45-6789A; a specific category of dependent might see 123-45-6789B6. Line 6 is where you choose your withholding rate. For Social Security benefits, the only permitted rates are:
- 7% of each monthly payment
- 10% of each monthly payment
- 12% of each monthly payment
- 22% of each monthly payment

Sign and date the form. It is not valid without a signature.1Internal Revenue Service. Form W-4V (Rev. To stop withholding entirely, complete a new Form W-4V, fill in lines 1 through 4, and check the box on line 7, which reads “I want you to stop withholding federal income tax from my payment(s).” Sign, date, and send it to the same field office.1Internal Revenue Service. When the Change Takes Effect
The SSA processes withholding changes based on when your request arrives relative to the current payment cycle. If the office receives your form early enough in the month, the change can take effect with the next payment. If it arrives after that month’s processing cutoff, it rolls to the following month. SSA’s internal procedures give an example in which a request received in late March for an April effective date had to be pushed to the May payment, because the form arrived after the monthly processing window had closed.6Social Security Administration. You can monitor status by logging in to your my Social Security account, which shows current deductions and net payment amounts.7Social Security Administration. How Can I Have Income Taxes Withheld From My Social Security Benefits? Your mailed benefit statement also confirms the withholding percentage once it takes effect. If the deduction has not appeared after two full payment cycles, call your local office to check for a processing error.
What Form W-4V Does Not Cover
Form W-4V applies only to federal income tax withholding. It does not handle state income taxes.1Internal Revenue Service. Form W-4V (Rev. January 2026) Voluntary Withholding Request If you live in a state that taxes Social Security benefits, arrange state withholding separately through your state tax agency or make state-level estimated payments.
The form also does not apply to Supplemental Security Income. SSI payments are not considered taxable income for federal purposes, so there is nothing to withhold.8Internal Revenue Service. Social Security Income If you receive both Social Security retirement or disability benefits and SSI, only the Social Security portion is eligible for voluntary withholding.
