Who Actually Receives the Form

The form goes to whoever cuts your pension or annuity check. Finding the Correct Mailing Address

The most reliable source is your most recent Form 1099-R. Every payer must issue this form annually for pension and annuity distributions, and the top-left section shows the payer’s name and full mailing address.

If your 1099-R is not handy, check a recent benefit statement or pension payment stub.   • Use your plan administrator’s secure online portal. Many payers let you upload a completed W-4P or fill in the equivalent withholding elections digitally, and some accept electronic signatures. Electronic submission typically produces an immediate confirmation and avoids mail delays.

Whichever route you take, keep a copy of the completed form and any confirmation receipt.

When Your New Withholding Takes Effect

Verify the change by checking the federal tax line on your next benefit statement or payment stub after that expected effective date. If two full payment cycles pass with no adjustment, contact your plan administrator to confirm the form was received and processed.

When to Send an Updated Form

You can submit a new W-4P any time your tax situation changes in a way that affects the right withholding amount. Common triggers include getting married or divorced, gaining or losing a dependent, starting or stopping a second pension or job, and a significant change in non-pension income such as investment or rental earnings.4Internal Revenue Service. There is no limit on how often you may update the election.

What Happens If You Never Send One

Skipping the form does not mean skipping withholding. Your payer applies default assumptions that may not match your situation.

  • If your first payment was received after 2021, the payer treats you as single with no adjustments in Steps 2 through 4. For many retirees, particularly those who are married or have dependents, this results in noticeably more tax being withheld than necessary.6Internal Revenue Service.   • If your first payment was received before 2022 and you have never filed a form, the payer continues withholding as if you were married claiming three allowances under the older system. Under that treatment, no tax is withheld if the taxable portion of your annual payments is less than roughly $2,690 per month.6Internal Revenue Service. Who Should Not Use Form W-4P

Form W-4P covers only periodic payments — installments made at regular intervals over more than one year. A separate form, Form W-4R, handles nonperiodic payments and eligible rollover distributions such as a lump-sum withdrawal.1Internal Revenue Service. About Form W-4P, Withholding Certificate for Periodic Pension or Annuity Payments If you receive both types of distributions from the same plan, you may need to send both forms to your payer.

Form W-4P also covers only federal income tax. If you live in a state with an income tax, your payer may also withhold state taxes from your pension, and a separate state form may be required. Contact your plan administrator to find out which form applies.

Keep a Copy

Retain a copy of every W-4P you submit, along with any delivery confirmation or electronic receipt.
