Where to Fax Form 3911: Fax Numbers by State and What to Include

To file a refund trace, fax IRS Form 3911 to the Refund Inquiry Unit assigned to the state listed on the return with the missing refund. The IRS splits the country into eight fax destinations, and where to fax Form 3911 depends entirely on that state, not on where you live now. Sending it to the wrong unit forces the IRS to reroute it internally and adds weeks to the process.1Internal Revenue Service. About Form 3911, Taxpayer Statement Regarding Refund

Use the State From the Return, Not Your Current State

Confirm which state appeared on the return you filed, not necessarily where you live now. If you moved after filing and the return listed your old address, use the fax number for the old state.1Internal Revenue Service. About Form 3911, Taxpayer Statement Regarding Refund

What to Include With the Fax

Send a cover sheet with your name, the words “Form 3911,” and the total number of pages. Keep the fax transmission confirmation report. That printout is your proof of the date the IRS received your request if timing ever becomes a question.

There is no online portal for Form 3911. As of the most recent IRS update in December 2025, fax and mail are the only submission methods; the IRS Document Upload Tool does not accept it.1Internal Revenue Service. About Form 3911, Taxpayer Statement Regarding Refund

Check First Whether You Need to File at All

Not every delayed refund calls for Form 3911. Start with the “Where’s My Refund” tool at irs.gov or the IRS2Go app. If it shows the refund was issued but nothing arrived, that is the trigger for a trace.2Internal Revenue Service. Refund Inquiries For a missing direct deposit, contact your bank first and give it at least two weeks to investigate before faxing anything to the IRS.3Internal Revenue Service. Refund Inquiries 18

You can also open a trace by phone instead of faxing. Call 800-829-1954 for the automated system, or 800-829-1040 to speak with a representative. The automated line does not work for joint filers; if you filed jointly, you either need to talk to a representative or submit Form 3911 by fax or mail.2Internal Revenue Service. Refund Inquiries

Joint Returns Need Both Signatures

If the missing refund came from a joint return, both spouses must sign Form 3911. The IRS will not begin the trace with only one signature. If one spouse signs, the Refund Inquiry Unit returns the form for both signatures, which resets your processing time. The IRS accepts handwritten, electronic, or digital signatures.4Internal Revenue Service. IRM 21.4.2 Refund Trace and Limited Payability

The form instructions do not address what to do when an ex-spouse refuses to sign or a spouse is unreachable. If that is your situation, calling the IRS at 800-829-1040 and explaining the circumstances to a representative is the best starting point, since the automated process has no workaround for it.

Match the Refund Amount to IRS Records

The refund amount you enter on the form has to match what the IRS has on file, not what you originally calculated. If the IRS adjusted the return, the actual refund figure may differ. Pulling your tax transcript before you fax the form is the cleanest way to confirm the right number and avoid a bounce-back.5IRS.gov. Form 3911 (Rev. 10-2022) Taxpayer Statement Regarding Refund
