Where you get a copy of your 1095 tax form depends on which version you need. Form 1095-A comes from the Health Insurance Marketplace where you bought coverage. Form 1095-B comes from your insurance company or a government program like Medicare or Medicaid. Form 1095-C comes from your employer, if that employer had 50 or more full-time equivalent workers. Figure Out Which 1095 You Need
The three forms are not interchangeable, and most people only receive one or two in a given year.
- Form 1095-A (Health Insurance Marketplace Statement) goes to anyone who bought coverage through HealthCare.gov or a state-run exchange. It feeds directly into Form 8962, the worksheet that reconciles the premium tax credit.
- Form 1095-B (Health Coverage) confirms that you had qualifying coverage. It comes from private insurers, Medicare, Medicaid, CHIP, and other coverage providers.
- Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) comes from employers with 50 or more full-time equivalent employees and reports what coverage they offered you.
If you had Marketplace coverage and also worked for a large employer during the year, expect a 1095-A and a 1095-C, and know that you may have to chase them down separately.
Getting Form 1095-A From the Marketplace
This is the form worth pursuing right away. Without it you cannot complete Form 8962, and if advance premium tax credits were paid on your behalf during the year, filing Form 8962 is required. Log in to your Marketplace account at HealthCare.gov or your state exchange’s website. Select the application for the tax year you need, not your current-year application. Choose “Tax Forms” from the menu, and under “Your Form 1095-A,” select “Download PDF.”3HealthCare.gov. Prior years work the same way: pick the application for the year you need, and the corresponding 1095-A should appear under Tax Forms.4Health Insurance Marketplace. How to Find Your Form 1095-A Online If it is not there, call the Marketplace call center at 1-800-318-2596 (TTY: 1-855-889-4325) and ask for a copy.
Once you have the form, check the monthly premium amounts in Part III against your own payment records before using any of the figures on your return.
Getting Form 1095-B From Your Insurer or Government Program
Form 1095-B comes from whoever provided your coverage. For a private insurer, log in to the member portal on the carrier’s website and look for a section labeled “Tax Documents” or “Tax Forms” where you can download a PDF. If you do not have online access, call the customer service number on your insurance card and ask for a copy to be mailed.
Medicare beneficiaries can call 1-800-MEDICARE (1-800-633-4227) to request a replacement Form 1095-B. TTY users can call 1-877-486-2048. For Medicaid, contact your state’s Medicaid agency. Many states offer an online portal where you can download tax documents, while others require a phone call to request a mailed copy.
Getting Form 1095-C From Your Employer
If you worked for an employer with 50 or more full-time equivalent employees at any point during the year, that employer is required to provide you with Form 1095-C.6Internal Revenue Service. Look in the tax statements or benefits section of whatever system your employer uses.
If you cannot find it online, contact your payroll or HR department. Starting with the 2024 reporting year, employers no longer have to automatically mail a 1095-C. They can satisfy their obligation by posting a notice on their website telling you the form is available upon request.7Internal Revenue Service. 2025 Instructions for Forms 1094-C and 1095-C In practice, you may need to ask for the form rather than wait for it.
Former employees are still entitled to a 1095-C for any month they were a full-time employee. If your portal access has expired, contact the former employer’s HR department. COBRA participants on a self-insured employer plan should also receive a 1095-C, since the employer reports COBRA enrollment in Part III of that form.6Internal Revenue Service. Why the Form May Not Arrive in the Mail
A federal law enacted in December 2024 made permanent an alternative delivery method for Forms 1095-B and 1095-C. Instead of mailing the form to every covered individual, employers and insurance providers can post a clear notice on their website stating that you may request a copy, and then furnish it within 30 days of the request.8Internal Revenue Service. If you are waiting on a 1095-B or 1095-C that never shows up, check the provider’s website first. If you see a notice about requesting tax forms, follow those instructions. If there is no notice and no form, contact the provider directly.
Form 1095-A is not affected. The Marketplace still has to send that form to you, and you should have it by mid-February.3HealthCare.gov. How to Use Form 1095-A
When Each Form Should Be Available
Forms covering the 2025 calendar year arrive or become available in early 2026, and the deadlines differ by form type.
- Form 1095-A: the Marketplace must furnish statements to individuals by January 31, 2026. It may appear in your online account as early as mid-January.9Internal Revenue Service. Instructions for Form 1095-A (2025)
- Form 1095-B: insurers and coverage providers must furnish copies by March 2, 2026, an automatic extension from the original February 2 deadline.10Internal Revenue Service. 2025 Instructions for Forms 1094-B and 1095-B
- Form 1095-C: employers must furnish copies by March 2, 2026, an automatic extension from the original January 31 deadline.7Internal Revenue Service. 2025 Instructions for Forms 1094-C and 1095-C
These are the deadlines by which the provider must make the form available. If the provider uses the website-notice method, they must furnish the form within 30 days of your request or by the deadline above, whichever is later. No additional extensions beyond March 2 are available.
What to Have Ready Before You Request a Copy
Gather a few things before you log in or call. You will need the full name and Social Security number for every person listed on the policy, including any covered spouse or dependents. For a 1095-C request, a recent pay stub or W-2 showing the employer’s Employer Identification Number can speed things up. For an insurer, your member ID number from the insurance card is the fastest way for a representative to find your record.
Confirm the tax year you need. Each calendar year has its own form. Most Marketplace and insurer portals keep several years of forms available for download, so a prior-year copy is usually still accessible online.
If the Form Has Errors
Errors on Form 1095-A are the most urgent to fix because they directly affect your return. If the monthly premiums, the second-lowest-cost Silver plan amounts, or the advance premium tax credit figures in Part III look wrong, call the Marketplace at 1-800-318-2596. The Marketplace will issue a corrected form, marked with a “Corrected” checkbox at the top.11Internal Revenue Service. For errors on Form 1095-C, contact your employer’s HR or payroll department. Employers that file a corrected 1095-C with the IRS are required to furnish you an updated copy.12Internal Revenue Service. Instructions for Forms 1094-C and 1095-C (2025)
Errors on Form 1095-B usually have less tax impact, since the form is informational. If it shows the wrong coverage months, contact the insurer for a corrected version. Accurate coverage dates matter more if you live in a state with its own health insurance mandate, where the 1095 form is your evidence of compliance.
You Can File Without Form 1095-B or 1095-C
This is where most of the confusion lives. The IRS is explicit: do not wait for Form 1095-B or 1095-C before filing, and do not attach any 1095 form to your return. Form 1095-A works differently in practice. You cannot accurately complete Form 8962 without it, and if you received advance premium tax credits and file without reconciling them, the IRS will follow up.2Internal Revenue Service.
