Form 5498 is an informational document, so you do not attach it to your tax return or file it yourself. Your IRA custodian sends one copy to you and another to the IRS. What to do with Form 5498 on your tax return is simpler than it looks: use the numbers on it to confirm your IRA deduction, track your after-tax basis, verify any rollover or conversion, and check whether you owe a required minimum distribution or a penalty for over-contributing. The form is a confirmation record, not a filing input.
Why It Usually Arrives After You File
Most people file their return before Form 5498 shows up. Custodians have until June 1, 2026 to send the 2025 form, because IRA contributions for the prior year can be made through the April 15 deadline and the form cannot be finalized until that window closes.1Internal Revenue Service. Instructions for Forms 1099-R and 5498 (2025) You will get a shorter statement earlier — by early February — covering the December 31 account value and any RMD information.
Because you already know what you contributed, file on your own records. When Form 5498 arrives, compare it against those records and against what you claimed on your return. That comparison is the whole job.
Confirm Your Traditional IRA Deduction
Box 1 reports traditional IRA contributions for the tax year, including any you made through April 15 and designated for the prior year.2Internal Revenue Service. Form 5498 IRA Contribution Information If you took a deduction, it goes on Schedule 1 (Form 1040), Line 20.3Internal Revenue Service. 2025 Schedule 1 (Form 1040) The Box 1 amount should match what you deducted, up to the limits you qualify for. If you or your spouse is covered by a workplace retirement plan, income phase-outs may reduce or eliminate the deduction even though the contribution itself is fine.
For 2026, the contribution limit is $7,500, up from $7,000 in 2025. At age 50 or older, the catch-up adds $1,100, for a total of $8,600. The catch-up amount is now indexed for inflation under the SECURE 2.0 Act.4Internal Revenue Service. 401(k) Limit Increases to $24,500 for 2026, IRA Limit Increases to $7,500
Track Non-Deductible Contributions on Form 8606
If your income is too high to deduct a traditional IRA contribution and you contribute anyway, that money goes in after tax. Report it on Form 8606 so the IRS has a record you have already paid tax on those dollars.5Internal Revenue Service. Instructions for Form 8606 (2025) Skip this and you can end up paying tax on the same money twice when you draw the account down in retirement.
The IRS instructions for Form 8606 tell taxpayers to keep copies of Form 5498 to verify the nontaxable portion of future distributions. Form 5498 is proof of what went in; Form 8606 records how much of it was after-tax. Together they establish your basis.
Verify Rollovers and Conversions
Box 2 shows amounts rolled into the IRA from another retirement plan or IRA. Box 3 shows amounts converted from a traditional or SIMPLE IRA to a Roth. Both should match a corresponding Form 1099-R issued by the sending account.6Internal Revenue Service. Instructions for Forms 1099-R and 5498 (2025) A clean rollover is a non-taxable event; a mismatch between the two forms is what triggers IRS notices, so catching it early is worth the ten minutes.
A Roth conversion is taxable in the year you do it. You calculate the taxable portion on Form 8606, which matters especially when your traditional IRA holds a mix of deductible and non-deductible contributions.
Check Box 11 for a Required Minimum Distribution
If Box 11 is checked, you must take a required minimum distribution from that IRA for the following year. In 2026, RMDs begin at age 73. The penalty for missing one is a 25% excise tax on the amount you should have withdrawn, dropping to 10% if you correct the shortfall within two years.7Internal Revenue Service. Retirement Plan and IRA Required Minimum Distributions FAQs You report the penalty, or the correction, on Form 5329.
Your custodian may fill in Box 12a with the deadline and Box 12b with the calculated RMD amount. If Box 11 is checked but Box 12b is blank, the custodian is required to either provide the amount or offer to calculate it in a separate statement.2Internal Revenue Service. Form 5498 IRA Contribution Information A blank Box 12b does not cancel the obligation. If the checkbox is marked, the distribution is due.
Watch for Excess Contributions
Contributing more than the annual limit triggers a 6% excise tax on the excess for every year it stays in the account.8Internal Revenue Service. Retirement Topics – IRA Contribution Limits The tax compounds annually. Over-contribute by $500 and leave it there, and you owe $30 the first year, another $30 the next, and so on until you pull the excess out.
To avoid the penalty, withdraw the excess plus any earnings it produced before your tax filing deadline, including extensions. The Box 1 figure on Form 5498 is what the IRS compares to your allowable limit, so if it looks too high, act before the deadline instead of waiting for a notice.
Fixing Errors on the Form
Compare every box against your own records. The frequent problems are contributions assigned to the wrong tax year, incorrect rollover or conversion amounts, and a wrong fair market value. If something is off, contact your custodian and ask for a corrected form. They are generally cooperative when the error is on their end and you catch it early.
Boxes 2 and 3 deserve the closest look. A wrong rollover amount can make a tax-free transfer look like a taxable distribution. An inflated conversion figure means more tax than you actually owe. If a custodian will not correct an error, document what you asked for and when, and bring the paperwork to a tax professional who can help you respond to any IRS notice that results.
How Long to Keep Form 5498
Keep every Form 5498 for at least three years after you file the related return, which is the IRS’s standard audit window.9Internal Revenue Service. How Long Should I Keep Records? If you underreported income by more than 25% of what your return shows, the IRS has six years. If you never filed, or filed a fraudulent return, there is no limit.
Hold the records much longer if you ever made non-deductible contributions or did a Roth conversion from a traditional IRA that held any. Keep every Form 5498 and every Form 8606 until the account is fully emptied.10Internal Revenue Service. Topic No. 305, Recordkeeping Those documents are what prove your basis and keep you from paying tax twice on the same dollars decades from now. Digital copies are fine; store them somewhere you will still have access to in 20 or 30 years.
