What to Do If You Can’t Find Your 1095-A Form

If you’ve lost your 1095-A form, a digital copy is almost certainly waiting in the HealthCare.gov or state exchange account you used to enroll, and you can download it as a PDF in a few minutes. The Marketplace is required to furnish Form 1095-A by January 31 for the prior year’s coverage, and the online version is usually available before paper copies arrive in mailboxes.1Internal Revenue Service. 2025 Instructions for Form 1095-A If you can’t get into your account, the Marketplace can re-mail it after a phone call.

Download It From Your Marketplace Account

Sign in to the same account you used to enroll. On HealthCare.gov, look for a “Tax Forms” link on your dashboard, pick the tax year, and the site will generate a PDF you can save, print, or import into tax software.2HealthCare.gov. How to Use Form 1095-A

If you enrolled through a state exchange, HealthCare.gov will not have your form. For plan year 2026, twenty-one states and the District of Columbia run their own marketplaces: California, Colorado, Connecticut, Georgia, Idaho, Illinois, Kentucky, Maine, Maryland, Massachusetts, Minnesota, Nevada, New Jersey, New Mexico, New York, Pennsylvania, Rhode Island, Vermont, Virginia, and Washington.3Centers for Medicare & Medicaid Services. Call the Marketplace for a Re-Mailing

If you cannot get into your account, call the Marketplace at 1-800-318-2596 (TTY: 1-855-889-4325). The line is open 24 hours a day, seven days a week, except federal holidays.4HealthCare.gov. Contact Us Have your full legal name, date of birth, Social Security number, and the mailing address on file ready for identity verification. Once you’re verified, a representative can trigger a new copy to be mailed. Expect it to take a couple of weeks by standard mail, and possibly longer during peak tax season.

For state-based exchanges, call your state’s marketplace customer service line rather than the federal number. The correct phone number is on your state exchange website.

What to Do If the Tax Deadline Is Close

The IRS is direct about this: if you’re expecting a Form 1095-A, wait to file until you have it.5Internal Revenue Service. Questions and Answers About Health Care Information Forms for Individuals Filing without it means you can’t complete Form 8962 to reconcile your premium tax credit, and a return missing that reconciliation can trigger processing delays or a Letter 12C from the IRS asking for the missing information.6Internal Revenue Service. If April is bearing down and the form still hasn’t shown up, file Form 4868 for an automatic extension. That pushes your filing deadline to October. An extension gives you time to file, not time to pay. If you expect to owe, estimate the amount and send payment with the extension so interest doesn’t accrue.

Check the Form Once You Have It

When your replacement 1095-A arrives, look it over before you use it. Common problems include a misspelled name, wrong coverage months, incorrect premium amounts, or a blank Second Lowest Cost Silver Plan (SLCSP) column. If any of that is wrong, contact the Marketplace and ask for a corrected version, which will arrive with the “CORRECTED” box checked at the top.7Internal Revenue Service. A blank SLCSP column is a special case. HealthCare.gov has a tax tool that will look up the correct SLCSP premium for your household and situation, and the figure the tool returns is what you put on Form 8962.8HealthCare.gov. You may also get a form with the “VOID” box checked. That happens when a 1095-A was issued in error, for example on a stand-alone dental plan or a catastrophic plan that shouldn’t have generated one. Disregard a voided form entirely.7Internal Revenue Service. Instructions for Form 1095-A

If you switched plans mid-year or different family members were on different policies, expect a separate 1095-A for each enrollment period. All of them get combined on a single Form 8962.9Internal Revenue Service. Premium Tax Credit – Claiming the Credit and Reconciling Advance Credit Payments If you don’t see a form you were expecting from one of those enrollment periods, request that one too before you file.

Why the Form Matters

Form 1095-A reports three numbers for each month of Marketplace coverage: your total monthly enrollment premium, the SLCSP premium for your area, and the advance premium tax credit paid to your insurer on your behalf.9Internal Revenue Service. Premium Tax Credit – Claiming the Credit and Reconciling Advance Credit Payments Those figures flow into Form 8962, which reconciles the advance credits you actually received during the year against the credit your final income entitles you to. If your income came in higher than what you estimated at enrollment, you may owe some of the advance credit back. Skipping the reconciliation has real costs. If you were entitled to a larger credit than what was paid in advance, you forfeit that refund by not filing Form 8962. And if you fail to file and reconcile for two consecutive years, the Marketplace can end your advance premium tax credits for future coverage, meaning you would owe the full unsubsidized premium every month until you catch up on your tax filings.11CMS: Agent and Brokers FAQ Home.
