What Is FSA Tax on Your Costco Receipt? Meaning and Reimbursement

The “FSA” line on your Costco receipt is not a tax and does not change what you pay at the register. It is Costco’s way of flagging items that qualify for reimbursement from a health care Flexible Spending Account, the employer benefit that lets you pay for certain medical expenses with pre-tax dollars. The flag is a convenience marker, nothing more.

What the FSA Line Is Doing on Your Receipt

Costco tags eligible products with an “F” or “FSA” indicator next to the item and totals the FSA-eligible amount near the bottom of the receipt. The point-of-sale system cross-references its inventory against a standardized eligible-product list and marks qualifying items automatically. This is a store feature, not a government requirement.

Nothing about the flag affects the transaction itself. Sales tax is calculated at your local rate the same way it would be for any other product. No line is added, no line is removed. The marker exists so that when you go to submit for reimbursement, you don’t have to hunt through IRS rules to figure out what qualifies.

One caveat matters. Costco has stated that it “cannot identify specific item eligibility prior to a transaction being tendered.”1Costco. What Payment Methods Are Accepted at Costco? The receipt flag is a guide. Your plan administrator makes the final call on whether any given expense qualifies.

Where the Tax Savings Actually Come From

If you sense that money is being saved somewhere, it is, but not at the checkout lane. The savings happen on your paycheck. When you enroll in a health care FSA through your employer, you choose an annual contribution amount, and that money is deducted from your paycheck before federal income tax, Social Security tax, and Medicare tax are calculated.2Internal Revenue Service. Health Savings Accounts and Other Tax-Favored Health Plans The arrangement is authorized under Section 125 of the tax code, which lets these contributions bypass your taxable wages.3Office of the Law Revision Counsel. 26 USC 125 – Cafeteria Plans

The combined savings are larger than most people assume. In the 22% federal income tax bracket, you also avoid 6.2% in Social Security tax and 1.45% in Medicare tax on each dollar contributed, for roughly 29.65% before any state income tax.4Internal Revenue Service. Social Security and Medicare Withholding Rates A $100 purchase of eligible items at Costco effectively costs about $70 in take-home pay, because those dollars were never taxed to begin with. The Social Security piece applies on earnings up to $184,500 in 2026.5Social Security Administration. Contribution and Benefit Base

None of that shows up when Costco scans a product marked “FSA.” The tax advantage lives entirely in your payroll deduction. The receipt flag only tells you which items are eligible to be paid for, or reimbursed, out of that pre-tax pool.

Using an FSA Debit Card at Costco

If your plan issues a Visa or Mastercard FSA debit card, Costco accepts it at three locations only: the Pharmacy, the Optical department, and the Hearing Aid department.1Costco. What Payment Methods Are Accepted at Costco? The main warehouse registers do not process FSA debit cards, even when the item is FSA-eligible. This catches many shoppers off guard: they pick up allergy medicine or bandages from the warehouse floor and expect to swipe the FSA card up front.

For eligible items bought at the main registers, you pay with a regular method and then submit the receipt to your FSA administrator for reimbursement. At the Pharmacy counter the process is more direct. Costco participates in the Inventory Information Approval System (IIAS), which verifies item eligibility at the point of sale when the FSA card is swiped.6SIGIS. Merchants The system checks each scanned item against a standardized product list and charges the FSA card only for qualifying purchases, which often removes the need to submit a separate receipt for that transaction.

Submitting the Receipt for Reimbursement

When you pay out of pocket for eligible items, the Costco receipt with FSA markings is your proof of purchase. Most plan administrators want you to upload a scan or photo through an online portal or app. The receipt needs to show the merchant name, the date, a description of each item, and the amount paid. Some administrators also require the patient’s name on the receipt or a separate written statement identifying who the items are for.

The FSA flags speed up review because eligible items are easy to spot, but they do not guarantee approval. Your plan administrator applies its own rules, and some plans use narrower lists than the IRS allows. If you lose the paper receipt, Costco members can reprint purchase records at the membership desk or view them online.

What Counts as FSA-Eligible at Costco

Eligibility is governed by IRS Publication 502, which defines qualifying medical expenses under Section 213(d) of the tax code.7Internal Revenue Service. Publication 502 – Medical and Dental Expenses The categories you’re most likely to see flagged at Costco include:

  • Over-the-counter medications such as pain relievers, allergy medicine, antacids, and cold remedies. The CARES Act permanently removed the doctor’s prescription requirement for OTC medications, effective January 1, 2020.8Internal Revenue Service. • Menstrual care products including tampons, pads, liners, and cups, also made permanently eligible under the CARES Act.8Internal Revenue Service. IRS Outlines Changes to Health Care Spending Available Under CARES Act
  • First aid and diagnostic supplies: bandages, thermometers, blood pressure monitors, and home diagnostic kits.
  • Sunscreen with SPF 15 or higher, which generally qualifies because it serves a medical purpose.
  • Prescription eyewear and contact lenses through Costco Optical.
  • Hearing aids and their batteries through Costco’s Hearing Aid department.

General toiletries, cosmetics, vitamins marketed for general wellness, and nutritional supplements typically do not qualify. The line can feel arbitrary. An SPF moisturizer might be eligible while a face wash containing the same SPF is not, which is exactly why the receipt flag is useful.

If an Item Wasn’t Really Eligible

The consequences here are administrative, not punitive at the register. If you submit a non-eligible item for reimbursement and it slips past your administrator, you are responsible for correcting it. Typically the administrator flags the expense during a later review and asks you either to substitute a valid receipt for another eligible expense or to repay the amount. If you do neither, the disbursement gets added back to your taxable income for that year.

FSAs do not carry the same explicit penalty structure that applies to Health Savings Accounts, where non-qualified distributions trigger a 20% additional tax. Denied claims, repayment demands, and potential loss of card auto-pay privileges are the practical risks. Intentional fraud falls under general tax fraud provisions and could draw IRS scrutiny, but an accidental tube of toothpaste is not going to trigger an audit. The far more common way to lose money with an FSA is forfeiting an unspent balance at the end of the plan year, so the receipt flag is worth paying attention to while you still have funds to use.
