What Is Form 1040-V? Filling Out and Mailing the Voucher

Form 1040-V is the IRS payment voucher you send with a check or money order when you owe a balance on your federal income tax return.1Internal Revenue Service. Form 1040-V (2025) Payment Voucher for Individuals It works with Form 1040, Form 1040-SR, and Form 1040-NR. If you pay electronically, you don’t need it. If you’re mailing a paper payment, the voucher is a scannable cover sheet that helps the IRS credit the money to the right account.

When You Actually Need Form 1040-V

You only need this voucher if you’re paying by check or money order through the mail. The IRS offers several free or low-cost electronic options that process faster and give you immediate confirmation.

Using the voucher isn’t legally required, but skipping it when you mail a check raises the odds that your payment sits unprocessed or gets applied to the wrong account. Either can trigger a failure-to-pay penalty even though you sent the money on time.

How to Fill Out the Voucher

Download the current form from IRS.gov rather than photocopying an old one. The scanning equipment expects a specific layout. The form has three numbered lines and an address block.1Internal Revenue Service. Form 1040-V (2025) Payment Voucher for Individuals

  • Line 1: your Social Security number. On a joint return, use the SSN of the person listed first.
  • Line 2: your name and address exactly as they appear on your tax return.
  • Line 3: the payment amount in dollars and cents.

If you have a foreign address, complete the extra fields for country, province or state, and foreign postal code. Permanent residents of Guam or the U.S. Virgin Islands should not use Form 1040-V.6Internal Revenue Service. Where to File Addresses for Taxpayers Filing Form 1040-V

How to Prepare the Check

Make the check or money order payable to “United States Treasury.” Not “IRS,” not any individual.1Internal Revenue Service. Form 1040-V (2025) Payment Voucher for Individuals Then write the following directly on the check so the IRS can still credit it if the voucher gets separated during processing:

  • Your SSN (the first one listed on a joint return).
  • A daytime phone number.
  • The tax year and form, such as “2025 Form 1040.”

Enter the amount on the right side of the check as $XXX.XX. It must match Line 3 of the voucher.

Where to Mail Form 1040-V

The address depends on where you live.1Internal Revenue Service. Form 1040-V (2025) Payment Voucher for Individuals

  • Alabama, Florida, Georgia, Louisiana, Mississippi, North Carolina, South Carolina, Tennessee, and Texas: Internal Revenue Service, P.O. Box 1214, Charlotte, NC 28201-1214.
  • All other U.S. states and the District of Columbia: Internal Revenue Service, P.O. Box 931000, Louisville, KY 40293-1000.

Place the voucher and check loosely in the envelope. No staples, paper clips, or tape. Fasteners damage the high-speed scanners at IRS processing centers.

Payment Deadline and Late Penalties

Your payment is due by the filing deadline. For 2025 returns, that’s April 15, 2026.7Internal Revenue Service. Pay Taxes on Time An extension of time to file is not an extension of time to pay.

Miss the deadline and two penalties can stack:

  • Failure to pay: 0.5% of the unpaid balance per month or partial month, capped at 25%.8Office of the Law Revision Counsel. 26 USC 6651 – Failure to File Tax Return or to Pay Tax
  • Failure to file: 5% of the unpaid tax per month, also capped at 25%. When both apply in the same month, the failure-to-file penalty is reduced by the failure-to-pay amount for that month.9Internal Revenue Service. Failure to File Penalty

Interest compounds daily on top of that. The individual underpayment rate is 7% per year for the first quarter of 2026,10Internal Revenue Service. Interest Rates Remain the Same for the First Quarter of 2026 dropping to 6% starting April 1, 2026.11Internal Revenue Service. If You Can’t Pay the Full Amount

You can still use Form 1040-V to send a partial payment, and doing so reduces the base that penalties and interest apply to. For the remaining balance, the IRS offers several arrangements:13Internal Revenue Service. Payment Plans – Installment Agreements

  • Short-term payment plan of 180 days or less: no setup fee, regardless of how you apply.
  • Long-term installment agreement with direct debit: $22 setup fee online, or $107 by phone or mail.
  • Long-term installment agreement without direct debit: $69 setup fee online, or $178 by phone or mail.
  • Low-income taxpayers: setup fees are waived for direct debit agreements and reduced to $43 otherwise.

While an installment agreement is in effect, the failure-to-pay penalty rate falls from 0.5% to 0.25% per month.12Internal Revenue Service. Failure to Pay Penalty

Proving You Paid on Time and Tracking the Payment

How you send a check near the deadline matters. Under 26 U.S.C. ยง 7502, a payment postmarked by the due date counts as paid on time even if the IRS receives it days later.14Office of the Law Revision Counsel. 26 USC 7502 – Timely Mailing Treated as Timely Filing and Paying USPS registered or certified mail gives you the strongest proof: the registration or certification date is treated as the postmark and serves as legal evidence of delivery. Certain IRS-designated private delivery services from carriers like FedEx and UPS also qualify.

Keep a copy of the completed voucher, your mailing receipt, and any tracking confirmation. If the payment is ever lost or delayed, those records are what protect you.

Wait at least two weeks before checking on the payment. Look at your bank first to see whether the check has cleared. If it hasn’t, call the IRS at 800-829-1040 to ask whether it has been credited to your account.15Internal Revenue Service.
