What Is a TIN Number for Healthcare Providers?

A Taxpayer Identification Number for healthcare providers is the nine-digit IRS number that links you or your practice to every tax filing and insurance claim. If you practice solo under your own name, your Social Security Number is your TIN. If your practice is a corporation, LLC, partnership, or has employees, it needs its own Employer Identification Number (EIN). Federal law requires this number on every tax return, claim form, and insurance contract you touch.1Office of the Law Revision Counsel. 26 USC 6109 – Identifying Numbers

Which TIN Applies to Your Practice

The right number depends on how your practice is legally structured. A solo practitioner with no employees uses their Social Security Number, which federal law designates as the identifying number for individuals on tax documents.1Office of the Law Revision Counsel. 26 USC 6109 – Identifying Numbers Professional income flows directly to the individual tax return.

Once the practice is organized as a separate legal entity, such as a professional corporation, LLC, or partnership, it needs its own EIN. The IRS treats the entity as a distinct taxpayer because it earns income and pays employees independently of any one person.2Internal Revenue Service. Instructions for Form SS-4 Even a sole proprietor who hires staff or sets up a qualified retirement plan must obtain an EIN, because the practice now has payroll reporting obligations separate from the owner’s personal return.3Centers for Medicare and Medicaid Services. Many providers hold both numbers: a personal TIN (their Social Security Number) and an entity TIN (their practice’s EIN). Which one you put on a given document depends on whether you are acting as an individual, for example receiving personal 1099 income, or billing through the practice entity.

How a TIN Differs From an NPI

Providers often mix up the TIN with the National Provider Identifier. They serve different purposes and come from different agencies. The TIN is a tax number issued by the IRS. The NPI is a 10-digit clinical identifier issued by CMS under federal regulations requiring a standard unique identifier for every healthcare provider.4eCFR. 45 CFR 162.406 – Standard Unique Health Identifier for Health Care Providers

The TIN identifies who gets paid and who owes tax. It appears in Box 25 of the CMS-1500 claim form, on W-9 forms, and on tax returns. The NPI identifies who provided the clinical service. It appears in Box 33a of the CMS-1500, on prescriptions, and on referrals.

Both numbers appear on most insurance claims, and they have to correspond. If the TIN in Box 25 does not match the billing provider’s NPI in Box 33a, the payer cannot confirm that the entity requesting payment is the same one that rendered the service, and the claim may be delayed or denied.5Centers for Medicare and Medicaid Services. Medicare Claims Processing Manual – Chapter 26 You apply for an NPI through the National Plan and Provider Enumeration System, which is a separate process from getting a TIN.6HHS.gov. How to Get an EIN

You apply for an EIN using Form SS-4. The IRS collects the same information no matter which submission method you choose.7Internal Revenue Service. About Form SS-4, Application for Employer Identification Number Have these details ready before you start:

  • The practice’s exact legal name as registered with your state’s Secretary of State, plus any trade name.
  • The entity type: sole proprietorship, partnership, corporation, LLC, or another category.8Internal Revenue Service. Form SS-4, Application for Employer Identification Number
  • The responsible party: the individual who controls or manages the entity and its assets. This person provides their Social Security Number or Individual Taxpayer Identification Number so the IRS can tie accountability to a real person.8Internal Revenue Service. • The reason for applying (starting a new practice, hiring employees, or forming a new entity).
  • The physical business address and the date the practice began, or will begin, operating.

Online

The IRS online EIN application is the fastest route. It is free and issues your number immediately on approval. The tool runs an interview-style questionnaire, validates your entries, and generates a confirmation notice you can print right away. It is available Monday through Friday from 6:00 a.m. to 1:00 a.m. (next day), Saturday from 6:00 a.m. to 9:00 p.m., and Sunday from 6:00 p.m. to midnight, all Eastern Time.9Internal Revenue Service. Get an Employer Identification Number You have to complete it in one session. The application expires after 15 minutes of inactivity and cannot be saved.

Fax or Mail

A faxed Form SS-4 comes back with an EIN in about four business days.2Internal Revenue Service. Instructions for Form SS-4 Mail is the slowest option, at roughly four to five weeks. The IRS lists the current fax numbers and mailing address by state.10Internal Revenue Service. Where to File Your Taxes for Form SS-4

Whichever method you use, keep the confirmation letter the IRS sends. You will need it for banking, insurance credentialing, and Medicare enrollment.

Where Your TIN Shows Up in Billing and Payer Paperwork

Your TIN appears on virtually every insurance claim your practice submits. On the CMS-1500 form used for outpatient and professional services, the billing provider’s EIN or Social Security Number goes in Box 25. Payers use that tax identification, tied to the correct NPI, to route reimbursement. Claims submitted without it may be delayed.5Centers for Medicare and Medicaid Services. Medicare Claims Processing Manual – Chapter 26

Accuracy matters more than many providers realize. The IRS operates a TIN Matching Program that lets payers check a provider’s name-and-TIN combination against IRS records before filing information returns. The tool verifies up to 25 combinations instantly, or up to 100,000 in bulk within 24 hours.11Internal Revenue Service. Taxpayer Identification Number Matching Tools A mismatch caused by a misspelling, a name change, or an outdated number can freeze payment until the discrepancy is resolved.

W-9 and Backup Withholding

Every time you join a new insurance network, the payer will ask you to complete IRS Form W-9 to certify your TIN. The payer needs the number to report what it paid you during the year, typically on Form 1099-NEC.12Internal Revenue Service. Form W-9, Request for Taxpayer Identification Number and Certification

If you fail to provide a valid TIN on the W-9, or the number does not match IRS records, the payer must begin backup withholding. That means 24% of every payment owed to you is deducted and sent to the IRS.12Internal Revenue Service. Form W-9, Request for Taxpayer Identification Number and Certification You can claim a credit for those amounts when you file, but the cash-flow hit while you wait can be significant.

When You Need a New EIN

Certain changes to your practice’s legal structure require an entirely new EIN rather than an update to your existing one. The IRS lists these triggers:13Internal Revenue Service. When to Get a New EIN

  • A sole proprietor incorporates or forms a partnership. The new entity needs its own EIN.
  • A corporation changes structure by merging into a new corporation, converting to a partnership or sole proprietorship, or receiving a new state charter.
  • A partnership ends and the partners start a new one, or one partner takes over as a sole proprietor.
  • An LLC terminates and reforms as a corporation or partnership.

A corporation that only changes its name or state of incorporation, without changing its underlying business structure, keeps its existing EIN.13Internal Revenue Service. When to Get a New EIN Using the wrong EIN on claims and tax returns creates problems that can take months to unwind, so check the IRS guidance before reorganizing.

Keeping Records Current

Even when the structure stays the same, some changes still have to be reported. If the responsible party on your EIN application changes, for example because a founding partner retires and another physician takes over, you must notify the IRS within 60 days on Form 8822-B. The same form covers a change of business mailing or physical address.14Internal Revenue Service. About Form 8822-B, Change of Address or Responsible Party – Business

Updating the IRS is only step one. Notify every insurance payer, credentialing organization, and state licensing board that has your TIN on file. Mismatches between IRS records and payer records can trigger backup withholding, claim denials, or delayed reimbursement.

Using an EIN to Keep Your SSN Off Forms

Sole proprietors who bill under their Social Security Number expose it on every claim form, W-9, and credentialing application they submit. Each disclosure adds identity-theft risk. Getting an EIN, even when you are not required to have one, lets you use that number on billing forms and insurance paperwork instead.

Box 25 of the CMS-1500 accepts either an EIN or a Social Security Number, so sole proprietors can choose to bill with the EIN.5Centers for Medicare and Medicaid Services. Medicare Claims Processing Manual – Chapter 26 Your Social Security Number is still required on your personal tax return; an EIN does not replace it there.2Internal Revenue Service. Instructions for Form SS-4 Cutting the number of places your SSN circulates is still a practical step.

Providers Without SSN Eligibility

Healthcare professionals who are not U.S. citizens or permanent residents and are not eligible for a Social Security Number may still need a taxpayer identifier. The IRS issues Individual Taxpayer Identification Numbers (ITINs) for this purpose. You apply on Form W-7 and must submit original documents, or certified copies, establishing both your identity and your connection to a foreign country.15Internal Revenue Service. Instructions for Form W-7

A valid passport is the only single document that satisfies both the identity and foreign-status requirements. Without one, you need at least two other qualifying documents, such as a national identification card and a foreign driver’s license, and at least one must include a photograph.15Internal Revenue Service. Instructions for Form W-7 Common reasons to apply include filing a U.S. tax return on income earned here or claiming reduced withholding under a tax treaty.
