What Is a Loan Verification Form and How Does It Work?

A loan verification form is a document your lender sends to a third party โ€” your employer, your bank, a prior mortgage servicer, your landlord, or the IRS โ€” to independently confirm the income, assets, employment, or payment history you listed on your loan application. Instead of trusting the numbers you wrote down, the lender goes to the source. What comes back on those forms decides whether you’re approved, how much you can borrow, and the rate you’ll pay.

Federal law is the reason this step exists at all. Under Regulation Z’s ability-to-repay rule, a mortgage lender cannot approve a loan without a reasonable, good-faith determination that you can afford the payments, and it must verify your income or assets using “reasonably reliable third-party records.”1eCFR. 12 CFR 1026.43 – Minimum Standards for Transactions Secured by a Dwelling Verification forms are the primary way lenders satisfy that obligation.

The Forms Lenders Actually Use

Each form targets a different slice of your finances. Knowing which ones apply to you tells you who your lender will contact and what documents to have ready.

Verification of Employment (Form 1005)

Built around Fannie Mae’s standardized Form 1005, this form confirms your job status, tenure, and pay. Your lender sends it directly to your employer’s payroll or HR department, which fills in your base pay, overtime, commissions, and bonuses, and states whether your continued employment is expected.2Fannie Mae. Lenders pair this with your most recent pay stub (dated no earlier than 30 days before your application) and one or two years of W-2s. A year-end pay stub showing full-year earnings can substitute for a W-2, and the documents must clearly identify your employer and be computer-generated or typed rather than handwritten.3Fannie Mae. Verification of Deposit (Form 1006)

A Verification of Deposit, Fannie Mae Form 1006, proves you have the cash to close and to cover any required reserves. It goes to your bank, which reports account type, current balance, average balance, and account status. Some banks charge a fee to complete it.4Fannie Mae. Verification of Deposits and Assets

For a purchase, asset documentation must cover the two most recent full months of activity. A refinance requires one. If your most recent statement is more than 45 days old at application, expect a request for a supplemental bank-generated document showing your current balance.4Fannie Mae. Large deposits get scrutinized. Any single deposit greater than 50% of your total monthly qualifying income on a purchase transaction requires documentation of the source. A clearly labeled direct deposit or tax refund is fine; an unexplained cash deposit needs a written explanation and supporting records.5Fannie Mae. Verification of Mortgage

If you already own or have owned a home, lenders request a Verification of Mortgage from the current or prior servicer to capture 12 to 24 months of payment history. On an FHA loan, three or more payments over 30 days late within the prior 12 months triggers a downgrade to manual underwriting and a much more intensive review of your file.6U.S. Department of Housing and Urban Development. Verification of Rent

First-time buyers without a mortgage history can have rental payments verified instead. Lenders look for at least 12 consecutive months of housing payments, documented through direct verification by your landlord, canceled checks, bank statements showing the payments, or copies of money orders. IRS Tax Transcript (Form 4506-C)

Most mortgage lenders also pull your tax transcripts directly from the IRS to confirm your application matches what you actually reported. You authorize this by signing Form 4506-C, which the lender submits through the IRS’s Income Verification Express Service.8Internal Revenue Service. If your application says you earned $95,000 last year but your return shows $72,000, this is where the gap surfaces. A significant mismatch can end the approval.

If You’re Self-Employed

Without an employer to verify income, lenders lean on your tax returns. The standard requirement is two years of signed personal returns with all schedules; depending on your business structure, business returns may be required too. If your business has operated at least five years and you’ve held 25% or more ownership throughout, one year of returns may suffice. Either way, the underwriter runs a cash-flow analysis that converts your gross revenue, expenses, and deductions into a usable net income figure โ€” and the deductions that lower your tax bill also lower the income a lender can count.10Fannie Mae. What You Sign to Make Verification Possible

None of this happens without your authorization. Your bank and employer are prohibited from releasing your records to a third party until you’ve given written permission. Many lenders also ask you to sign Form SSA-89, which authorizes the Social Security Administration to confirm that your name, SSN, and date of birth match its records. The authorization runs 90 days from signature unless you specify otherwise, and the form warns that a false representation to obtain information from Social Security records can bring a misdemeanor conviction and a fine up to $5,000.13Social Security Administration. Before you sign anything, pull together your Social Security number, current and previous addresses, employer contact information, and account numbers. Incomplete information gives lenders a reason to bounce your package back.14Consumer Financial Protection Bureau. Create a Loan Application Packet

How the Verifications Come Back Together

Once you’ve signed, the lender takes over. Forms go out by secure channel, and completed responses typically come back within a few business days. Some lenders skip the paper VOE and pull your data from The Work Number, an Equifax-run database that aggregates employment and payroll records from millions of employers.15The Work Number. You also don’t always need a formal 1005 or 1006. Bank statements can replace a Verification of Deposit if they clearly identify the institution, list you as the account holder, include account numbers, cover the required period, and show all transactions and ending balances.4Fannie Mae. When results come in, an underwriter compares them line by line against your application. Small differences between a pay stub and a VOE are normal. Significant discrepancies trigger deeper scrutiny, additional documentation requests, or a “conditional approval” that requires you to resolve specific issues before final sign-off.

If the Verification Sinks Your Application

When verification results lead to a denial, Regulation B requires the lender to send a written adverse action notice within 30 days of having enough information to decide. The notice must state the specific reasons for the denial or tell you that you have 60 days to request them in writing, and it must identify the federal agency that oversees the lender.17Consumer Financial Protection Bureau. 12 CFR 1002.9 – Notifications

One detail worth knowing: if your application was incomplete but the lender had enough information to evaluate it anyway, “incomplete application” is not a legitimate reason for denial. The Cost of Lying on a Verification Form

Fudging the numbers is a federal crime. Under 18 U.S.C. ยง 1014, knowingly making a false statement or inflating the value of property to influence a federally related mortgage lender carries up to 30 years in federal prison, a fine up to $1,000,000, or both. Prosecution doesn’t require that the loan was approved. The crime is making the false statement to influence the decision, whatever the outcome. A conviction can also bring restitution, supervised release, and forfeiture. Even inflating your income by a few thousand dollars on a verification form falls inside the statute if done knowingly.
