A mistake on a passport application is usually fixable, and the right fix depends on when you spot it. If you catch it before mailing, throw the form out and start a clean one. If your application is already in processing, you’ll either wait for the State Department to send a suspension letter asking you to correct the problem, or respond to one you’ve already received. If the passport has already been printed with an error, Form DS-5504 handles corrections without a new application fee. What you should not do is cross out, white out, or write over anything on a form you plan to send in.
If You Haven’t Submitted Yet
Start over. Don’t cross anything out, don’t use correction fluid, and don’t scribble in the margins. Processing agents read thousands of these forms, and any alteration makes yours harder to process and more likely to be flagged.
Fill out a fresh form using the State Department’s online Form Filler, which lets you complete the application on a computer and print it on single-sided paper. Double-sided printouts are not accepted.1U.S. Department of State. Apply for a Child’s Passport Under 16 Handwriting corrections onto a form you filled out with Form Filler will also delay processing, so if you need to change anything after printing, print a new copy. Use black ink only.
Before sealing the envelope, lay out every document the application calls for and check each one: citizenship evidence with the original seal, a photocopy of your ID (front and back), a compliant 2×2 inch photo taken within the last six months, and the correct payment made out to the right entity. A few minutes here can save weeks later.
If Your Application Is Already Being Processed
Once your application is in the mail or handed off at an acceptance facility, you can’t pull it back. Check the Online Passport Status System at passportstatus.state.gov using your last name, date of birth, and the last four digits of your Social Security number. If the status shows “Additional Information Needed,” the State Department has already found a problem and is waiting on you.
For direct help, call the National Passport Information Center at 1-877-487-2778. Representatives take calls Monday through Friday from 8:00 a.m. to 10:00 p.m. Eastern Time, and Saturday and Sunday from 10:00 a.m. to 3:00 p.m. Eastern Time.2U.S. Department of State. If You Received a Suspension Letter
When the State Department finds a problem, it puts the application on hold and sends you a letter or email explaining what’s wrong. The letter might ask for a corrected document, a new photo, missing fees, or clarification about inconsistent information. You have 90 days from the date of that letter to respond.3U.S. Department of State. Respond to a Letter or Email
That deadline is not flexible. If you don’t respond in time, the State Department treats the application as abandoned. You lose the fees you already paid and start the entire process over with a new application and new payment. Read the letter carefully; it tells you exactly how to respond, whether by email, mail, or phone.
If the Passport Arrived With an Error
Sometimes the mistake is on the State Department’s end: your name is misprinted, your date of birth is wrong, or another data field doesn’t match your documents. Sometimes you legally changed your name shortly after receiving a new passport. Form DS-5504 handles both, without charging you the full application fee again.
You can use DS-5504 if:
- The passport contains a printing or data error. Corrections are free, including expedited processing, as long as the passport is still valid.
- You changed your name by marriage or court order, and the passport was issued less than one year ago. No application fee is charged, though you’ll pay separately if you want expedited service or faster delivery.
- Your passport was limited to two years or less because you couldn’t fully prove citizenship or identity at the time of issue.
Send in your current passport, a new 2×2 inch color photo, and a document supporting the correction, such as a marriage certificate for a name change or a corrected birth certificate for a data error.4U.S. Department of State. If Your Application Was Rejected Outright
A full rejection means starting fresh with Form DS-11. Read the rejection letter closely; it spells out what went wrong and what to fix. Common reasons include insufficient citizenship evidence, an unacceptable photo, or missing parental consent for a child’s application.5U.S. Department of State. Passport Forms
For photo-only rejections, you generally have 90 days from the date on the letter to submit a new compliant photo. Miss that window and the application is voided; you’ll reapply with a new form and new fees. When you resubmit, include the original rejection letter along with the corrected materials, and send everything to the address the letter specifies.

Life-or-death emergencies follow a separate track. If an immediate family member abroad has died, is dying, or has a life-threatening illness or injury, you can get emergency service on weekends and holidays. Immediate family means a parent, child, spouse, sibling, or grandparent. You’ll need documentation of the emergency, such as a hospital letter on official letterhead signed by a doctor, plus proof of upcoming foreign travel. What Ignoring the Mistake Costs
A first-time adult passport book costs $165 total: a $130 application fee plus a $35 acceptance facility fee. A child’s passport book runs $135. Expedited processing adds $60, and 1-to-3-day delivery adds $22.05.8U.S. Department of State. Passport Fees
If your application is rejected or abandoned, the acceptance facility fee is gone. Federal regulations state that the execution fee is not refundable.9eCFR. 22 CFR Part 51 Subpart D – Fees The application fee is also not refundable except in narrow situations, such as when the fees were collected from someone legally exempt from paying them. The expedited processing fee is refunded only when the State Department itself misses the promised timeframe. In practice, a rejected application means paying most of the fees a second time.
Time is the other cost. Routine processing currently runs four to six weeks, and expedited processing runs two to three weeks. Mailing time can add up to two weeks in each direction on top of that.10U.S. Department of State. Processing Times for U.S. Passports A suspension you have to respond to, followed by reprocessing, can easily stretch to two or three months.
Traveling with a passport that contains errors has its own risks. A name that doesn’t match your airline ticket can get you turned away at the gate, and border agents in some countries will refuse entry when passport details don’t line up with other travel documents.
Honest Mistakes Are Not Fraud
Correcting an honest error through the channels above is routine government business. It’s different from knowingly putting false information on the application, which is a federal crime under 18 U.S.C. ยง 1542. Penalties include up to 10 years in prison for a first or second offense, up to 15 years for repeat offenses, up to 20 years when the offense connects to drug trafficking, and up to 25 years when it connects to international terrorism. These penalties apply to making the false statement and to using a passport obtained through fraud.11Office of the Law Revision Counsel. 18 USC 1542 – False Statement in Application and Use of Passport Writing the wrong zip code or misspelling a middle name is not what the statute is aimed at. Fixing it through DS-5504, a suspension response, or a resubmission is the answer.
