Disability income is reported on whichever form the payer is required to issue: a W-2 if the benefit comes from your employer or its insurer, an SSA-1099 for Social Security Disability Insurance, a 1099-R when the payment comes from a pension or annuity, and an RRB-1099 for railroad disability. Some disability payments generate no form at all because they aren’t taxable. So the question of what form disability income is reported on has more than one right answer, and the source of the payment decides it.
Disability Payments That Don’t Generate a Tax Form
Before hunting for a missing form, check whether one should exist. Several categories of disability income are excluded from federal gross income, and payers don’t issue tax forms for them.

- VA disability compensation and pension payments, along with VA grants for wheelchair-accessible homes and adapted vehicles, are not included in gross income.2Internal Revenue Service. Veterans Tax Information and Services

- Benefits from a private disability policy whose premiums you paid entirely with after-tax dollars are not taxable income.4Internal Revenue Service. Life Insurance and Disability Insurance Proceeds
If your only disability income falls in these categories, no form is coming and none is needed.
The Form You Receive by Source
Form W-2 for Employer-Sponsored Disability
When your employer pays disability benefits directly, or through a third-party insurer acting on the employer’s behalf, the payments are reported on Form W-2. Federal law requires the statement by January 31 of the year after payment.5Office of the Law Revision Counsel. 26 US Code 6051 – Receipts for Employees The taxable disability amount sits in Box 1 with your other wages. If a third-party insurer issued the payments, Box 13 will have the “Third-party sick pay” box checked. Box 12 sometimes carries code “J” for non-taxable sick pay, which you should not add to your income total. Box 4 shows any federal tax already withheld.
Form SSA-1099 for Social Security Disability
The Social Security Administration mails Form SSA-1099 each January to everyone who received SSDI during the prior year.6Social Security Administration. Form 1099-R for Pension-Based Disability
Disability benefits paid from a pension or annuity plan come on Form 1099-R. Box 1 shows the gross distribution and Box 2a shows the taxable portion. If the payer couldn’t determine the taxable amount, Box 2b will be checked and you’ll calculate it using the General Rule or Simplified Method from IRS Publication 575.8Internal Revenue Service. Form RRB-1099 for Railroad Disability

Same Form, Different Tax Result: The Premium Question
Receiving a W-2 for disability payments doesn’t automatically mean the full amount is taxable, and it’s worth understanding why before you copy numbers onto your return. The taxability of employer-related disability benefits comes down to who paid the premiums and how. If your employer paid, or if you paid through a pre-tax cafeteria plan, the benefits are fully taxable. If you paid every premium with after-tax dollars, they’re tax-free. The cafeteria plan piece trips people up. A deduction on your pay stub feels like you’re paying for coverage, but if it was pre-tax, the IRS treats it as employer-paid. Verify the setup with your HR department before you file, because it can change whether any of the benefit is taxable at all.
Where the Numbers Go on Form 1040
Once the form arrives, the source dictates the line. The IRS already has copies of every W-2, SSA-1099, and 1099-R sent to you, and mismatches generate automated notices.
Employer Disability Payments
Taxable disability payments from an employer plan go on Line 1 of Form 1040 as wages, but only until you reach your plan’s minimum retirement age — the earliest age you could have started a regular pension if you weren’t disabled. SSDI Benefits
Enter the total from Box 5 of Form SSA-1099 on Line 6a. The taxable portion goes on Line 6b.3Internal Revenue Service. - Single filers with combined income between $25,000 and $34,000: up to 50% of benefits may be taxable.
- Single filers with combined income above $34,000: up to 85% of benefits may be taxable.
- Joint filers with combined income between $32,000 and $44,000: up to 50% of benefits may be taxable.

Below the first threshold for your filing status, SSDI benefits are entirely tax-free.
Pension-Based Disability

If a Form Is Missing or Wrong
If January 31 passes without your W-2 or 1099-R, contact the payer first.
