A WEX benefits card covers whatever qualified expenses sit inside the benefit accounts your employer has loaded onto it. That usually means one or more of the following: a Health Savings Account, a medical Flexible Spending Account, a Health Reimbursement Arrangement, a Dependent Care FSA, a commuter benefits account, or a lifestyle spending account. Each account has its own rulebook, and the card enforces those rules at the register.
How One Card Handles Several Accounts
WEX calls the product a “stacked” benefits debit card. Every account linked to it sits in a separate “purse,” and when you swipe, the system decides which purse to pull from based on the merchant type and the items being bought.1WEX Inc. Modern Consumer Driving Need for Benefits Debit Card At retailers that use an Inventory Information Approval System, the terminal identifies eligible items line by line and approves them with no paperwork afterward. About 85 percent of WEX card purchases clear this way.2WEX Inc. Why FSA Substantiation Matters When a merchant cannot verify electronically, you may be asked to upload an itemized receipt through the WEX app or online account to prove the purchase qualified.
Medical FSA and HSA Expenses
A general-purpose medical FSA and an HSA both key off the same IRS definition in Section 213(d): costs for the “diagnosis, cure, mitigation, treatment, or prevention of disease, and for the purpose of affecting any part or function of the body.”3IRS. Publication 502: Medical and Dental Expenses WEX’s own eligible-expense database does not flag anything as covered by an FSA but excluded from an HSA.4WEX Inc. Eligible Expenses
In practice, the card works for:
- Doctor visits, hospital bills, insurance copayments, and deductibles
- Prescription drugs and medical devices
- Dental care including cleanings, fillings, crowns, root canals, braces, and implants
- Vision care including prescription eyeglasses, contact lenses, LASIK, and cataract surgery5WEX Inc. HSA Medical FSA Dental Expense Eligibility6WEX Inc. Vision Expenses HSA FSA

- Fertility treatments such as IVF, fertility medications, and temporary egg or sperm storage (surrogacy and long-term fertility storage are not eligible)8WEX Inc. Fertility Benefit Needs With HSA FSA
There is one meaningful gap between the two accounts. An HSA can pay COBRA premiums with the card as long as the balance is there; the same swipe funded by an FSA will be declined.9WEX Benefits Knowledge Base. Paying for COBRA Premiums With the WEX Benefits Card Life insurance premiums are not eligible from either account, even when packaged with COBRA.
WEX also administers two narrower FSA variants. A Limited Medical FSA, usually paired with an HSA, covers only qualifying dental, vision, and preventive care. A Combination (post-deductible) FSA starts as vision and dental only, then opens up to full medical FSA coverage once you meet the IRS deductible.10WEX Inc. Flexible Spending Account (FSA) If your employer put you on one of these, the card will decline general medical purchases that a full FSA would have approved.
Over-the-Counter Items
The CARES Act, effective in 2020, opened up most over-the-counter medications and health products for card purchase without a prescription. Eligible OTC items include pain relievers such as acetaminophen and ibuprofen, cold and flu medicine, allergy medication, heartburn drugs, menstrual products like tampons and pads, sunscreen, first-aid supplies, nasal saline sprays, and eye drops.11WEX Inc. 5 Common Expenses Now Eligible for Your HSA and FSA Funds12WEX Inc. Products that walk the line between medical and cosmetic use are treated more strictly. Acne medicine is generally eligible because acne is a disease; routine skincare is not. Items that could go either way typically need a doctor’s note or medical necessity form before the card will approve them.4WEX Inc. HRA Coverage
Health Reimbursement Arrangements are employer-funded, so eligibility varies more than with an HSA or FSA. A traditional HRA usually reimburses the same Section 213(d) medical expenses, but the plan document controls what actually qualifies.4WEX Inc. Eligible Expenses Two specialized versions worth knowing:
- An Individual Coverage HRA (ICHRA) lets employers of any size fund an account so employees can buy their own individual health insurance. The HRA may reimburse premiums plus other qualified medical expenses, or premiums alone, depending on plan design.15WEX Inc. 4 Common Types of HRAs
- A Qualified Small Employer HRA (QSEHRA), for employers with fewer than 50 full-time-equivalent employees that offer no group health plan, reimburses individual insurance premiums and other qualified medical expenses up to IRS annual maximums.16Benefit Express. HRA Document
In early 2026, WEX launched a specialized HRA for GLP-1 weight-loss medications such as semaglutide. The employer sets funding levels and reimbursement rules, and employees use the card at pharmacies or through manufacturer-direct programs to access GLP-1 drugs outside the main medical plan.17WEX Inc. Dependent Care FSA
A Dependent Care FSA is a separate account with a completely different purpose: paying for care of dependents while you (and your spouse, if you have one) are working, looking for work, or attending school full time. Eligible expenses include daycare centers, babysitters, and nannies for children from birth through age 12, before- and after-school programs, summer day camp, and elder or adult daycare for a disabled dependent or spouse.18WEX Benefits. Two mechanics catch people off guard here. First, the card may be declined if the provider’s payment terminal is not coded as a childcare facility; you can pay out of pocket and file a claim instead.20WEX Benefits Knowledge Base. Pay With Your WEX Benefits Card Using Funds From Your Dependent Care FSA Second, unlike a medical FSA, this account is not pre-funded. The balance builds with each payroll deduction, and the card will only work once the accumulated funds cover the charge.
Commuter Benefits
If your employer offers commuter benefits through WEX, the card can pay for transit and parking related to getting to and from work. Covered transit includes trains, buses, subways, ferries, and vanpools seating at least six adults. The parking benefit covers lots and meters near the workplace.21WEX Benefits. The card has to be used directly at a transit or parking authority, or through their official website or app. It will be declined at non-transit merchants. In a handful of cities, WEX supports smart-commute integration that loads funds onto local transit cards.23WEX Benefits Knowledge Base. Lifestyle Spending Account
Some employers add a lifestyle spending account (LSA), which is a post-tax benefit with far more flexibility than the tax-advantaged accounts. What qualifies is set entirely by the employer. Common categories include gym memberships, fitness classes, exercise equipment, personal trainers, fitness trackers, race entry fees, and ski or golf passes, along with emotional-wellness spending like meditation classes, non-medical counseling, personal development courses, and retreats.24WEX Inc. A Note on the Medicare Advantage Flex Card
WEX also issues a prepaid flex card for Medicare Advantage plans, which is a different product from the employer benefits card above. Health plan administrators configure the eligible categories, which can include dental, vision, hearing aids, fitness club memberships, chiropractic care, over-the-counter medications and health-related foods, utilities, rent, fuel, and non-emergency medical transportation.26WEX Inc. What the Card Will Not Cover
Some expenses are consistently ineligible across every account type. Cosmetic procedures such as teeth whitening, veneers, tummy tucks, and elective cosmetic surgery cannot be paid with the card.5WEX Inc. When You Get a Receipt Request
If a purchase does not verify automatically at checkout, WEX will notify you and ask for documentation. Most plans give you 200 days from the transaction date to send it in.29WEX Benefits Knowledge Base. Miss the deadline and the card can be temporarily suspended until you provide the receipt or repay the unverified amount. The IRS also lets you offset a denied claim by submitting documentation for a different eligible out-of-pocket expense from the same plan year.30WEX Benefits.
