A Cigna Health Savings Account covers the medical, dental, vision, prescription, and mental health expenses that the IRS defines as qualified under Section 213(d) of the Internal Revenue Code, along with a narrow set of insurance premiums. Because eligibility follows federal tax rules rather than a Cigna-specific benefits menu, most costs tied to diagnosing, treating, or preventing disease qualify, while purely cosmetic procedures and general-wellness purchases do not.1IRS. Medical Care, Equipment, and Supplies
The account pays for the everyday costs most people associate with health care: doctor and clinic visits, hospital stays, laboratory fees, ambulance services, and nursing services.2Cigna. Eligible Expenses Surgery, radiology, and prescription drugs are also covered.3HSA Bank. Cigna HSA Enrollment Guide
A long list of supplies and devices qualifies too: blood-pressure monitors, CPAP machines, crutches, hearing aids, glucose meters, heart defibrillators, artificial limbs, back braces, and surgical bandages.2Cigna. Eligible Expenses Durable medical equipment such as hospital beds, wheelchairs, respirators, and dialysis machines is eligible, though the therapeutic purpose usually needs to be certified in writing by the treating provider.4Cigna. Preventive care sits alongside these expenses. Federal law requires HSA-qualified high-deductible health plans to cover in-network preventive services at no additional cost before the deductible, which on Cigna HDHPs includes annual physicals, certain vaccines, and screenings for conditions like diabetes and colon cancer.5Cigna. High-Deductible Health Plan Pros and Cons Recent IRS guidance also lets HDHPs cover over-the-counter oral contraceptives, male condoms, continuous glucose monitors for people with diabetes, and certain insulin products before the deductible without disqualifying the plan.6IRS. Dental and Vision
Eligible dental expenses include X-rays, fillings, extractions, dentures, dental implants, and orthodontia such as braces or clear aligners. Teeth bleaching and purely cosmetic veneers are not covered unless medically necessary.2Cigna. Eligible Expenses
On the vision side, eye exams, prescription eyeglasses, contact lenses, LASIK surgery, and blue-light-blocking glasses all qualify.2Cigna. Eligible Expenses3HSA Bank. Cigna HSA Enrollment Guide
Prescriptions, OTC Drugs, and Menstrual Products
Prescribed medications and insulin are straightforwardly eligible. Since the CARES Act took effect on January 1, 2020, over-the-counter drugs no longer need a prescription for HSA reimbursement. That covers pain relievers like ibuprofen and acetaminophen, allergy medications, antacids, antibiotic ointments, cold and flu remedies, acne treatments, sleep aids, and many other common OTC products.2Cigna. Eligible Expenses
The same law made menstrual care products qualified medical expenses, including tampons, pads, liners, cups, and sponges. Feminine moisturizers, powders, and deodorants do not qualify.7HealthEquity. CARES Act HSA Updates
Mental Health and Substance Abuse
Psychiatrist and psychologist visits are reimbursable, as are inpatient treatment programs for alcohol or drug abuse, including the meals and lodging provided by the treatment center during a stay. Hypnotherapy prescribed by a physician to treat a medical condition also qualifies. Marriage counseling does not.2Cigna. Eligible Expenses Services provided for non-medical reasons, such as general stress management, grief counseling, or family counseling not tied to a diagnosed condition, are generally not HSA-qualified either.8Talkspace. Maternity, Fertility, and Family Planning
Cigna’s eligible-expense list is broad on reproductive care. Birth control (pills, injections, and devices), fertility treatments including in-vitro fertilization and donor egg extraction, and abortion services all qualify. On the maternity side, childbirth classes, doula services (when the primary purpose is delivery), birthing tubs, breast pumps, nursing bras, and lactation supplies are eligible.2Cigna. Eligible Expenses Sterilization and vasectomy procedures are also covered under IRS rules.1IRS. Publication 502 – Medical and Dental Expenses
Chiropractic, Acupuncture, and Physical Therapy
For HSA purposes, chiropractic care, acupuncture, and physical therapy are all reimbursable medical expenses under IRS rules.2Cigna. Eligible Expenses You can pay for these services with HSA dollars regardless of your underlying health plan.
Whether your Cigna medical plan covers the services at a favorable cost-sharing rate is a separate question that depends on the plan. One Cigna HSA-qualified plan in Georgia, for example, covers chiropractic care and physical therapy at 10 percent coinsurance in-network with a combined 40-visit annual cap shared across physical therapy, speech therapy, occupational therapy, and chiropractic care, and lists acupuncture among services the plan does not cover.9Cigna. Summary of Benefits – Access Plus Bronze 6500 HSA Even when a plan excludes acupuncture from its benefits, you can still pay the acupuncturist out of pocket and reimburse yourself from the HSA, because the IRS treats acupuncture as a qualified medical expense.1IRS. Insurance Premiums You Can Pay With HSA Funds
Most health insurance premiums cannot be paid from an HSA, but there are important exceptions. HSA funds can go toward COBRA continuation premiums, health coverage while receiving unemployment compensation, Medicare premiums for Parts A, B, and D, employer-sponsored retiree health insurance, and long-term care insurance premiums.2Cigna. Eligible Expenses Medicare supplemental policies such as Medigap are specifically excluded.3HSA Bank. Cigna HSA Enrollment Guide
What a Cigna HSA Does Not Cover
The IRS draws a clear line at expenses that are purely cosmetic or for general well-being. Among the most commonly asked-about ineligible items:
- Cosmetic procedures such as face lifts, hair transplants, electrolysis, liposuction, and teeth bleaching, unless they correct a deformity from a congenital abnormality, an accident, or a disfiguring disease.2Cigna. Eligible Expenses
- Gym memberships and exercise equipment bought for general fitness. These can become eligible if a physician provides a diagnosis showing the activity is needed to treat a specific medical condition.2Cigna. Eligible Expenses
- Herbs, vitamins, and nutritional supplements taken for general well-being, unless supported by a letter of medical necessity.1IRS. Publication 502 – Medical and Dental Expenses
- Weight-loss programs for general health. Programs prescribed by a physician to treat a diagnosed disease such as obesity or heart disease can be eligible.1IRS. Publication 502 – Medical and Dental Expenses
- Dancing and swimming lessons (even if doctor-recommended), maternity clothes, infant formula, household help, marijuana, and the cost of adoption itself.2Cigna. Eligible Expenses
Items on the line, such as naturopathic treatments, holistic therapies, or products used for general well-being, may become eligible when a physician provides a letter of medical necessity tying them to a diagnosed condition.2Cigna. Eligible Expenses
Whose Expenses Qualify
HSA money can pay for eligible expenses incurred by you, your spouse, or your tax dependents.2Cigna. Eligible Expenses The operative phrase is “tax dependent,” not “person covered by your health plan.” An adult child can stay on a parent’s medical plan until age 26 under the ACA, but HSA funds cannot be used for that child’s expenses unless the parent also claims them as a dependent on their federal tax return. An adult child who is not a tax dependent can open their own HSA.10Cigna. One practical note on any of these expenses: keep receipts and Explanations of Benefits. There is no time limit on reimbursing yourself for a past qualified expense, as long as the expense was incurred after the HSA was established and you have kept the documentation.2Cigna. Eligible Expenses
