VisaScreen Certification: Agency Choice, Costs, and Validity

To work in the United States as a foreign-educated physical therapist, you must first obtain a healthcare worker certificate — the credential commonly sold as VisaScreen — before a consular officer or USCIS will approve your visa. VisaScreen certification for foreign-educated physical therapists is required by 8 U.S.C. § 1182(a)(5)(C), which makes any non-physician healthcare worker inadmissible without a certificate confirming that their education matches U.S. standards, that they can communicate in English, and that every professional license they hold is valid and unrestricted.1Office of the Law Revision Counsel. 8 USC 1182 – Inadmissible Aliens Two agencies are authorized to issue the certificate for physical therapists, and the full process — credential evaluation, English testing, and primary-source document verification — routinely takes six months or longer.

The requirement applies regardless of visa category. Whether you’re pursuing an H-1B, TN status under the USMCA, or an employment-based green card, you need the certificate in hand before your interview or adjustment of status.2CGFNS International. VisaScreen Visa Credentials Assessment Being educated in an English-speaking country doesn’t remove the requirement, though it may exempt you from the English exam portion.

CGFNS or FCCPT: Which Agency to Use

Only two organizations are authorized by the Department of Homeland Security to issue healthcare worker certificates for physical therapists, and their certificates carry equal weight with USCIS and consular officers.3U.S. Citizenship and Immigration Services. Health Care Worker Certification The choice is about cost, speed, and how specialized you want your reviewer to be.

The Commission on Graduates of Foreign Nursing Schools (CGFNS) handles all seven regulated healthcare professions. Its product is the VisaScreen: Visa Credentials Assessment, processed through the TruMerit platform for $740.4CGFNS International. Fee Schedule and Policies CGFNS also requires a self-reported typewritten summary of supervised clinical experience, which is unique to its process.

The Foreign Credentialing Commission on Physical Therapy (FCCPT) works only with physical therapists. Its equivalent product, the Type 1 for Immigration certificate, costs $1,340 and takes roughly 23 to 25 weeks from the point all documents are received. FCCPT also sells a USCIS status letter for applicants who need interim proof that credentialing is in progress: $55 for standard (about three weeks) or $155 for expedited (about one week).5Foreign Credentialing Commission on Physical Therapy. One practical advantage of FCCPT: its educational evaluation can satisfy both immigration certification and NPTE eligibility in a single review, sparing you a duplicate credential assessment later.

English Proficiency Requirements

Federal law requires proof of competence in spoken and written English through a standardized test, and the Secretary of Health and Human Services has sole discretion — not subject to further review — over which tests and scores qualify.1Office of the Law Revision Counsel. 8 USC 1182 – Inadmissible Aliens

CGFNS accepts seven exams: TOEFL iBT, IELTS Academic, PTE Academic, OET, TOEIC, Cambridge English (B2 First, C1 Advanced, or C2 Proficiency), and the Michigan English Test.2CGFNS International. VisaScreen Visa Credentials Assessment Minimum scores for the three most common exams are:

  • TOEFL iBT: combined 63 across reading, listening, and writing, plus 26 on speaking — in practice a minimum total of 89.6Foreign Credentialing Commission on Physical Therapy. Services
  • IELTS Academic: overall band 6.5 with at least 7.0 on speaking.2CGFNS International. VisaScreen Visa Credentials Assessment
  • PTE Academic: overall 55 with no sub-score below 50 in reading, writing, or listening, and a speaking score of 63. The 63 speaking threshold took effect December 1, 2024.2CGFNS International. VisaScreen Visa Credentials Assessment

Scores must arrive directly from the testing agency; self-reported results are rejected. Most test scores expire two years after the test date, so a slow document-collection phase can force a retake.2CGFNS International. VisaScreen Visa Credentials Assessment Applicants who completed their entire physical therapy education in English at institutions in certain English-speaking countries (Australia, Canada, Ireland, New Zealand, the United Kingdom, or the United States) may qualify for an exemption, though the criteria vary by agency. Missing the speaking threshold is the most common reason candidates retest, and each retake costs the full exam fee.

Educational Equivalency and How to Fix Deficiencies

Your foreign physical therapy education must be found substantially equivalent to a first professional degree in the United States. U.S. programs now award a Doctor of Physical Therapy, while many countries still train PTs at the bachelor’s level, which is where gaps often surface.

The Federation of State Boards of Physical Therapy publishes the Coursework Evaluation Tool used for this comparison. The current version, CWT 6, is based on the 2014 CAPTE evaluative criteria and has been in use since January 2017.7Federation of State Boards of Physical Therapy. Coursework Tools CWT Both CGFNS and FCCPT use this framework. It compares your curriculum, clinical hours, and course content against a U.S.-trained therapist’s.

A shortfall isn’t a hard stop. Recognized remediation pathways include:

  • Supplemental coursework. FCCPT’s Planned Learning and Assistance Network (PLAN) assigns an advisor who identifies deficiencies and pre-approves courses. Duke University offers a CAPTE-accredited 10-week online course covering U.S.-specific topics like administrative models of care, ethics, and direct access practice.8Federation of State Boards of Physical Therapy. Resolving Deficits in Education
  • Post-graduate clinical experience. Up to 300 hours can be credited if you have at least 1,000 hours of direct patient care within the most recent three years at a facility employing at least three practicing physical therapists.8Federation of State Boards of Physical Therapy. Resolving Deficits in Education
  • CLEP exams. The College Level Examination Program can cover general education content, but not professional coursework, and some jurisdictions cap how many subjects they’ll accept.8Federation of State Boards of Physical Therapy. Resolving Deficits in Education
  • Transitional DPT credits, which count only if the institution holding the records is regionally accredited.8Federation of State Boards of Physical Therapy. Resolving Deficits in Education

Remediation can add months. Pre-approve any coursework with a PLAN advisor before you enroll, or you risk paying for classes that don’t count.

Documents and Primary-Source Verification

Both agencies require primary-source verification: your schools and licensing authorities must send official records directly to the credentialing agency. You cannot hand-deliver or forward transcripts yourself. Document collection is usually the longest part of the process because it depends on how quickly foreign institutions respond.

FCCPT’s required set includes:

  • From you: a notarized attestation and notarized copies of all diplomas.
  • From your school: official transcripts of your PT education, clinical internship documentation, and official course syllabi.
  • From licensing authorities: verification of your eligibility to practice in the country where you were educated, plus verification of every license you have held since graduation — active, expired, or inactive.
  • If applicable: transcripts from any non-PT higher education, NPTE score transfers if you already hold a U.S. license, and English test scores sent directly from the testing agency.5Foreign Credentialing Commission on Physical Therapy. Getting Started

CGFNS asks for a similar set plus the self-reported summary of supervised clinical experience.2CGFNS International. VisaScreen Visa Credentials Assessment Both agencies want verification of every license you’ve ever held in any jurisdiction worldwide, and license validations must be dated within the last three years. Documents in other languages need certified translations. Before you file, confirm with each institution and licensing board that they can send sealed records to the United States. A tracking spreadsheet by institution helps surface bottlenecks early.

Applying, Processing Time, and Certificate Validity

CGFNS applications run through TruMerit; FCCPT uses its own portal. Either way, you’ll receive a unique file number after payment for online tracking. Enter degree names, license numbers, issuance dates, and expiration dates exactly as they appear on official records — any mismatch between your entries and what your institutions report triggers clarification requests or rejection. FCCPT gives you 12 months from the application date to submit all required documents; miss that window and the file may be closed.5Foreign Credentialing Commission on Physical Therapy. FCCPT quotes 23 to 25 weeks from complete documentation to certificate.5Foreign Credentialing Commission on Physical Therapy. Getting Started CGFNS does not publish a fixed timeline; wait times depend on caseload and complexity.

A healthcare worker certificate is valid for five years from issuance. If your visa extension, adjustment of status, or green card processing lands outside that window, you need a current certificate — an expired one leaves you inadmissible under the same statute that required it. CGFNS lets you place a renewal order in the year before expiration or within six months after, and recommends allowing another six months for the renewal to process. Renewal requires an updated passport photo, fresh license validations (if older than three years), and updated English proof where applicable.9CGFNS International. The Certificate Isn’t a License

The healthcare worker certificate gets you through immigration. It doesn’t authorize you to treat patients. To practice, you need a license from the state where you plan to work, and most states require you to pass the National Physical Therapy Examination.10Federation of State Boards of Physical Therapy. Non-US Candidates Each state sets its own rules, and some require the credentials evaluation before you can even register for the NPTE, or add a jurisprudence exam on state practice law. Contact the licensing board where you plan to work before assuming the certificate alone is enough. NPTE registration is $485, paid to FSBPT, with separate testing center fees.11Federation of State Boards of Physical Therapy. Exam Registration and Payment

What It All Costs

Fees stack up quickly. Plan for:

  • Healthcare worker certificate: $740 through CGFNS or $1,340 through FCCPT.
  • English proficiency exam: typically $200 to $300 per sitting for TOEFL iBT, IELTS Academic, or PTE Academic.
  • NPTE registration: $485, plus testing center fees.
  • State licensure application: varies widely, generally a few hundred to several hundred dollars.
  • Document translation and notarization: depends on volume and languages.
  • Criminal background check: roughly $40 to $50 in most states.

All in, expect $1,500 to $2,500 or more before you hold both the certificate and a state license, not counting supplemental coursework if your evaluation flags deficiencies. Budget for at least one English test retake as a contingency.
