VA Form 0857e is the Request for Medical Documentation that the Department of Veterans Affairs sends to your healthcare provider after you, as a VA employee, ask for a reasonable accommodation for a disability that isn’t visible and isn’t already documented in your file. You don’t complete the medical portion yourself. The VA’s Reasonable Accommodation Coordinator (RAC) fills in the top of the form identifying you, your requested accommodation, and the job duties at issue, then hands it to you to take to your treating provider, who answers five specific questions and returns it.

When You Receive the Form

You only see Form 0857e after you’ve already made an accommodation request. That request can be oral or written, and it can go to your supervisor, manager, RAC, or HR.1Department of Veterans Affairs. If your disability is visible or already documented, the RAC may not send the form at all.

What the Form Asks Your Provider

The bottom half of the form is a short questionnaire for your doctor, therapist, or other treating provider. It asks for five things:

  • The nature, severity, and duration of the impairment.
  • The major life activities the impairment limits.
  • The degree of limitation on each of those activities.
  • Why the requested accommodation is needed, given those limitations.
  • How the accommodation would help you perform your job or access benefits of employment.

Your Deadline and How to Handle the Form

You have 20 business days from the date the RAC gives you the form to return the completed medical documentation.3Department of Veterans Affairs. A few practical points:

  • Brief your provider before the appointment. Many haven’t seen this form. Tell them it’s a workplace accommodation request, not a disability claim or a general records release, and point them to items (a) through (e) on the form.
  • Keep the answer focused on the condition tied to your accommodation. The VA can only ask about the condition driving the request, not every health issue you have.

If the provider’s answers are vague or don’t clearly cover the five items, the RAC may ask for more information. What Happens After You Submit It

The 30-calendar-day processing clock for your accommodation request, which paused while you were gathering documentation, restarts when the RAC receives the completed form.4Department of Veterans Affairs. The RAC reads the documentation and makes two determinations: whether you have a disability covered by the Rehabilitation Act, and what your functional limitations are. The RAC does not share your diagnosis or the underlying medical documents with your supervisor or the Decision-Making Official (DMO). Those officials receive only a coverage determination and a description of your functional limits.4Department of Veterans Affairs. The RAC may also ask the DMO to complete an Essential Function Analysis identifying which duties in your position are truly essential.1Department of Veterans Affairs. Understanding the Reasonable Accommodation Process The DMO then schedules an interactive meeting with you to talk through which essential functions your limitations affect and what accommodations could work. Common options at the VA include modified schedules, telework, ergonomic equipment, assistive technology, adjusted leave, and workspace changes. After that conversation, the DMO approves the accommodation as requested, offers an alternative, or denies it.

If the Request Is Denied

If no accommodation would let you perform the essential functions of your current job without undue hardship, the VA is required to search for a vacant position at the same grade or lower that you’re qualified for and can perform with or without accommodation.1Department of Veterans Affairs. How Your Medical Information Is Protected

The completed form and any supporting documentation go only to the RAC and stay in a locked file cabinet separate from your Official Personnel Folder.4Department of Veterans Affairs.
