USC Credit Union Forms: Categories, Deadlines, and Submission

USC Credit Union forms are posted together at usccreditunion.org/forms, where members can file transaction disputes, place stop payments, update account information, change beneficiaries, send wires, enroll in balance protection, or close an account. Most forms submit online; a few are also available as printable PDFs. This guide walks through which form handles what, what you need before you start, how to submit, and the deadlines that actually bite.

The Five Form Categories

The forms page is organized into five groups, and each one routes to a different internal team. Picking the right category is the first thing that determines whether your request gets processed quickly or bounces back.

Account Updates

These forms change your profile. You can add a new name or Social Security number to your account, change a beneficiary, request a higher daily debit card limit, open a student organization account, or update authorized signers on an existing student organization account. Closing your account is handled separately under “Other Forms,” with both an online version and a printable PDF.

Transaction Disputes

There are five dispute forms, one for each transaction type: debit or credit card charges, ACH or electronic transfers, check withdrawals, online account takeovers, and Zelle disputes. Using the right one matters. A credit card dispute filed on the ACH form will likely be returned. Once the credit union has the completed form, standard processing runs three to five business days.

Stop Payments

Three stop payment forms are available: outstanding checks, ACH or electronic transfers, and cashier’s checks. Each requires the exact dollar amount and the payee name so staff can flag the right transaction before it posts.

Wire Transfers

The “Send an Outgoing Wire” form is listed under “Other Forms.” You will need the recipient’s full name, bank name, routing number, and account number. International wires also require the recipient bank’s SWIFT code. Fees are $20 for outgoing domestic wires, $40 for outgoing international, $5 for incoming domestic, and $10 for incoming international. Student checking accounts get two free incoming international wires per year.

Balance Protection

One form handles enrollment or changes to the credit union’s debt protection program, which can cover loan payments in qualifying situations such as disability or involuntary job loss.

What to Have Ready Before You Start

Having the right information in hand before you open a form prevents rejections and follow-up calls. At minimum, gather:

  • Your full legal name, exactly as it appears on your government-issued photo ID. Mismatches between the form and the ID on file are one of the most common reasons forms get kicked back.
  • Your Social Security number. Required on most account-change and membership forms.
  • Your account number, found on your online banking dashboard, your monthly statement, or the bottom of your checks.
  • USC Credit Union’s ABA routing number, 322079557, for direct deposit setup and wire transfers.
  • An unexpired government-issued photo ID such as a driver’s license or passport. Federal rules require credit unions to verify identity using an unexpired photo document. Non-U.S. persons can provide a passport with country of issuance, an alien identification card, or another government-issued document showing nationality or residence with a photograph.

For wire transfers, you also need the recipient’s routing number (domestic) or SWIFT code (international), full account number, and legal name and address.

For business or student organization accounts, more documentation applies. The credit union must identify any individual who owns 25 percent or more of the entity and at least one person with significant management control, such as a president, treasurer, or general partner. For each of those people, expect to provide a name, date of birth, address, and identification number.

Keep Your SSN Current

Your Social Security number on file drives tax reporting. The credit union issues Form 1099-INT on any account that earns $10 or more in interest during the year. If your SSN is wrong or missing, the credit union must apply backup withholding at 24 percent on your interest. If yours needs updating, use the “Add a New Name or SSN to your Account” form before interest posts.

Dispute Deadlines

Deadlines are where disputes get won or lost, and the clock depends on what kind of transaction went wrong.

Credit Card Disputes

For a credit card billing error, federal law gives you 60 days from the date the credit union sends the billing statement to submit a written dispute. Your notice has to identify your name and account number, state what you believe the error is and its dollar amount, and explain why you think it is wrong. The credit union must acknowledge receipt within 30 days and resolve the investigation within two billing cycles, no longer than 90 days.

Debit Card, ACH, and Electronic Transfer Disputes

For electronic fund transfers, including debit card charges, ACH withdrawals, and Zelle payments, you also have 60 days, but the clock starts when the credit union sends or makes available the periodic statement reflecting the error. Your notice must identify your name and account number and describe the suspected error, including the type, date, and amount. If you report by phone first, the credit union can require written confirmation within 10 business days. It then has 10 business days to investigate and report results, which it can extend to 45 days if it provisionally credits your account during the investigation.

Both deadlines are hard cutoffs. Miss the 60-day window and the credit union has no legal obligation to investigate, even if the error is obvious. File early.

How to Submit a Completed Form

USC Credit Union accepts forms through four channels. The right one depends on how quickly you need the request processed and whether originals are required.

Online

Most forms link directly to an online submission portal. This is the fastest option for dispute forms, stop payments, and account updates. A digital submission creates an immediate record that your request was received and skips mailing delays entirely. Account closures have both an online version and a printable PDF.

In Person

Four branches accept hand-delivered forms:

  • Flower Street Branch: 3720 S Flower Street, CUB 1st Floor, Los Angeles, CA 90007
  • Health Sciences Campus Branch: 1969 Zonal Avenue, SRH 101B, Los Angeles, CA 90033
  • University Village Branch: 3096 South McClintock Ave, Suite 1430, Los Angeles, CA 90089
  • Campus Center Branch: 3601 Trousdale Pkwy, STU 106, Los Angeles, CA 90089

Walking a form in is worth the trip when you have a time-sensitive request, need a signature witnessed, or want to ask questions as you submit.

By Mail

There are two addresses. Use the P.O. Box for standard postal mail and the street address for overnight carriers:

  • Regular mail: USC Credit Union, P.O. Box 512718, Los Angeles, CA 90051-0718
  • Overnight (FedEx/UPS): USC Credit Union, 3720 S. Flower Street, Los Angeles, CA 90007

For anything time-sensitive, especially dispute forms with federal deadlines, use a trackable service so you have proof of when the credit union received it.

Phone and Email

For general questions about which form to use or how to fill one out, contact member services at (213) 821-7100 or usccu@usccreditunion.org. Phone staff can walk you through the online submission in real time.

Why Forms Get Rejected

A rejected form means starting over. These are the issues that trip people up most often:

  • Missing signatures. Every form with a signature line requires one. Unsigned forms are returned without processing.
  • Name mismatches. The name on your form must match the name on your account. If you recently changed your legal name, update your account records first using the “Add a New Name or SSN” form before submitting anything else.
  • Illegible handwriting. If you fill out a printable PDF by hand, the credit union has to scan it into its systems. Entries that are hard to read can be rejected even when technically complete.
  • Wrong form for the transaction. A debit card dispute filed on the ACH form, or an ACH stop payment filed on the check form, goes to the wrong team and delays everything.
  • Outdated form versions. Forms are updated periodically. Download the current version from usccreditunion.org/forms rather than reusing a saved copy.

After You Submit

Most account updates and stop payments process within a few business days. The credit union typically confirms by email or through your online banking dashboard. If something is missing or unclear, a staff member will reach out, so check your email (including spam) and your banking notifications. Dispute forms specifically run on a three-to-five-business-day processing window once the completed form is received.

If you mailed something and have not heard back within a week, call (213) 821-7100 with your tracking number handy. For online submissions, your banking portal should show a record that the form was received, which at least confirms nothing was lost in transit.
