The TRICARE catastrophic cap is the ceiling on what your family pays out of pocket for covered care in a calendar year. For 2026, that ceiling runs from $1,000 to $4,635 depending on your sponsor’s service entry date, duty status, and plan.1TRICARE. Catastrophic Cap Once you hit it, TRICARE pays 100 percent of the allowable charge for covered services through December 31. What actually counts toward the cap is narrower than most families expect, so the dollar figure only tells half the story.
2026 Cap Amounts
Two things set your number: your sponsor’s status, and whether your family sits in Group A (sponsor entered service before January 1, 2018) or Group B (sponsor entered on or after that date).2TRICARE. Beneficiary Groups

TRICARE Reserve Select, TRICARE Retired Reserve, and Continued Health Care Benefit Program members follow Group B amounts. Group A caps are fixed and don’t change year to year. Group B caps adjust annually with federal cost-of-living calculations, which is why Group A active duty families still enjoy the lowest ceiling.
What Counts Toward the Cap
Only specific out-of-pocket costs move you closer to the ceiling. Your regional contractor tracks them automatically as claims are processed.1TRICARE. Catastrophic Cap
- Enrollment fees for TRICARE Prime, credited from the first payment.
- Annual deductibles for outpatient care.
- Pharmacy copayments at retail pharmacies, military treatment facilities, and through the mail-order program.
- Cost-shares for covered services, including inpatient stays, outpatient visits, lab work, and mental health care, based on the TRICARE-allowable charge.
Once those combined payments reach your family’s cap, TRICARE pays 100 percent of the allowable charge for any remaining covered care through December 31.
What Does Not Count
Several categories of spending stay outside the cap no matter how large they get. These charges remain your responsibility even after you’ve hit the ceiling.
Premiums. Monthly or quarterly premiums for TRICARE Reserve Select, TRICARE Retired Reserve, TRICARE Young Adult, and the Continued Health Care Benefit Program are excluded entirely.1TRICARE. Catastrophic Cap You keep paying them all year regardless of how much care you use.
Point-of-service charges. If you’re on TRICARE Prime and see a non-network provider without a referral, TRICARE treats the claim as point-of-service: a $300 individual deductible ($600 per family) plus 50 percent of the allowable charge, on top of any extra provider fees.5TRICARE. Point-of-Service Option None of it counts toward the cap. A single ER visit at an out-of-network facility without proper referral documentation can generate a large bill that sits entirely outside the cap’s protection.
Non-covered services and balance billing. Anything TRICARE doesn’t cover, such as cosmetic or experimental procedures, is excluded.1TRICARE. Catastrophic Cap Non-participating providers can charge up to 115 percent of the TRICARE-allowable amount, and the gap between the allowable charge and the billed amount is yours to pay and does not count.6TRICARE Manuals. TRICARE Policy Manual – Balance Billing Limits
Extended Care Health Option costs. If a special-needs dependent uses ECHO, the monthly cost-share for ECHO services does not apply to your regular catastrophic cap. ECHO has its own separate limits.7TRICARE. What Is the TRICARE Catastrophic Cap?
The Calendar Year Reset
Every January 1, your accumulated total resets to zero.1TRICARE. Catastrophic Cap Nothing carries over. A family that hit the cap in September and paid nothing from October through December starts paying deductibles and copays again in January.
That reset is worth planning around. If a major procedure could be scheduled in late December or early January, the timing decides which year absorbs the cost-share. Families already near the cap in late fall may find that scheduling care before year-end means TRICARE covers it in full, rather than pushing it into a fresh year with a reset ceiling.
When Your Duty Status Changes Mid-Year
Separating from active duty, retiring, or moving to a reserve component changes which cap applies to you, but you don’t lose credit for what you’ve already paid. Your regional contractor carries forward accumulated deductibles and cost-shares and applies them to the new, higher cap.8TRICARE Manuals. TRICARE Reimbursement Manual – Catastrophic Loss Protection
The new cap takes effect for services received after midnight on the day your status changes. If your family had already spent $900 toward a $1,000 active duty cap and the sponsor retires mid-year, that $900 rolls into the retiree cap. You don’t start over, but the gap between what you’ve paid and the new ceiling grows.
Tracking Your Cap and Fixing Errors
Your regional TRICARE contractor tracks qualifying expenses automatically. You can see your running total through the contractor’s online portal, and each processed claim generates an Explanation of Benefits statement showing what was credited toward the cap.1TRICARE. Catastrophic Cap
Review those statements. Processing errors happen, and a cost-share that should have counted can slip through uncredited. Catching mistakes early is far easier than untangling them months later.
If you’re charged for a covered service after you’ve already hit the cap, request a reconsideration through your regional contractor, not the provider. The window is tight: 90 calendar days from the date on the Explanation of Benefits.9TRICARE Manuals. TRICARE Operations Manual – Claims Adjustments and Recoupments When the contractor corrects the claim, it updates your catastrophic cap and deductible records in the Defense Enrollment Eligibility Reporting System so your totals stay accurate for the rest of the year. Save every EOB; heavy medical years generate dozens of claims, and the one that pushed you past the cap is the reference point you’ll need if a later charge should have been paid at 100 percent.
