Standard Form 1199A: Filling Out and Submitting the Direct Deposit Sign-Up

To fill out SF 1199A, print your payee information and payment details in Section 1, enter the paying agency’s name and address and sign Section 2, then take the form to your bank or credit union so an authorized representative completes Section 3 and mails the certified form to the agency. One boundary before you start: if you receive Social Security, Supplemental Security Income, civil (non-military) federal retirement, or railroad retirement benefits, the Bureau of the Fiscal Service directs you to FMS Form 1200 or an online enrollment channel instead, even though those payment types appear as checkboxes on the 1199A.2Bureau of the Fiscal Service. Before You Start

Download a blank SF 1199A from the General Services Administration at gsa.gov,3GSA. Direct Deposit Sign-Up Form from the Bureau of the Fiscal Service forms page,2Bureau of the Fiscal Service. Forms or from your paying agency. Your bank or credit union may also keep blank copies on hand.

Before writing anything down, pull together three pieces of information: your claim or payroll ID number from a benefit statement, pay stub, or agency correspondence; the exact name and mailing address of the agency that pays you; and your bank account number and account type. Use black ink. If you make one payment type electronic, only one payment type goes on this form. A separate SF 1199A is required for each type of federal payment you want deposited.

Section 1: Payee Information

Section 1 is yours to complete. Print your full legal name (last name first, then first name and middle initial) exactly as it appears in the paying agency’s records, followed by your current mailing address, city, state, ZIP code, and a telephone number with area code. The agency uses that number if something on the form doesn’t reconcile with its records.

Check one box for your payment type. The choices printed on the form are Social Security, SSI, Railroad Retirement, Civil Service Retirement (OPM), VA Compensation or Pension, federal salary/military civilian pay, military active, military retire, and military survivor, plus a blank “Other” line.4Internal Revenue Service. Enter your claim or payroll ID number carefully. This alphanumeric code is what links your enrollment to the correct payment file at the agency, and a wrong or missing ID is one of the most common reasons an enrollment stalls, because the agency simply cannot match the form to your account.

Mark whether your account is checking or savings, then write in your depositor account number. Transposing a single digit here means the ACH system cannot route your payment, so verify the number against a voided check or your bank’s account documentation before you sign.

Section 2: Agency Information and Signature

In Section 2, write the name and mailing address of the government agency that sends your payments. For a VA claimant, that’s the regional office handling the claim; for a service member, it’s the Defense Finance and Accounting Service office processing military pay. If you don’t know the correct address, call the agency before you finish the form. The bank mails the completed SF 1199A to whatever address you write here, so an error at this step sends the paperwork to the wrong place.

Sign and date the Payee/Joint Payee Certification below the agency information. Your signature does three things at once: it certifies that you’re entitled to the payment, that you’ve read the terms printed on the back of the form, and that you authorize both the deposits into the account you listed and the government’s right to recover funds deposited after you become ineligible or after a beneficiary’s death.4Internal Revenue Service. Read the back of the form before you sign. It creates a binding agreement, and it obligates joint account holders to notify both the agency and the bank promptly if the beneficiary dies.5U.S. Department of Labor. Instructions for 1199A Form

Section 3: Financial Institution Certification

You do not fill out Section 3. Take or mail the partially completed form to your bank or credit union, where an authorized representative enters the institution’s nine-digit routing transit number, confirms that the account number and account type match their records, signs the certification, and prints their title along with the institution’s name, address, and telephone number.4Internal Revenue Service. Do not copy the routing number in yourself. If it’s wrong, the enrollment gets rejected and you start over with a new form. Let the bank fill in its own numbers.

Most banks and credit unions certify the form at no charge as routine customer service. If yours refuses or isn’t familiar with the SF 1199A, call the paying agency. Many agencies accept alternate enrollment methods that skip the bank certification step.

Joint Account Holders

The form has an optional Joint Account Holders’ Certification. A joint holder’s signature isn’t required for the form to process, but adding it creates a clearer record and acknowledges the terms on the back, including the obligation to return funds deposited after a beneficiary’s death.5U.S. Department of Labor. Submitting the Form

The financial institution mails the completed SF 1199A to the agency identified in Section 2.1Bureau of the Fiscal Service. Direct Deposit Sign-Up Form The form prints as a multi-part document: the original goes to the agency, a copy stays with the bank, and your copy stays with your records. If the bank hands the form back for you to mail, use certified mail or another method with a tracking receipt so you can prove it was sent.

There is no universal processing time; the timeline depends entirely on the paying agency. Some activate direct deposit within one or two payment cycles, and you may receive a final paper check during the transition. If your payment hasn’t arrived electronically after two full payment cycles, contact the agency directly to confirm receipt and status.

Faster Alternatives to the Paper Form

For many federal payments, you can skip the paper SF 1199A entirely. Social Security recipients can set up or change direct deposit through the “my Social Security” account at ssa.gov.8Social Security Administration. Federal employees and military members generally enroll through their agency’s payroll portal: myPay for the Defense Finance and Accounting Service, Employee Express for many civilian agencies. If your agency offers an electronic option, use it. It’s faster, it eliminates the bank certification step, and it removes a layer of data entry errors.

If you don’t have a bank or credit union account, the Direct Express Debit Mastercard is another electronic option for federal benefit payments. A Note on False Information

The SF 1199A is a federal form. Under 18 U.S.C. 1001, knowingly falsifying a material fact or making a fraudulent statement in a matter within federal jurisdiction can result in a fine, up to five years in prison, or both.11Office of the Law Revision Counsel. 18 U.S. Code 1001 – Statements or Entries Generally Submitting someone else’s account information to redirect their federal payments would fall under that statute. On the ordinary side, the Privacy Act Notice on the form also warns that leaving required fields blank can delay or prevent enrollment, so fill in every field the form asks for.
