SSDI Paper Application: Forms, Filing Date, and Denials

The Four Forms in the Packet

A complete paper filing rests on four documents, each doing a different job:

What to Gather Before You Start Writing

Pulling documentation together first saves real time and prevents the kind of incomplete filings that stall claims for weeks.

Medical Records

Medical evidence carries more weight than anything else in a disability claim. Gather names, addresses, and phone numbers for every doctor, therapist, hospital, and clinic that has treated your condition — not just providers from the past year, but anyone with relevant records.5Social Security Administration. Work History

Under a rule that took effect in June 2024, the agency evaluates your past relevant work based on the last five years before your disability began, not the 15-year window used in older claims.6Social Security Administration. Personal and Financial Details

Have your Social Security number and proof of identity ready. Federal law requires benefit payments to be made electronically, so bring your bank account and routing numbers to the form.7Social Security Administration. Lock In a Protective Filing Date First

Before you sit down with a stack of forms, contact the SSA and state your intent to file. That establishes a protective filing date, which sets the earliest possible start for your benefits even though your paperwork isn’t done. You can do this by phone, in person at a field office, or in writing, and a family member or representative can do it for you. Once set, you have six months to submit the completed application. Miss that window and the protective date expires, meaning your benefits get calculated from whatever later date you actually file. Filling Out the Forms

SSA-16: The Alleged Onset Date

The most consequential field on Form SSA-16 asks when you believe your condition became severe enough to keep you from working.10Social Security Administration. SSA-3368: Be Specific, Not Dramatic

The disability report asks how your condition limits everyday physical tasks like lifting and walking, and mental tasks like concentrating or following instructions. Specific descriptions serve you better than emphatic ones. “I can stand for about 10 minutes before needing to sit” is more useful than “I can barely stand.” What you write here gets compared against the physical and mental demands of your past jobs, so keep it consistent — if you describe severe lifting restrictions on the disability report but your work history lists a desk job, that gap doesn’t help.

SSA-827: Sign in Ink

Legible handwriting across every form matters more than it sounds. Paper submissions get scanned, and unclear entries create processing delays that you bear as waiting time.

Submitting the Packet

Mail or deliver the entire packet to the local Social Security field office that serves your zip code. The office locator on the SSA website identifies the correct address. Certified mail with return receipt requested gives you tracking and delivery confirmation, which protects you if a dispute later arises about when the agency received your forms.

If your submission includes original documents like a birth certificate, the agency scans them into your file and mails them back. What Happens After You Submit

Review unfolds in two phases. First, a Social Security field office checks the non-medical requirements: whether you have enough work credits, whether current earnings fall below the substantial gainful activity threshold, and whether you meet age and residency criteria.5Social Security Administration. If you clear those hurdles, the file moves to your state’s Disability Determination Services office. DDS is a state-run agency fully funded by the federal government, and its examiners evaluate the medical evidence. If your records don’t paint a complete picture, DDS may schedule a consultative examination with an independent physician at no cost to you.13Social Security Administration. As of early 2026, the average processing time for initial disability claims is about 193 days, roughly six and a half months.14Social Security Administration. The agency mails a determination letter explaining whether you’ve been approved or denied, along with the monthly benefit amount if approved.

Workers’ Compensation Can Reduce Your Payment

Report the exact workers’ comp amount on your application so the agency calculates the reduction correctly from the start, rather than creating an overpayment you’d have to repay later.

If Your Claim Is Denied

  • A hearing before an administrative law judge who was not involved in the earlier decisions. A significant number of initially denied claims get approved at this stage.
  • Appeals Council review, which can grant, deny, or dismiss the request, or send the case back to the judge.
  • A civil action in U.S. District Court if the Appeals Council denies review.

Always appeal rather than filing a brand-new application. A new application resets your protective filing date and can erase months of potential back pay. Your original date stays intact through appeals.

Getting Help With the Claim

You can appoint an attorney or non-attorney representative at any stage by submitting Form SSA-1696.18Social Security Administration. When Approved Benefits Actually Start

SSDI benefits do not begin the month your disability started. Federal law imposes a five-month waiting period, so payments start in the sixth full calendar month after the agency determines your disability began.11Social Security Administration.
