There is no single SSA disability fax number. The correct line depends on which office is holding your case right now: your local Social Security field office handles administrative paperwork, your state’s Disability Determination Services handles medical evidence during the initial and reconsideration stages, the Office of Hearings Operations handles anything for an administrative law judge, and the Appeals Council has its own national fax for later review. Sending a document to the wrong office can delay it by weeks, because staff at one location cannot simply drop your fax into a file held somewhere else.
Confirm which office controls your file before you send anything. The most recent letter you received from SSA or DDS will name it.
Field Office Fax Number
Your local field office handles the non-medical side of the claim: identity verification, work history, and initial application paperwork.1Social Security Administration. Disability Determination Process Send administrative documents, proof of identity, and employment records here.
To get the number, use the Field Office Locator at ssa.gov/locator. Enter your ZIP code and the result page shows the office’s phone number, fax number, and street address.2Social Security Administration. Field Office Locator You can also call SSA’s national line at 1-800-772-1213, Monday through Friday, 8:00 a.m. to 7:00 p.m. local time, and ask for your local office’s fax number.3Social Security Administration. DDS Fax Number for Medical Evidence
Once the field office accepts your application, it forwards the case to your state’s Disability Determination Services for medical review. DDS is a state agency that evaluates whether your medical evidence meets the federal definition of disability.4Social Security Administration. Disability Evaluation Under Social Security Medical records, diagnostic test results, physician statements, and functional reports go to DDS, not to the field office.
There is no single national DDS fax line, and the DDS fax number is almost always different from your field office fax. The most reliable source is the letter DDS sent you asking for information; the fax instructions are printed on it. If you’ve lost the letter, look up your state’s Professional/Medical Relations Officer through SSA’s directory at ssa.gov/disability/professionals/procontacts.htm, which lists DDS contact information by state.5Social Security Administration. If DDS sent your doctor’s office a barcode cover sheet with the records request, that barcode is critical. DDS uses it to route the fax automatically into your electronic case file.6Social Security Administration. Use Electronic Records Express to Send Records Related to Disability Applications Whenever a barcode has been issued, place it as the first page of every fax.
Hearing Office Fax Number
If your claim is denied at the initial and reconsideration levels, your case moves to a hearing before an administrative law judge at an Office of Hearings Operations location. That office has its own fax number, separate from the field office and DDS.
Find it through the Hearing Office Locator at ssa.gov/appeals/ho_locator.html. Each listing shows the hearing office that serves your field office, along with a phone number, a standard fax number, and a separate “eFile Fax” number for electronic filing through the Electronic Records Express system.7Social Security Administration. OHO’s Hearing Office Locator Use the standard fax number unless you’ve been specifically told to use the eFile line.
Watch the timing. All written evidence must reach the administrative law judge no later than five business days before your scheduled hearing. If you miss that cutoff, the judge can refuse to consider the evidence unless you can show the delay was caused by something beyond your control, such as a serious illness, a records request that came back late, or misleading instructions from SSA itself.8eCFR. 20 CFR 404.935 – Submitting Written Evidence to an Administrative Law Judge A record that arrives four days before the hearing instead of five can be excluded entirely.
Appeals Council Fax Number
If the administrative law judge denies your claim, the Appeals Council is next. It has a dedicated fax line for additional evidence or legal arguments: 833-509-0817. Use this number only when no barcode cover sheet has been issued for your case.9Social Security Administration. Appointed Representative Guide to Requesting Appeals Council Review and Submitting Evidence If the Council has issued a barcode for your file, place the barcode as the front page of the fax (skip the separate cover sheet) and use the fax number printed on that barcode instead.
Requests for Appeals Council review should generally go through your local field office first rather than directly to the Council. Field office staff can help you complete and submit the proper request form.
What to Put on Every Fax
Whichever office you are sending to, include a clear cover sheet with your full legal name, Social Security number, and daytime phone number. If you have a claim number that differs from your SSN, add it as well. More identifying information on the cover sheet means less risk of misfiling.
When SSA or DDS has provided a barcode cover sheet, use it as the very first page. Barcodes route faxed documents electronically into your case file; without one, staff have to sort and assign your pages manually.6Social Security Administration. Use Electronic Records Express to Send Records Related to Disability Applications
Keep individual fax transmissions under 200 pages. If you need to send more, split the submission into segments and put the barcode cover sheet at the front of each segment.6Social Security Administration. Use Electronic Records Express to Send Records Related to Disability Applications Hospital records and surgical histories easily exceed that cap, so check the page count before you start feeding pages in.
Save every fax confirmation page. It documents the date, time, and page count you transmitted. That timestamp is your best evidence if a dispute later arises about whether you met a deadline.
Verify the Number Before You Dial
Every disability fax carries your Social Security number, medical diagnoses, treatment records, and sometimes financial information. A fax that reaches the wrong machine puts all of that in a stranger’s hands with no way to recall it.
Check the number digit by digit against the correspondence you received or the locator result. If you got the number by phone, write it down and confirm it again before dialing. SSA advises sending documents that contain personally identifiable information only to a secure fax location.10Social Security Administration. Properly Safeguarding Personally Identifiable Information On your side, that means faxing from a place where confirmation pages and copies won’t be left sitting out, and avoiding shared machines in public spaces when you can.
Alternatives That May Track Better Than Fax
Fax is not the only option, and some methods give cleaner delivery records.
my Social Security Uploads
Individual claimants can upload certain forms and supporting documents through the “Upload Documents” tool in a my Social Security account at ssa.gov. This provides immediate electronic confirmation that the file reached SSA’s system.11Social Security Administration. Can I Electronically Submit Documents to Social Security Not every document type is available for upload, and SSA cannot accept original or certified copies electronically, so some items still have to be faxed or mailed.
Electronic Records Express
Electronic Records Express is a separate system for medical providers, school professionals, attorneys, and claimant representatives to submit health and educational records directly to SSA or DDS.12Social Security Administration. Electronic Records Express Individual claimants cannot register for ERE accounts on their own. If you have a disability attorney or representative, they can submit evidence through ERE or the Appointed Representative Services portal, which provides upload confirmation and direct file association with your case.13Social Security Administration. Mail, Drop Box, or In Person
You can mail documents to your local field office; SSA lists mailing alongside fax and drop box delivery as an accepted method.14Social Security Administration. Submit Forms and Upload Documents For time-sensitive materials near a deadline, certified mail with a return receipt creates a postal record of delivery that is harder to dispute than a fax confirmation. You can also hand-deliver documents to your field office during business hours or use its drop box if one is available.
