Every Social Security form is available as a free PDF at ssa.gov/forms, and there is no legitimate reason to pay a third-party site for one. The library covers retirement, disability, Social Security card applications, representative appointments, and dozens of less common requests. Downloading the Social Security PDF form you need before you start is useful even if you plan to apply online, because the PDF shows you exactly what information and documents you’ll have to gather.1Social Security Administration. Where to Download the Forms
Go to ssa.gov/forms. You can search by form number, like SSA-1-BK for retirement, or by keyword, like “disability” or “card.” Each form opens as a PDF, which any modern browser can display; Adobe Acrobat Reader works too if you’d rather use a dedicated reader.
Watch the address bar. The real site ends in .gov, uses HTTPS, and never asks for payment. SSA has warned that scam sites advertise forms and other agency documents to draw people in.2Social Security Administration. Which Form You Probably Need
Most people who come looking for an SSA form need one of five.
SS-5, Application for a Social Security Card
Use Form SS-5 to apply for an original number, replace a lost card, or correct information on your record. You can usually mail the signed form with your supporting documents to any Social Security office. If you are age 12 or older and have never had a Social Security number, you must apply in person.3Social Security Administration. Application for a Social Security Card The form is short. SSA-1-BK, Application for Retirement Benefits
The retirement application asks for your personal information, full marital history, dependent children, recent employer names and addresses, expected earnings, and direct deposit details. You’ll also make a Medicare Part B enrollment decision on this form.4Social Security Administration. SSA-16, Application for Disability Benefits
The disability application collects when your condition became severe enough to stop work, your recent employment, and whether the condition is work-related. You’ll need medical evidence and may be asked about any workers’ compensation you’ve received or filed for.5Social Security Administration. The SSA-16 rarely travels alone. You’ll almost certainly also complete Form SSA-3368-BK, the Adult Disability Report, which asks for a detailed breakdown of every job you held in the five years before you became unable to work, along with your medical conditions, treatments, and medications.7Social Security Administration. SSA-827, Authorization to Release Medical Information
Disability applicants also sign Form SSA-827, which lets doctors, hospitals, and other providers release records to SSA. The authorization covers all medical records, including substance abuse treatment and educational records relevant to your disability. You don’t have to list each provider by name; authorizing a class, such as “all treating physicians,” is acceptable.8Social Security Administration. Information on Form SSA-827
SSA-1696, Appointing a Representative
If you want an attorney or advocate to handle a case for you, notify SSA in writing. Form SSA-1696 is the standard way. A written statement or the online e1696 will also work.9Social Security Administration. Claimant’s Appointment of a Representative
Documents You’ll Have to Attach
This is where applications get rejected. SSA will not accept a plain photocopy or a notarized copy of a supporting document. You must submit either the original or a copy certified by the agency that issued it.10Social Security Administration. Learn What Documents You Will Need to Get a Social Security Card A certified copy is one stamped or sealed by the record custodian attesting to its accuracy.11Social Security Administration. Acceptability of Documentary Evidence For a birth certificate, that means ordering a certified copy from the vital records office in your state of birth, usually $10 to $30.
Everything must be current and unexpired. A receipt showing you’ve applied for a document but haven’t received it doesn’t count.10Social Security Administration. Returning Documents Submitted for an SSN Card Even so, mailing an original birth certificate or passport makes most people uneasy. Bringing the documents to a field office in person, so you hand them over and get them back the same visit, is a reasonable alternative.
How to Submit the Completed Form
Upload Through Your my Social Security Account
Many forms can be uploaded at ssa.gov/updocs once you’ve signed in. Uploadable items include Social Security forms, bank statements, pay stubs, financial records, and rental agreements.13Social Security Administration. Mail, Fax, or Drop Box
You can also mail completed forms and copies of supporting documents to your local field office, fax them, or use the office’s secure drop box.13Social Security Administration. Signatures and Paper
Paper forms sent by mail or drop box require a handwritten signature in pen and ink.16Social Security Administration. Application for a Social Security Card Form SS-5-FS Electronic submissions through SSA’s online system use click-and-sign, so no printing is needed.
Whichever route you use, keep a complete copy of everything: the form, every supporting document, and any mailing receipt or confirmation number. If something goes missing in processing, your copies are what protect you.
Statements and Benefit Verification Letters Aren’t in the Forms Library
Two of the most-requested SSA documents are not PDF forms you fill out. A Benefit Verification Letter, which lenders and landlords ask for, and your Social Security Statement, which shows earnings history and future benefit estimates, are both generated inside your my Social Security account and downloaded as PDFs from there.18Social Security Administration.
