The form is filed by the driver, not the member. Register as a Driver Before You Submit Anything

SafeRide will not accept a claim form from a driver who has not gone through registration and been approved by the state. What to Have in Hand Before You Fill It Out

Most of these details are easiest to capture at the time of booking and at the appointment itself.

  • Member’s Medicaid ID number. On the member’s Medicaid card.
  • Appointment date. The exact date the member was seen.
  • Provider or facility name and phone number.

Get the provider’s signature at the appointment. If you leave without it, you will have to go back or call the office for a signed copy, and your claim waits.

Filling Out the Form

The layout differs slightly between health plans, but every version has three blocks: driver, member, and trip details.

Driver Block

Trip Block

For each ride, fill in the appointment date, the Ride ID from booking, the provider or facility name, and the provider’s phone number. Most forms have room for multiple rides on one page, provided they all belong to the same member. Sign and date the certification at the bottom; your signature confirms the trips took place and lets SafeRide verify the information. The provider signs next to each ride entry. Where to Send the Completed Form

Completed forms go to SafeRide by email, fax, or mail. The email address depends on the plan and is printed on the form. Two examples from SafeRide’s published forms:

Filing Deadlines

The number of days you have to submit a claim after a trip is set by each state’s Medicaid agency, not by SafeRide, and SafeRide cannot extend or modify that deadline.1SafeRide Health. After You Submit

During review, SafeRide matches your trip details against its own booking records and the provider’s confirmation of attendance. Payment goes out through PayQuicker, an electronic payment platform, or by paper check to the address on file. Paper checks take longer to arrive. Why Claims Get Rejected
