Railroad Medicare Provider Enrollment: PTAN, EDI, and Updates

To enroll as a provider with Railroad Medicare, you request a separate Railroad Medicare Provider Transaction Access Number (PTAN) from Palmetto GBA, the specialty contractor that handles Part B claims for railroad beneficiaries nationwide. There are no CMS-855 forms involved. If you are already enrolled with your local Part B Medicare Administrative Contractor, you are eligible to treat railroad beneficiaries; you just need the railroad-jurisdiction PTAN before you can bill, and a separate Electronic Data Interchange (EDI) enrollment if you plan to submit claims electronically.¹

Who Needs a Railroad Medicare PTAN

Railroad Medicare is a separate jurisdiction from standard Medicare. Part B claims for railroad retirement beneficiaries do not go to your regular local MAC; they go to Palmetto GBA, which serves as the Railroad Specialty Medicare Administrative Contractor.² Because it is a distinct jurisdiction, you need a distinct PTAN issued by Palmetto GBA before any claim under your NPI can be processed.¹

The prerequisite is straightforward. You must first be enrolled with your local Part B MAC. That local enrollment is what Palmetto GBA verifies against when it issues the railroad PTAN, and no separate CMS-855 filing goes to Palmetto GBA.¹

How To Request a Railroad Medicare PTAN Online

The primary path is the PTAN Lookup and Request Tool on Palmetto GBA’s website. To use it, you supply three pieces of information:

The PTAN assigned by your local Part B MAC
Your National Provider Identifier, matching the PTAN type (individual NPI for an individual PTAN, group NPI for a group PTAN)
The last five digits of your Tax Identification Number

The tool verifies these against your local Part B MAC enrollment data, so that data must be current and correct or the request will fail.³

Two timing rules matter. Wait at least 14 business days after you receive your local Part B MAC PTAN before requesting the railroad PTAN, so the data has time to propagate. And Palmetto GBA asks that you not request a Railroad Medicare PTAN until you actually have a Part B claim to submit for a railroad patient.³

How Long It Takes

Most new PTANs requested through the online tool are assigned the same day. Some requests need additional research; in those cases, allow up to 30 calendar days before returning to the tool to check the result. If you trigger enrollment instead by submitting a paper CMS-1500 claim, allow the same 30 days for Palmetto GBA to verify your Part B enrollment and assign a PTAN.⁴

When the Online Tool Will Not Work

The tool has real limits. It will not process a request if:

You are enrolled under a Social Security Number rather than a TIN
Your local Part B MAC PTAN is linked to multiple NPIs
Your existing Railroad Medicare PTAN is inactive

Third-party billers and clearinghouses also cannot use the tool directly and must coordinate through the provider. In any of these situations, call the Provider Contact Center at 888-355-9165 and select option 3 for Provider Enrollment.³

The Fax Alternative

If you submit claims electronically, you can also request a Railroad Medicare PTAN by completing the dedicated PDF form and faxing it to (803) 382-2417. The form collects your legal business name, TIN, payment and practice addresses, individual and group NPIs, local MAC PTANs, and individual provider names. Up to five providers fit on one form; use additional forms beyond that. Palmetto GBA asks that you complete the form digitally before printing, since handwritten entries can slow processing.⁵

Group PTANs

You do not request a group PTAN separately. For a new group, the Railroad Medicare group PTAN is created automatically when the first individual group member requests a PTAN. That first member must already be enrolled with their local Part B MAC and have treated a railroad beneficiary. The system verifies the MAC enrollment, assigns the individual their new Railroad Medicare PTAN, and simultaneously creates the group PTAN.⁶

EDI Enrollment for Electronic Billing

The PTAN alone does not let you submit claims electronically. Once you have your Railroad Medicare PTAN, you enroll separately with Palmetto GBA’s Electronic Data Interchange system. Railroad Medicare is treated as its own jurisdiction, distinct from Palmetto GBA’s other lines of business, so you register a separate eServices account and receive a unique user ID for Railroad Medicare.⁷

The forms you submit depend on how you will bill:

For eServices portal access only, submit the EDI Enrollment Agreement.
For direct submission with no clearinghouse, complete the Direct Submitter forms and select a Network Service Vendor or Approved Vendor.
For billing services or clearinghouses, complete the Billing Service-Clearinghouse forms after confirming with the service which specific EDI functions they will handle.

Every path requires your Railroad Medicare PTAN, your NPI, and location demographic information. CMS requires version 5010 Base and Errata for all electronic transaction sets.⁸⁹

How To Know a Patient Is a Railroad Beneficiary

A railroad beneficiary’s Medicare Beneficiary Identifier looks identical to any other MBI. There is no special prefix or format. The tell is the physical Medicare card, which shows the Railroad Retirement Board logo in the upper left corner and the words “Railroad Retirement Board” printed at the bottom. You can also confirm electronically: an eligibility verification response returns the message “Railroad Retirement Medicare Beneficiary” in the 271 transaction or in the Additional Information field of the Palmetto GBA eServices portal.¹⁰

What Your Railroad Medicare Enrollment Does Not Cover

Palmetto GBA handles only Part B claims for railroad beneficiaries. Part A, home health, hospice, and durable medical equipment claims are processed by the relevant jurisdictional MACs, not by Palmetto GBA. Your Railroad Medicare PTAN and EDI enrollment do not apply to those claim types.¹¹

Keeping Your Enrollment Records Current

When any of your information changes, such as an address or a name, update your local MAC first. After you receive confirmation from the MAC, wait 7 to 14 business days before notifying Railroad Medicare so the update can flow through. Palmetto GBA’s website offers tools to verify and update enrollment records, and the web chat service can help in real time.⁴²

Where To Get Help With Enrollment

The Provider Contact Center is at 888-355-9165, Monday through Friday, 8:30 a.m. to 4:30 p.m. Eastern, Central, and Mountain, and 8:00 a.m. to 4:00 p.m. Pacific. Select option 3 for Provider Enrollment. Web chat handles provider enrollment, EDI, and eServices questions in real time.
