Passport Observation Page: What’s Printed and How to Fix Errors

The passport observation page is the section where the issuing government prints official notes about your passport or about you as the holder. In a U.S. passport these notes are called endorsements and sit on the page opposite your photo and biographical data. In a British passport they appear on page 3, printed in capital letters, and the page itself is labeled “observations.”¹ Whatever the label, the entries there carry the same legal weight as anything else in the booklet, and border officers do read them.

Where to Find It

International rules keep observations and endorsements off the biographical data page, which is reserved for your identity information. In a U.S. passport, endorsements are printed on what the State Department calls the Secretary’s message page, directly opposite the data page. If nothing has been added to your book, that page may just display the standard request for safe passage. In a UK passport, if no notes are needed, page 3 automatically prints “THERE ARE NO OFFICIAL OBSERVATIONS.”² Visa pages, which fill most of the rest of the booklet, are separate and reserved for entry stamps and visa stickers.

What Actually Gets Printed There

The U.S. State Department uses a formal system of numbered endorsement codes, each with fixed wording that the system prints into the passport.³ The entries you’re most likely to encounter fall into a handful of categories.

Limited Validity

When a passport is issued for less than the standard term, an endorsement explains why. A child adopted abroad might receive a passport that expires early and cannot be replaced without State Department authorization. Someone whose citizenship is under review might get a book valid only until that review concludes. The note lets the next official who handles the passport see the restriction immediately.

Diplomatic and Official Service

Diplomatic passports carry endorsements identifying the bearer’s role, ranging from general language such as “THE BEARER IS ABROAD ON A DIPLOMATIC ASSIGNMENT FOR THE UNITED STATES GOVERNMENT” to specific titles like ambassador or diplomatic courier. Family members of diplomats receive their own endorsements.³

Second Passport Books

Federal regulations generally prohibit holding more than one valid passport of the same type.⁴ When the State Department authorizes a second book anyway, typically because a first passport is tied up in visa processing or because stamps from one country would cause trouble at borders elsewhere, that second book carries an endorsement noting the exception. Second books are limited to four years of validity rather than the standard ten.³

Sex Offender Identifier

Federal law requires the State Department to include a unique visual identifier in the passport of any individual currently required to register as a sex offender after a conviction involving a minor. The statute bars issuing a passport without the identifier and authorizes revocation of any previously issued passport that lacks it.⁵ The endorsement remains as long as the person is on a registry.

Travel Restrictions

Courts or the State Department can limit where a passport can be used. The most restrictive endorsement reads “THIS PASSPORT IS VALID ONLY FOR RETURN TO THE UNITED STATES” with a specific expiration date, and it appears in situations including extradition, deportation, and passport revocation cases.³

UK Nationality and Identity Notes

British passports use the observation page somewhat differently. His Majesty’s Passport Office adds printed notes to explain the holder’s specific British nationality category or immigration status, clarify identity details such as hereditary titles, or indicate that the holder is on diplomatic or official service.²

Only the Issuing Authority Can Write There

Entries on the observation page can only be made by the passport-issuing authority. In the United States that is the State Department’s Bureau of Consular Affairs, including passport agencies, embassies, and consulates. In the UK it is His Majesty’s Passport Office. Airlines, foreign border officers, and private individuals have no authority to add anything to the page. Entries are usually placed at first issuance, at renewal, or when the holder requests an amendment that the authority approves.

If you write, draw, or stamp anything on the page yourself, you can invalidate the book. Federal regulations state that a passport becomes invalid when it “includes unauthorized changes, obliterations, entries or photographs” or has been materially changed in appearance, and the State Department can take possession of it or send written notice that it is no longer valid. If you cannot give a credible explanation for an altered or mutilated passport, the department can deny or restrict your next one.⁶ Replacing a damaged adult passport requires an in-person application on Form DS-11, the damaged book, and a signed statement explaining what happened, with an application fee of $130 plus a $35 facility acceptance fee.⁷

Novelty stamps from tourist sites are a common trap. They are not official government markings, and pressing them directly into your passport rather than onto a separate souvenir card can trigger the same invalidation rules. Use the separate cards those attractions offer.

Why to Check It Before Any Trip

Border officers and airline check-in staff flip to the endorsement or observation page to look for anything that restricts your travel, limits the passport’s validity, or flags a condition they need to act on. A passport endorsed as valid only for return to the United States will get you turned away at a departure gate for an international trip to a third country. A limited-validity notation might mean your passport expires before the six-month remaining-validity window that many countries require for entry.

Visa applications can be affected too. Some Schengen-area consulates have refused to accept passports when biographical information was changed through a manual observation rather than a newly issued book, on document-integrity grounds. If any note on your page modifies the information on your data page, ask the consulate before applying.

Open your passport and read the page before you travel. If something is printed there that you don’t recognize, contact the issuing authority before you go.

Fixing an Incorrect Entry or Data Error

If the State Department printed your name wrong, used the wrong sex marker, or made another data error, you can have it corrected at no cost using Form DS-5504, submitted by mail with the incorrect passport, a recent photo, and evidence of the correct information such as a certified birth certificate. No passport fee applies for error corrections while the passport is still valid.⁸

Timing changes what you get back. Report the error within one year of the passport’s issue date and the corrected adult passport will be valid for ten years. Report it after one year and the replacement only runs to the original expiration date.⁸

For a legal name change, the same DS-5504 mail process with no fee is available if both the passport was issued less than one year ago and the name change happened less than one year ago; include a certified marriage certificate, divorce decree, or court order.⁸ Outside those windows, you need a standard renewal on Form DS-82 by mail if eligible, or Form DS-11 in person, with adult book fees of $130 plus the $35 facility acceptance fee where DS-11 applies.⁷
