OSHA Overhead Crane Regulations: Inspections, Operation, and Penalties

OSHA’s overhead crane regulations, set out in 29 CFR 1910.179, govern who may run an overhead or gantry crane in a general industry workplace, how often the equipment must be inspected, how repairs and lockout must be handled, and how loads must be tested and marked. The standard is enforced with civil penalties that reach $16,550 for each serious violation and $165,514 for each willful or repeated violation under the 2025 schedule.1Occupational Safety and Health Administration. 2025 Annual Adjustments to OSHA Civil Penalties Most citations trace back to a small set of failures: missed or undocumented inspections, operators the employer cannot prove were qualified, and loads that exceeded the crane’s rated capacity.

Which Cranes the Standard Covers

Section 1910.179 applies to overhead and gantry cranes, including semigantry, cantilever gantry, wall cranes, and storage bridge cranes. These share a common design using trolleys with similar travel characteristics, whether the crane is fixed or mobile, manually operated or power-driven, so long as the hoisting mechanism is built into the machine itself.2eCFR. 29 CFR 1910.179 — Overhead and Gantry Cranes

Cranes used on construction sites are not covered by 1910.179. They fall under 29 CFR 1926 Subpart CC instead, which is a separate rule with its own operator certification requirements. If your worksite is construction rather than general industry, this article is not the right reference.

Who Can Operate the Crane

Only designated personnel may operate a crane covered by this standard. “Designated” has a specific meaning: the employer or the employer’s representative must select or assign the person as qualified to perform the duties involved.2eCFR. 29 CFR 1910.179 — Overhead and Gantry Cranes The regulation does not prescribe a mandatory curriculum or classroom hours, but the employer carries full responsibility for ensuring each operator actually knows how to run the crane safely.

In practice, training should cover the specific crane’s controls, safe hoisting techniques, standard hand signals, pre-shift inspection steps, and how to recognize common hazards like overloading and side-pulling. Employers who document operator proficiency through written tests and observed practical evaluations put themselves in a much stronger position during an OSHA inspection. Saying “we trained them” carries far more weight when there is paperwork to back it up.

Inspection Requirements

Cranes in regular service need two tiers of scheduled inspections: frequent and periodic. The intervals for each depend on how hard the crane works and the conditions it operates in.3Occupational Safety and Health Administration. 29 CFR 1910.179 – Overhead and Gantry Cranes

Frequent Inspections

Frequent inspections happen at intervals ranging from daily to monthly and target components exposed to routine wear. Every day, the operator or a designated inspector checks all functional operating mechanisms for maladjustment, air and hydraulic lines and valves for leakage, and hooks for visible deformation or cracks. Hoist chains and end connections are checked daily for excessive wear, twist, distorted links, or stretch beyond the manufacturer’s recommendations.

Hooks and chains get a monthly certification record on top of the daily visual check. That record must include the inspection date, the inspector’s signature, and the hook or chain’s serial number or other identifier. A hook must be pulled from service if it has cracks, more than 15 percent excess throat opening, or more than 10 degrees of twist from the plane of the unbent hook. Any deficiency identified during a frequent inspection must be evaluated for safety, and unsafe conditions must be corrected before the crane operates again.2eCFR. 29 CFR 1910.179 — Overhead and Gantry Cranes

Periodic Inspections

Periodic inspections are full examinations of the entire crane at intervals of one to twelve months depending on service severity. They cover everything in the frequent inspection plus additional items:3Occupational Safety and Health Administration. 29 CFR 1910.179 – Overhead and Gantry Cranes

  • Structural members for deformation, cracks, or corrosion
  • Loose bolts or rivets
  • Sheaves and drums for cracks or excessive wear
  • Pins, bearings, shafts, gears, rollers, and locking devices for wear, cracks, or distortion
  • Brake linings, pawls, and ratchets for excessive wear
  • Load, wind, and other indicators tested across their full range for accuracy
  • Gasoline, diesel, or electric power plants verified for proper performance
  • Chain drives for sprocket wear and chain stretch
  • Controller contactors, limit switches, and pushbutton stations for pitting or deterioration

Wire Rope Inspections

All running ropes must be thoroughly inspected at least once a month, and the employer must keep a certification record showing the date, the inspector’s signature, and an identifier for the ropes checked. That record must be accessible to appointed personnel.2eCFR. 29 CFR 1910.179 — Overhead and Gantry Cranes Inspectors watch for conditions that could mean an appreciable loss of original strength:

  • Rope diameter falling below nominal due to core support loss, corrosion, or outside wire wear
  • Broken outside wires, particularly concentrated in one area
  • Worn outside wires
  • Corroded or broken wires at end connections
  • End connections that are corroded, cracked, bent, worn, or improperly applied
  • Severe kinking, crushing, cutting, or unstranding

Any rope that has been idle for a month or more must get a thorough inspection before use, and an appointed person must approve it before it goes back into service.2eCFR. 29 CFR 1910.179 — Overhead and Gantry Cranes

Idle Crane Inspections

Cranes that have been sitting need inspection before they go back to work, and the scope depends on how long the crane has been idle. A crane idle for one month to less than six months needs a full frequent-level inspection plus a rope inspection. A crane idle for six months or more needs the frequent and periodic checklists plus a rope inspection. Standby cranes must be inspected at least every six months using the frequent inspection checklist plus a rope inspection, even if they have not been used.3Occupational Safety and Health Administration. 29 CFR 1910.179 – Overhead and Gantry Cranes

This is a common failure point. A crane that sat through a slow season cannot simply be powered on and put back to work. The appropriate inspection must be completed and documented first.

Load Testing and Rated Load Marking

Before initial use, all new or altered cranes must go through an operational test covering hoisting, lowering, trolley travel, bridge travel, and limit switches. After the operational test, the crane must pass a rated load test to confirm structural integrity. Test loads cannot exceed 125 percent of the rated load unless the manufacturer recommends otherwise, and test reports must be kept on file where appointed personnel can access them.2eCFR. 29 CFR 1910.179 — Overhead and Gantry Cranes

When a crane is modified or re-rated, a qualified engineer or the equipment manufacturer must thoroughly check the structural components and supporting structure for the new rated load, and the crane needs a new load test before returning to service.3Occupational Safety and Health Administration. 29 CFR 1910.179 – Overhead and Gantry Cranes

The rated load must be plainly marked on each side of the crane. If the crane has more than one hoisting unit, each hoist must have its rated load marked on it or on its load block, legible from the ground or floor.2eCFR. 29 CFR 1910.179 — Overhead and Gantry Cranes

Maintenance, Repairs, and Lockout

Before any repair work begins, the crane must be moved to a location that minimizes interference with other operations. All controllers must be placed in the off position, and the main or emergency switch must be opened and locked out. Hook repairs deserve extra care. Welding or reshaping a crane hook is not generally recommended under the regulation. If an employer does attempt such a repair, it must be done under competent supervision, and the hook must pass a rated load test before returning to service. Safe Operating Rules

Operators may not load the crane beyond its rated capacity except during required load testing. Before lifting, the operator must confirm the load is properly secured and balanced, and that the hoist rope is free of kinks. Several other rules govern day-to-day operation:

  • The operator should avoid carrying loads over personnel.
  • The operator must test the brakes each time a load approaching the rated capacity is handled.
  • Once a load is suspended, the operator must remain at the controls.
  • Every crane with a power traveling mechanism (except floor-operated models) must have a gong or other effective warning signal.2eCFR. 29 CFR 1910.179 — Overhead and Gantry Cranes
  • At the start of each shift, the operator must test the upper limit switch on each hoist under no load, inching the block into the limit or running at slow speed. Hand signals become mandatory when the operator cannot see the load or the point of operation. A designated signal person must be used in those situations, and both the operator and the signal person need to know the standardized signals.

Clearance and Electrical Safeguards

The regulation requires minimum clearances of three inches overhead and two inches laterally between the crane and any obstruction.3Occupational Safety and Health Administration. 29 CFR 1910.179 – Overhead and Gantry Cranes Tight clearances are a common hazard in older facilities where building modifications may have encroached on the crane’s travel path since it was originally installed.

Runway conductors of the open type must be positioned or guarded so that anyone entering or leaving the cab or crane footwalk cannot come into contact with them. Guards must also prevent bridge conductors from touching hoisting ropes if the two could come into contact.2eCFR. 29 CFR 1910.179 — Overhead and Gantry Cranes

Outdoor and High-Wind Requirements

Outdoor storage bridge cranes must be equipped with automatic rail clamps and a wind-indicating device that gives a visible or audible alarm at a predetermined wind velocity.2eCFR. 29 CFR 1910.179 — Overhead and Gantry Cranes If the rail clamps grip the rail heads, any beads or weld flash on the heads must be ground off so the clamps can hold.

For rail-mounted bridge and portal cranes operating outdoors, OSHA has interpreted the standard to require wind-indicating devices that transmit warnings at both a warning speed, where gantry travel stops and the crane prepares for shutdown, and a shutdown speed, where work stops entirely and the crane is secured against travel using every available means. Operating instructions for high wind conditions must be posted in the operator’s cab, and the employer must monitor local weather through a weather service or equivalent method.5Occupational Safety and Health Administration. Slings and Rigging

Loads must be attached to the load block hook using slings or other approved lifting devices, and hooks must not be loaded beyond the manufacturer’s recommendations.3Occupational Safety and Health Administration. 29 CFR 1910.179 – Overhead and Gantry Cranes Slings themselves fall under a separate standard, 29 CFR 1910.184, which adds its own inspection and removal requirements.

Every sling and its fastenings must be inspected by a competent person each day before use, and damaged slings must be pulled immediately. Alloy steel chain slings need a thorough periodic inspection at intervals no greater than twelve months, examining wear, defective welds, deformation, and increased length.6Occupational Safety and Health Administration. 29 CFR 1910.184 – Slings

Wire rope slings must be removed from service immediately for any of the following:

  • Ten randomly distributed broken wires in one rope lay, or five broken wires in one strand in one rope lay
  • Wear or scraping of one-third the original diameter of outside individual wires
  • Kinking, crushing, bird caging, or other distortion of the rope structure
  • Evidence of heat damage
  • Cracked, deformed, or worn end attachments
  • Hooks opened more than 15 percent of the normal throat opening or twisted more than 10 degrees
  • Corrosion of the rope or end attachments

Chain slings must be permanently removed from service if heated above 1,000 degrees Fahrenheit, and pulled if chain size at any point falls below the minimum listed in OSHA’s reference tables.6Occupational Safety and Health Administration. 29 CFR 1910.184 – Slings

Reporting Crane Incidents

A crane accident that results in a worker fatality must be reported to OSHA within eight hours. If a worker suffers an in-patient hospitalization, amputation, or loss of an eye, the employer has 24 hours to report.7Occupational Safety and Health Administration. OSHA’s Recordkeeping Requirements These deadlines apply to all employers regardless of company size, and missing them can result in a separate citation on top of whatever caused the incident.

Workers who report crane safety violations or refuse to operate unsafe equipment are protected from retaliation under Section 11(c) of the Occupational Safety and Health Act. Protected workers include all private-sector employees and U.S. Postal Service employees. Retaliation can include firing, demotion, pay cuts, reduced hours, threats, or harassment. Penalties for Noncompliance

OSHA adjusts its civil penalty amounts annually for inflation. As of the adjustment effective January 15, 2025, the maximum penalties are:1Occupational Safety and Health Administration. 2025 Annual Adjustments to OSHA Civil Penalties

  • Serious violation: up to $16,550 per violation
  • Other-than-serious violation: up to $16,550 per violation
  • Willful or repeated violation: up to $165,514 per violation

The most commonly cited crane violations involve skipped or poorly documented inspections, unqualified operators, missing documentation of operator qualifications, and exceeding rated load capacity. Each individual violation can carry its own penalty, so a single OSHA inspection that uncovers several deficiencies can add up quickly. A willful violation, where the employer knew about the hazard and did nothing, costs ten times what a serious violation does. That multiplier is what makes a well-run documentation and compliance program far cheaper than the alternative.
