Nursing loan repayment programs can wipe out most of your nursing school debt in exchange for working two or three years at a facility that struggles to recruit. The two largest federal options are the Nurse Corps Loan Repayment Program, which pays up to 85% of your qualifying balance over three years, and the National Health Service Corps Loan Repayment Program, which pays up to $75,000 for a two-year commitment and is entirely tax-free. Beyond those, there are programs for nurses who serve tribal communities, teach nursing, work for any qualifying public or nonprofit employer long enough, or join the military. Each has its own rules about which loans count, how awards are taxed, and what you owe back if you leave early.
Nurse Corps Loan Repayment Program
The Nurse Corps LRP is the federal program built specifically for nurses. It covers registered nurses, advanced practice registered nurses, and nurse faculty working at facilities designated as Critical Shortage Facilities. You need at least an associate or baccalaureate degree in nursing and must agree to serve for a minimum of two years.1Office of the Law Revision Counsel. 42 USC 297n – Loan Repayment and Scholarship Programs
The initial two-year contract pays 60% of your total outstanding qualifying nursing education debt, split as 30% each year. Complete those two years, sign a continuation contract for a third year, and you can receive another 25% of your original balance. The ceiling is 85%.2Health Resources and Services Administration. Non-faculty nurses must work at least 32 hours per week for a minimum of 45 weeks per service year, and at least 25% of those weekly hours (8 of 32) must be direct patient care. The rest can be administrative, teaching, or other non-clinical duties.2Health Resources and Services Administration. NHSC Loan Repayment Program
The National Health Service Corps LRP covers a broader range of health professionals. Among nurses, certified nurse practitioners and certified nurse midwives are eligible. You must work at an NHSC-approved site inside a Health Professional Shortage Area.3Office of the Law Revision Counsel. 42 USC 254l-1 – National Health Service Corps Loan Repayment Program
Full-time participants in primary care roles can receive up to $75,000 for an initial two-year commitment. Non-primary-care providers can receive up to $50,000. Applicants proficient in Spanish may qualify for enhanced awards of up to $80,000 (primary care) or $55,000 (non-primary care). After the initial contract, you can apply for one-year continuation contracts, though that funding isn’t guaranteed.4National Health Service Corps. NHSC Loan Repayment Program
Selection is competitive. NHSC reviews applications in descending order of Health Professional Shortage Area score, so working at a site with a higher shortage score improves your odds. Each area receives a numerical score based on provider-to-population ratios, poverty levels, and other local factors. If two applicants are otherwise equal, the one at the higher-scored site wins.5National Health Service Corps. Understanding Loan Repayment Funding Priorities
Indian Health Service Loan Repayment
Nurses working in facilities that serve American Indian and Alaska Native communities can apply for the Indian Health Service Loan Repayment Program. It offers up to $50,000 for an initial two-year commitment. Unlike some other federal programs, IHS participants can extend their contract annually until their qualified debt is fully paid, which makes this one of the more generous long-term options for anyone willing to work in tribal health.6Indian Health Service. Nurse Faculty Loan Program
If you plan to teach, the Nurse Faculty Loan Program cancels debt in exchange for classroom time. The loan has to come through a participating school’s NFLP fund. Once you work as a full-time nursing faculty member, the federal government cancels 20% of your balance plus accrued interest for each completed year of teaching. Five years of full-time teaching wipes out the entire loan. The minimum commitment is four years.7GovInfo. 42 USC 297n – Loan Repayment and Scholarship Programs
Public Service Loan Forgiveness
PSLF works differently from the service programs above. Instead of a short commitment with large upfront payments, PSLF forgives whatever federal Direct Loan balance is left after you make 120 qualifying monthly payments while working full-time for a qualifying employer. That’s effectively 10 years, and the payoff can be large if you carry a high balance on an income-driven plan where monthly payments stay low.8eCFR. 34 CFR 685.219 – Public Service Loan Forgiveness Program
Qualifying employers include federal, state, local, and tribal government organizations, plus nonprofits with 501(c)(3) status. Other nonprofits can qualify if a majority of their full-time staff provides qualifying public services.9Federal Student Aid. Public Service Loan Forgiveness Most hospitals, public health departments, and community health centers meet these criteria. For nurses, PSLF has the broadest workplace eligibility because it doesn’t require a shortage area.
You don’t have to be on an income-driven plan. Payments under the 10-year standard plan count, as do payments under any plan where the monthly amount at least matches the 10-year standard. The practical problem: if you pay on the standard 10-year plan for 120 months, the loan is already gone when you’d qualify for forgiveness. Most PSLF participants pick an income-driven plan so lower monthly payments leave a larger balance to be cancelled.8eCFR. 34 CFR 685.219 – Public Service Loan Forgiveness Program
Don’t skip the paperwork. Submit the PSLF employment certification form annually and whenever you change employers. The form confirms your employer qualifies and that your payments are counting. Nurses who wait until year 10 to submit everything often find payments that didn’t count, which can add years to the timeline.10Federal Student Aid. Changes for New Borrowers After July 2026
If you take out new federal student loans on or after July 1, 2026, your PSLF-eligible repayment plan options narrow. Only the Repayment Assistance Plan (RAP) qualifies for PSLF under the new framework. Borrowers with no new loans after that date can still use existing income-driven plans like ICR, PAYE, or IBR. If you’re in nursing school now and expect to borrow after mid-2026, pay close attention to which plan you enroll in once repayment starts.
Military Loan Repayment Programs
Each branch runs its own nurse loan repayment incentive. These programs tend to offer the largest annual payments, but they come with commissioning requirements, fitness standards, and multi-year active-duty obligations that run well past a civilian service commitment.
The Army’s Active Duty Health Professions Loan Repayment Program pays up to $40,000 per year for a maximum of three years. The active-duty obligation runs at least two years, and if you also receive special pay, the commitment extends to seven. The Army Reserve version pays up to $20,000 per year with a total cap of $60,000.11U.S. Army Recruiting Command. Army Nurses
The Navy’s Health Professions Loan Repayment Program pays up to $40,000 per year to active-duty nurses, but roughly 22% is withheld for federal income tax before the payment reaches your lender. Navy participation adds two years to your existing service obligation rather than running at the same time, so read the terms before you sign.12Navy Medicine. Health Professions Loan Repayment Program
State-Level Loan Repayment Programs
Most states run their own nursing loan repayment or forgiveness programs, usually through the state health department. These require an active, unencumbered license in that state and a two- to four-year commitment in a rural or underserved community. Award amounts vary widely. Some states offer around $10,000; others provide $50,000 or more across the full term.
Priorities differ too. Some state programs focus on primary care; others target specialties with acute shortages like psychiatric or geriatric nursing. Many are funded in part through the Public Health Service Act, which carries the tax benefit covered below. Because eligibility rules, award amounts, and application windows change by state, check with your state health department or the HRSA website for the program in your jurisdiction.
Which Loans Qualify
Not every student loan is eligible, and this is where applications fail. The Nurse Corps LRP accepts both government and private (commercial) loans used for tuition, reasonable educational expenses, and living costs while in nursing school. A consolidated or refinanced loan still qualifies as long as it contains only your eligible nursing education debt and nothing else.2Health Resources and Services Administration. Nurse Corps Loan Repayment Program Fiscal Year 2026 Application and Program Guidance
These loans do not qualify for Nurse Corps:
- Parent PLUS loans, even if they paid for your nursing education.
- Debt from a prior degree in another field.
- Consolidated loans containing any ineligible debt. Mix in a loan from a non-nursing degree or someone else’s loan and the whole consolidation is disqualified.
- Perkins Loans and Nurse Faculty Loan Program loans that are subject to their own cancellation provisions, unless you can document otherwise.
- Credit card debt and personal lines of credit.
- Loans from family members or unregulated lenders. The lender must be subject to federal or state examination and supervision.
PSLF uses a different standard. It applies only to federal Direct Loans. Older FFEL or Perkins balances need to be rolled into a Direct Consolidation Loan before any payments can count toward the 120.
Tax Treatment of Awards
Not all repayment awards are taxed the same way, and the difference can be thousands of dollars.
NHSC Loan Repayment Program payments are entirely tax-free. The IRS excludes them from gross income, so a $75,000 award is worth the full $75,000. State loan repayment programs funded through the Public Health Service Act or designed to increase health services in underserved areas receive the same tax-free treatment.13Internal Revenue Service. Publication 970, Tax Benefits for Education
Nurse Corps LRP payments are fully taxable. HRSA withholds federal income tax and FICA before sending payments to your lender, and you’ll receive a W-2 for the amounts. State and local income tax may apply too, depending on where you live.2Health Resources and Services Administration. Nurse Corps Loan Repayment Program Fiscal Year 2026 Application and Program Guidance If you’re eligible for both NHSC and Nurse Corps, the tax difference alone can make the smaller NHSC number worth more in your pocket.
One trade-off on the tax-free side: you cannot deduct as student loan interest any interest paid through NHSC or similar repayment assistance programs.13Internal Revenue Service. Publication 970, Tax Benefits for Education
PSLF forgiveness has historically been tax-free at the federal level. Forgiveness under income-driven repayment plans is temporarily tax-free through 2025 but may become taxable again. Military loan repayment is generally taxable; the Navy, for example, withholds roughly 22% before sending payments to lenders.12Navy Medicine. Health Professions Loan Repayment Program
What Happens If You Break Your Service Commitment
Leaving a service contract early is not just a lost future payment. Federal programs impose financial penalties for breach, and the numbers can be steep.
NHSC Breach
If you fail to begin or complete your NHSC full-time obligation, you owe three things: the total loan repayment amounts already paid on your behalf for the period you didn’t serve, a penalty of $7,500 for each month you didn’t complete, and interest on both at the maximum legal prevailing rate from the date of breach. Total recovery cannot be less than $31,000 regardless of the math.14Office of the Law Revision Counsel. 42 USC 254o – Breach of Scholarship Contract or Loan Repayment Contract Half-time participants face the same structure at $3,750 per unserved month.15National Health Service Corps. Fiscal Year 2026 NHSC Loan Repayment Program Application and Program Guidance
Nurse Corps Breach
Nurse Corps breach works differently. If you fail to begin or complete your two-year commitment, you repay all loan repayment amounts received, including the amounts withheld for federal taxes, plus interest at the maximum legal prevailing rate. Any outstanding debt becomes due within three years of the breach. After three years, the debt is treated as delinquent and additional charges apply. A breach may also permanently disqualify you from future Nurse Corps awards and other HRSA and federal programs.2Health Resources and Services Administration. Documents and How to Apply
Most federal programs ask for the same core paperwork. Gather it before you open the application:
- Employment verification with the facility name, its Health Professional Shortage Area score if applicable, and a point of contact who can confirm your work.
- Loan details: servicer names, current balances, and account numbers. Confirm every balance matches what shows in the National Student Loan Data System at studentaid.gov, because mismatches delay processing.
- Your state nursing license number and expiration date.
- Income documentation. Recent tax returns or W-2s. When an application asks for income, use the adjusted gross income from your most recent Form 1040.16Internal Revenue Service. Adjusted Gross Income
- PDF transcripts from your nursing school showing the degree earned.
Both Nurse Corps and NHSC applications run through the HRSA Bureau of Health Workforce online portal. You create an account, work through each section, upload documents as PDFs, and sign electronically. HRSA publishes a detailed checklist for NHSC applicants that’s worth downloading before you start.17Health Resources and Services Administration. NHSC Loan Repayment Programs Application Checklist
After submitting, you get a confirmation email with a tracking number. Processing takes several months.
