What the Form Does

You must finish your Initial Active Duty for Training. That means completing both basic training and your advanced individual training or equivalent technical school. You also need to remain in good standing with your unit. Unexcused drill absences or failing to meet administrative and readiness standards can suspend your eligibility.
How to Request the Form From Your Unit

Check the Form Before You Walk Away
Before you leave the personnel office with a printed copy, read every field against your own records. The items most likely to trip up your VA claim later are:
