Documents You Need
Start with Form SS-5, the Application for a Social Security Card. You can download it from ssa.gov or pick one up at any local SSA office.3Social Security Administration. Proof of Identity
You need one current, unexpired document that shows your name and identifying information:
- U.S. driver’s license
- State-issued non-driver identification card
- U.S. passport

Proof of Citizenship or Immigration Status
U.S. citizens can use a birth certificate, U.S. passport, Certificate of Naturalization, or Certificate of Citizenship. Evidence of the Correct Name
If the misspelling is a plain typo and your legal name never actually changed, a birth certificate or passport showing the correct spelling is enough to establish what your name should be.
If your name changed through marriage, divorce, or court order and that’s why the card is wrong, bring proof of that change: a marriage certificate, a divorce decree that states your new name, a Certificate of Naturalization showing the new name, or a court order approving the change.4Social Security Administration. In Person
Take your completed Form SS-5 and originals to your local SSA office; find the nearest one through the office locator at ssa.gov. This is the safer route, because a representative can review everything on the spot, flag anything missing, and hand your originals back after copying them.
By Mail
Mail Form SS-5 and your original documents to your local office; the mailing address is available through the same office locator.3Social Security Administration. Online Isn’t an Option Here

What Arrives, and When
Once SSA has a complete application, the corrected card should arrive by mail in about two weeks.7Social Security Administration. Does This Count Against Your Replacement Card Limit?
Federal regulations cap you at three replacement Social Security cards per year and ten per lifetime. Verified legal changes to a first name or surname do not count toward either limit.10Social Security Administration. Update Everyone Else Once the Card Arrives
The corrected card doesn’t fix records elsewhere on its own. The IRS recommends contacting your employer right away to update payroll so your next W-2 reflects the correct name, and asking for a corrected Form W-2c if a W-2 was already issued with the wrong spelling.1Internal Revenue Service. Beyond taxes and payroll, update your bank, your state DMV for your driver’s license, your insurance providers, and any other institution that has your Social Security number on file. Most will want to see the new card along with another form of ID.
