Money Order Affidavit Form for Lost or Stolen Orders

To recover funds from a money order that was lost or stolen, you file a money order affidavit or inquiry form with the company that issued it, pay a processing fee, and wait for that company to confirm the money order has not been cashed before it releases your refund. The specific form, fee, and waiting period depend on whether the money order was issued by USPS, Western Union, or MoneyGram, and the name of the issuer is printed on the money order itself, not necessarily the store where you bought it.

What to Gather Before You File

The most important item is the original purchase receipt. It carries the serial number the issuer uses to trace the money order through its system. Without that number, the search is slower, more expensive, and less certain.

Also collect the exact purchase date, the store or post office where you bought it, the face amount, and the full names and addresses of both you and the intended payee. Have all of this in front of you before you start the form. Missing details are the most common cause of delayed claims.

Filing a USPS Money Order Inquiry

For a postal money order, the form is PS Form 6401, the Money Order Inquiry. You can download it from the USPS website or pick one up at any post office. Fill in the serial number from your receipt, the purchase date, the post office number, and the amount, then sign it and present it to a postal employee along with your customer receipt.1United States Postal Service. The processing fee is $21 for a lost or stolen money order. There is no charge if the money order was only damaged and you still have it.2United States Postal Service. Sending Money Orders

USPS will not issue a refund until at least 60 days have passed since the money order’s issue date. That waiting period applies even if you file the inquiry the day the money order goes missing. If the trace shows the money order was already cashed, you receive a photocopy of it instead of a refund.1United States Postal Service. PS Form 6401 – Money Order Inquiry

When a trace shows the money order has been cashed, the claim shifts from a routine refund into a possible forgery or fraud investigation.

USPS sends you a photocopy of the cashed money order showing the endorsement. That copy is your starting point for identifying a forged signature and filing a police report.1United States Postal Service. PS Form 6401 – Money Order Inquiry

Western Union has a formal forgery claim process. You submit the notarized Purchaser’s Affidavit with supporting documents such as a police report, a copy of the money order before it was altered, or invoices showing the original debt. Everything must be mailed within 11 months of the date the money order was cashed. Western Union then submits a collection request to the bank that first deposited the money order. The average forgery claim takes 90 to 120 days to resolve, and recovery is not guaranteed because the depositing bank makes the final call on whether to return the funds.3Western Union.
