The Manulife withdrawal form you need depends on the account you hold: for a workplace group retirement plan (RRSP, DPSP, TFSA, or non-registered savings), it’s form GP0766, and if you want up to $50,000 you can often skip the paper entirely and request the withdrawal through your online member portal.1Manulife. Manulife FAQ Central Individual life insurance policyholders use different forms depending on whether they want a partial withdrawal, a policy loan, or a full surrender. The form you pick, the tax withheld, and the documents you attach all follow from your account type, so confirm that first.
Pick the Form That Matches Your Account
Your plan name and contract number appear on your benefits card and claim statements, and they tell you which form applies.1Manulife. Manulife FAQ Central Submitting the wrong one is the fastest way to have your request bounced back.

Withdraw Online If You Can
For group retirement accounts, requesting the withdrawal online is faster than mailing a form and can cut processing time roughly in half.5Manulife. Manage My Money Not every plan supports it. To check, sign in, go to “More account options,” click “My Profile,” and select “View My Plan Material.”1Manulife. Manulife FAQ Central
When online is available, sign in, open “More account options,” click “My Account,” and select “Make a Withdrawal.” Follow the prompts through to submission. Online withdrawals are capped at $50,000.1Manulife. Manulife FAQ Central If you need more than that, or your plan doesn’t allow online requests, you’ll have to submit the paper GP0766.
Filling Out the GP0766
The paper form asks for your plan contract number, member certificate number, the dollar amount you want withdrawn, and your banking details for direct deposit. Have all of it in front of you before you start. Errors in these fields are the most common reason a form gets returned.
Banking Details
The simplest way to give Manulife your deposit information is to attach a void cheque that shows your name and address.2Manulife. Withdrawal Request If you don’t have one, use a bank letter or fill in the fields manually: your Canadian financial institution’s name, transit number, institution number, and account number. The form uses Canadian banking identifiers, not U.S. routing numbers.
Identity Verification
Manulife requires valid, unexpired, original Canadian government-issued photo identification (federal or provincial) to verify who you are.6Manulife Bank. Manulife Bank Identification Verification Form and Signature Card Your plan contract and member certificate numbers link the request to your account. A service representative may also ask for your Social Insurance Number for tax reporting.
Signing
You sign the form in the designated area to authorize the withdrawal. If your bank account is joint and the financial institution requires both signatures, both holders sign. Joint accounts differ: an “OR” account lets one owner act alone, while an “AND” account needs every owner’s signature before Manulife will process the request.7Manulife Bank. Where to Send the Paper Form
With the form signed and your void cheque or bank letter attached, mail or fax it to the address matching your province:2Manulife. Withdrawal Request
- Outside Quebec: Manulife, Group Retirement, P.O. Box 396, Waterloo, ON N2J 4A9. Fax: 1-866-945-5110.
- Quebec: Manulife, Group Retirement, 2000 Mansfield, Suite 1410, Montréal, QC H3A 3A2. Fax: 1-866-945-5109.
Individual insurance policyholders using NN0387 or NN0522 should follow the submission instructions printed on those forms, which may point to a different address. Many individual insurance requests can also be handled through the personal insurance portal after signing in at manulife.ca.4Manulife. Individual Insurance – Support
How Long It Takes
Timing depends on how you submitted the request and what you’re withdrawing.5Manulife. Manage My Money
- Online request: the transaction shows in your account history within 3 business days, and you receive the money in up to 10 business days.
- Paper form filled out correctly: a cheque arrives in the mail in about 10 business days. If nothing arrives within 15 business days of mailing, check the Transaction History section of your online account.
- Stock fund withdrawals: about 4 business days by direct deposit. Share sales settle two business days after the trade, and payment releases only after settlement, so the money lands on or after the third business day.
Tax Withheld on RRSP Withdrawals
When you withdraw from an RRSP, Manulife withholds tax and remits it before you receive the balance. The rate depends on the amount, not your income:8Canada Revenue Agency. Tax Rates on Withdrawals
- Up to $5,000: 10% withheld (5% in Quebec).
- $5,001 to $15,000: 20% withheld (10% in Quebec).
- Over $15,000: 30% withheld (15% in Quebec).
The withheld amount is not necessarily the final tax bill. The withdrawal is added to your taxable income for the year, and you may owe more at filing time if withholding didn’t cover your marginal rate. You also permanently lose the RRSP contribution room you used. TFSA withdrawals, by contrast, are not subject to withholding tax or added to taxable income, and the room comes back the following calendar year.9Manulife. RRSP vs TFSA Saving Guide Non-registered withdrawals may trigger capital gains tax on any realized investment gains.
Locked-In Accounts Are Different
If your Manulife account holds locked-in funds transferred from a registered pension plan into a locked-in RRSP (LRSP) or life income fund (LIF), you generally can’t withdraw money freely, and the standard GP0766 alone won’t do it. Federal or provincial pension legislation restricts access until retirement, with limited exceptions for financial hardship (low expected income or high medical or disability-related costs) and small balances (turning 55 or older with all locked-in accounts totalling $37,300 or less in 2026).10OSFI. Unlocking Funds From a Pension Plan or From a Locked Retirement Savings Plan
If you have a spouse or common-law partner and you’re unlocking under either provision, your spouse or partner must sign a consent form, typically Form 2: Attestation Regarding Spouse/Common-Law Partner.10OSFI. Unlocking Funds From a Pension Plan or From a Locked Retirement Savings Plan For transfers to a LIF, Manulife requires the spouse’s written consent signed outside the immediate presence of the plan member and witnessed independently.11Manulife. Spousal Waiver Separated but not divorced? Your spouse’s consent is still needed unless a court order or separation agreement clearly gives up their right to a share. A former common-law partner you no longer live with is not a spouse under the federal Pension Benefits Standards Act and doesn’t need to consent.
Insurance Policies: Watch for Surrender Charges
Withdrawing from or surrendering a Manulife life insurance policy with a cash value component may trigger surrender charges, depending on how long you’ve held the policy. They’re highest in the early years and decline over time, letting the insurer recover upfront costs. Your policy contract sets the exact schedule, and the NN0387 surrender form flags that charges may apply.
Before surrendering a whole life or universal life policy, consider whether a policy loan on form NN0522 fits better. Loans against cash value don’t trigger surrender charges or an immediate tax event, though the unpaid balance reduces the death benefit and accrues interest.
