Lifeline annual recertification is the yearly check that confirms you still qualify for the federal phone and internet discount. Most subscribers are cleared automatically through a database match and never have to do anything. If that automated check can’t confirm your eligibility, USAC sends you a notice, and from that date you have 60 days to submit FCC Form 5630 with proof that you still qualify.1eCFR. 47 CFR 54.405 – Carrier Obligation to Offer Lifeline Miss that deadline and your carrier is required to remove you from the program.
When You Don’t Have to Do Anything
Before any notice goes out, USAC runs every subscriber through an automated database check. If the system can confirm you still participate in a qualifying program or still meet the income threshold, your recertification is handled behind the scenes.2Universal Service Administrative Company. Recertification Plenty of long-time subscribers have never filled out a recertification form for this reason.
You’ll know the automated check failed because USAC will contact you. The notice arrives by email or postal mail, and follow-up reminders may come as pre-recorded phone messages.3Universal Service Administrative Company. Recertify Once you receive one, the 60-day clock is running.
What to Gather Before You Start
You’ll need your full legal name as it appears on your Social Security card or state ID, your date of birth, and the last four digits of your Social Security number.4Universal Service Administrative Company. Lifeline Program Annual Recertification Form Nicknames won’t match federal records.
What you need after that depends on how you qualify.
If you qualify through a federal assistance program such as SNAP, Medicaid, SSI, Federal Public Housing Assistance, or Veterans Pension benefits, bring a document proving current participation. A benefit award letter, statement of benefits, benefit verification letter, or a screenshot from your online benefits portal all work. The document must show your name, the program name, the issuing agency, and either an issue date within the last 12 months or a future expiration date.5Federal Communications Commission.
