The “LadySpineDoc lawsuit” that has circulated on social media is not actually a lawsuit. Dr. Betsy Grunch, the Gainesville, Georgia neurosurgeon who posts as @ladyspinedoc, has been under a U.S. Department of Justice investigation since at least mid-2025 over her relationships with medical device companies, but no charges have been filed and no civil suit has been made public. Grunch denies any wrongdoing.1Gainesville Times. DOJ Investigating Betsy Grunch, a Gainesville Neurosurgeon and Social Media Influencer
What the DOJ Is Investigating
Reporting by the Gainesville Times in August 2025 identified the focus of the inquiry as Grunch’s professional relationship with Spineology, a St. Paul, Minnesota-based manufacturer of spinal implants, and potentially other device companies.1Gainesville Times. DOJ Investigating Betsy Grunch, a Gainesville Neurosurgeon and Social Media Influencer Investigators are examining potential violations of the federal Anti-Kickback Statute, which makes it a felony to knowingly accept payments or anything of value in exchange for referring patients to products or services reimbursed by Medicare or Medicaid.2HHS Office of Inspector General. Federal Anti-Kickback Statute
According to reporting on the investigation’s focus, the question is whether Grunch or other Longstreet Clinic physicians accepted improper payments from device companies for procedures or products that were not medically necessary.3Primetimer. Anti-Kickback violations are felonies carrying up to five years in prison and fines up to $25,000 per violation, plus potential exclusion from Medicare and Medicaid and civil liability under the False Claims Act for up to three times the government’s losses. The statute reaches arrangements where “even one purpose” of a payment is to influence referrals.2HHS Office of Inspector General. How the Investigation Became Public
The probe surfaced when Northeast Georgia Health System, which had announced plans in April 2025 to acquire the Longstreet Clinic, paused the deal in August 2025. NGHS said it had received a civil investigative demand from the DOJ notifying it that an “unknown number of Longstreet physicians” were under federal investigation.6Gainesville Times. The Gainesville Times later obtained an internal NGHS memo dated August 13, 2025, sent by the health system’s legal department and marked in large red font as highly confidential. The memo instructed NGHS staff to preserve all documents related to the DOJ investigation involving Grunch.7Gainesville Times. The merger resumed in December 2025, with NGHS targeting an early 2026 close. Officials declined to say exactly when work restarted or to comment on the investigation’s role in the pause.8AccessWDUN. NGHS-Longstreet Merger Resuming, Aiming for Early 2026
The Industry Payments at Issue
Publicly available data from the CMS Open Payments program, as compiled by ProPublica, shows Grunch received $42,271 in disclosed industry payments in 2018, spread across 17 companies and 42 separate transactions. Consulting fees made up the largest share at $32,400, followed by $7,894 in travel and lodging and $1,978 in food and beverage. The single largest product-related payment was $17,250 tied to the Simmetry Sacroiliac Joint Fusion System made by RTI Surgical. Spineology, the company at the center of the DOJ inquiry, accounted for $8,308 linked to its Optimesh graft containment product.9ProPublica. What Grunch and Longstreet Have Said
On August 25, 2025, Longstreet Clinic posted on social media that it “is not being investigated by the U.S. Department of Justice.” Asked whether individual physicians at the clinic were under a federal probe, a spokesperson declined to comment.10Gainesville Times. Two days later, the clinic confirmed that Grunch “remains employed” and said the inquiry did not affect her ability to see patients, practice medicine, or hold her license and board certifications. Longstreet also said it had not received a civil investigative demand or any investigative documents from the DOJ or any other agency, and declined further comment on that basis.11AccessWDUN. The TikTok Feud That Spread the Story
The investigation reached a much broader audience in early 2026 through a public clash on TikTok between Grunch and Savannah Sparks, a pharmacist who posts as @Rx0rcist. Sparks criticized Grunch for promoting GLP-1 weight-loss medications and nutritional supplements associated with author Mel Robbins, arguing Grunch was not transparent about the drugs and that endorsing products from a non-physician was ethically questionable.3Primetimer. Sparks also raised the federal investigation and publicly challenged Grunch to explain why she was allegedly no longer able to operate at local hospitals. Grunch responded on TikTok by calling Sparks an “Only Fans star” and writing: “So you listen to stories as told by an OF creator. Got it.” Sparks answered with a video criticizing Grunch for attacking her personally rather than addressing the hospital-privileges question, and followed up on International Women’s Day in March 2026 by wearing a printout of Grunch’s comment on camera.12India Times.
