To fill out IMM 5562, the Supplementary Information: Your Travels form, you list every international trip you have taken in the last ten years, and you do the same for your spouse or common-law partner and each dependent child aged 18 or older, whether or not they are coming with you to Canada.1Immigration, Refugees and Citizenship Canada. Whose Travel History Goes on the Form
You fill in your own travel history, your spouse or common-law partner’s, and the history of any dependent child who is 18 or older. Each person’s trips go into a separate section so it is clear whose is whose. Dependent children under 18 are not listed.
Family members who are staying behind still belong on the form. A spouse who is not immigrating with you, or an adult child studying in another country, needs a complete travel record. Leaving out someone who should be included can be treated as misrepresentation under section 40 of the Immigration and Refugee Protection Act, which brings a five-year ban from Canada and refusal of the application.2Justice Laws Website. Which Trips You Have to Report
Report every trip you took outside your country of origin or country of residence during the past ten years. If a person turned 18 fewer than ten years ago, the window starts on their 18th birthday.1Immigration, Refugees and Citizenship Canada. Tourism, business, training, family visits, medical treatment, short shopping runs across a land border, long overseas work assignments. A weekend trip counts the same as a six-month posting.
The form does not exempt day trips or layovers. If you cleared customs in a transit country, list it. If you stayed airside without entering the country, the case for leaving it off is stronger, but when in doubt, put it in. Officers care much more about unexplained gaps than about an extra line.
What to Enter for Each Trip
For every trip, enter the start date, the end date, the city and country, and the purpose. Tourism, business, and training are given as examples, but you can write whatever plain description fits, like “family visit” or “conference.” The identity fields at the top of the form — full legal name, date of birth, and unique client identifier if you have one — must match your passport and your other application forms exactly.
Adding More Rows
In the Permanent Residence Portal, click the “+” button to add rows. On the PDF, print an extra page laid out the same way as the table, write your name and the form title on it, and include it with your package.1Immigration, Refugees and Citizenship Canada. Opening and Validating the PDF
The PDF often shows a “Please Wait” message or refuses to open in a web browser. IRCC’s PDF forms use encoding that needs Adobe Acrobat Reader 10 or higher and will not work in a browser’s built-in viewer.3Immigration, Refugees and Citizenship Canada. On a PC, right-click the download link, choose “Save target as” or “Save link as,” save it to your computer, then right-click the saved file, pick “Open with,” and select Adobe Acrobat Reader. On a Mac, hold Control and click the link, choose “Download linked file,” and open the saved file in Adobe Reader rather than Preview.
IRCC recommends filling in the PDF electronically to avoid handwriting errors. Once everything is entered, click the “Validate” button at the bottom of the form before you print and sign. Submitting the Form With Your Application
IMM 5562 is not filed on its own. It goes in as part of your complete permanent residence application. If you are applying through the Permanent Residence Portal, you complete the digital version inside that system. If you are applying on paper, you print, sign, and mail the validated PDF with the rest of your forms to the appropriate processing centre.

Fixing a Mistake After You Submit
If you notice after submitting that you forgot a trip or entered wrong dates, use the IRCC web form to send a correction. Go to the web form page, select “Update your application (for example, submit documents),” and enter your application number and unique client identifier.6Immigration, Refugees and Citizenship Canada. You will get an acknowledgement of receipt after submitting. Disclosing an honest mistake yourself is far better than waiting for an officer to find it. Sending several web forms about the same issue will not speed anything up. One clear, complete correction is enough.
Reconstructing Ten Years of Travel
Few people remember every trip they took a decade ago. Start with your passports, current and expired, and work through the stamps page by page. If you travel visa-free to many countries, stamp coverage may be thin, so also pull airline booking confirmations, credit card statements showing foreign transactions, and old emails from hotels and rental agencies.
If you took a land-border day trip and have no documentation, list it anyway with your best estimate of the date. Note that the date is estimated. An approximate entry is better than a missing one. Officers know perfect recall over ten years is unrealistic; what they look for is a good-faith effort to be thorough. Deliberate omissions are treated very differently, and that is where misrepresentation findings come from.
Work through the timeline in order and keep a running spreadsheet before you transfer anything to the form. Chronological review makes it easier to spot gaps where you may have forgotten a trip. Once your list is complete, filling in the form itself takes relatively little time compared with the research behind it.
Why Accuracy Here Really Matters
The travel history feeds directly into IRCC’s background and security screening. Officers compare what you declare against entry and exit records, passport stamps, and other sources, and they use the form to check consistency across your file. If your travel dates conflict with employment records or other documents, expect a follow-up.
