How to Request AF Form 422: MyIMR Steps, Remarks, and Validity

To request an AF Form 422, the Notification of Air Force Member’s Qualification Status, log in to the Air Force Portal with your Common Access Card, open the MyIMR module inside the Aeromedical Services Information Management System, and submit the request under Medical Readiness with the specific reason and target program named. A healthcare provider at your local Medical Treatment Facility then reviews your record and signs the form, which stays valid for 12 months from the date of issuance.1TRICARE. Most bounced 422s trace back to things the member could have handled before hitting submit. Work through the checks below first, then start the request.

Before You Submit

Open MyIMR and look at your Individual Medical Readiness status. If anything is red — a lapsed Periodic Health Assessment, overdue immunizations, an expired dental exam — the reviewer will not sign the form until you fix it. Resolving a dental issue after the request is sitting in the queue can add weeks you probably do not have.

Check your PULHES profile while you are in there. The system scores six areas of physical and mental fitness: Physical capacity and stamina, Upper extremities, Lower extremities, Hearing and ears, Eyes, and psychiatric stability. Each factor carries a numerical designator from fully qualified through severe enough to trigger disability processing. If a score looks outdated or wrong, get it corrected before you submit rather than after the reviewer flags it.

Dental class matters on its own. Class 1 means no treatment pending. Class 2 means non-emergency treatment outstanding but unlikely to cause problems within 12 months, and you are still deployable.3Air Force. Dental IMR – Understanding Dental Classes Class 3 flags a condition likely to cause a dental emergency, and Class 3 members are restricted to duties at their home station — no deployments, no TDY, no formal schools until it is resolved.4Headquarters RIO. Dental Submitting a 422 while you are Class 3 almost guarantees a disqualifying result or a hold pending treatment.

Pull together anything the military system does not already have. If you have been treated by a civilian provider off base, get those records ready to upload with the request. The reviewer can only evaluate what is in front of them, and a gap where an off-base condition was being managed will prompt questions you could have answered up front.

Some programs need more than the standard workup. Rated OTS applicants — pilots, combat systems officers, remotely piloted aircraft pilots — must pass color vision testing using the Cone Contrast Test.5Air Force Accessions Center. AD AF OTS PA COA If you wear glasses or contacts, have your current prescription documented. If you have had PRK or LASIK, dig up the surgery date, because the remarks section will ask for it. Members transferring to the Air Reserve Component also need a current DD Form 2697 (Report of Medical Assessment) and their most recent Periodic Health Assessment attached alongside the PULHES profile.2Department of the Air Force. Submitting the Request Through MyIMR

Get into the Air Force Portal with your CAC and open ASIMS. Inside ASIMS, go to the MyIMR interface and find the Medical Readiness tab, which has the AF Form 422 request field.67th Medical Group – Dyess Air Force Base. New Module Makes Medical Clearance Process Easier

Fill in the reason with specifics. If you are retraining, list the target AFSC so the reviewer applies the right physical standards. If you are applying for OTS, state whether you are a rated or non-rated applicant. If you are heading to a special duty assignment, name it. A vague entry such as “retraining” or “PCS” forces the reviewer to come back and ask what standards to evaluate you against, and that round trip costs days.

Attach your outside records now, not later. The digital interface takes uploads at the request step, and adding them then keeps the reviewer from pausing to request them mid-review.

Once you submit, the request routes to the Medical Standards Management Element at your MTF. Some bases run base operational medicine 422 reviews through a separate clinic, so if you have not heard anything in a few days, confirm the right shop received it.

Overseas PCS moves and family-status changes carry a parallel step: if your family is enrolled in the Exceptional Family Member Program, an EFMP-Medical screening runs alongside your own clearance to verify the gaining installation can support your family.8Air Force Medical Service. Getting the Remarks Language Right

This is where preventable delays cluster. The medical reviewer writes the remarks, but you need to know what they should say for your program. A form that reads “medically qualified” in general terms will be rejected by a board that expects an exact templated statement. Compare the remarks block against the template for your specific action before your provider signs.

OTS Applicants

Non-rated OTS applicants need remarks confirming full record review, medical qualification for continued enlistment and commissioning, clearance for worldwide duty, no Assignment Limitation Code, no active Medical Evaluation Board, no AF Form 469 duty-limiting condition, no fitness-testing exemptions, and whether the member has had PRK or LASIK along with the surgery date. Rated applicants need the same statement plus confirmation of passing the Cone Contrast Test. Special Tactics Officer, Combat Rescue Officer, and Tactical Air Control Party Officer applicants use similar wording specific to their target specialty.5Air Force Accessions Center. Palace Chase and Palace Front

Members moving to the Active Guard Reserve program through Palace Chase or Palace Front need the form to carry one of three exact phrases: “Cleared without limitations,” “Cleared with limitations,” or “Not qualified.” Applications without that templated language will not be processed.9Air Reserve Personnel Center. Special Duty Assignments

Military Training Instructor, Military Training Leader, and Academy Military Trainer positions require the reviewing medical authority to verify specific capabilities and include a statement on physical and mental fitness, stamina to supervise trainees under adverse conditions, and clear speech.10Department of the Air Force. Special Duty Assignment Category (SPECAT) Guide If you do not meet the minimum profile for one of these positions, the medical authority decides whether the condition can be waived and annotates that recommendation on the form.

Review Time and What You Do With the Signed Form

Plan on at least three to five duty days for a straightforward case. Members selected for Professional Military Education or short-notice assignments are encouraged to start the request as soon as they are notified, because the reviewer may need additional evaluations, specialty consults, updated labs, or new imaging.11Buckley Space Force Base. Medical Clearance Process for Air Force NCO, SNCO Academies Do not wait until the last week before your report date.

The reviewer signs the form with one of three standard outcomes: qualified, qualified with limitations, or not qualified. Once signed, the form is available in ASIMS to download and print. You are responsible for handing the signed copy to the Force Support Squadron or the administrative office running your personnel action. The Wing Medical Group transmits the clearance data to the Personnel Data System, but the physical copy still needs to sit in your application package.12Department of the Air Force. How Long the Form Lasts

A signed AF Form 422 is valid for 12 months from the date of issuance. If a new condition arises inside that window and you no longer meet retention standards, the form has to be updated regardless of the expiration date.2Department of the Air Force. DAFMAN 48-123 – Medical Examination and Standards OTS applicants who become disqualified after selection must notify Air Force Recruiting Service immediately.5Air Force Accessions Center. AD AF OTS PA COA

If your personnel action drags past 12 months, request a new form. Enlisted members applying for a commissioning program whose form expires before the class date cannot extend the existing form; they need a new commissioning physical at the nearest MTF or MEPS.

If You Come Back Not Qualified

A “not qualified” result does not automatically end your action. If a waiver authority can accept the risk on your condition, the aeromedical provider prepares a waiver package and routes it through the chain in DAFMAN 48-123 — flight surgeon, MAJCOM Surgeon General, and higher review at AFMRA/SG3PF Medical Standards when needed.2Department of the Air Force. Mistakes That Cost You Time

The pattern behind most delays is small and fixable. Wrong or missing remarks language leads the list, followed by requests that do not name the target AFSC or program, then outdated IMR items the member could have cleared before submitting, and finally civilian records the reviewer never sees because nobody uploaded them.

Before you hit submit: MyIMR green, dental class current, prescription and surgery dates ready, outside records attached, target program named, and the correct remarks template pulled up so you can check the reviewer’s language before the form is signed. That short list handles almost everything that would otherwise send the form back to you.
