To report your CSA 1099-R on your tax return, copy the gross distribution from Box 1 to Line 5a of Form 1040, the taxable amount from Box 2a to Line 5b, and the federal income tax withheld from Box 4 to Line 25a.1Internal Revenue Service. Instructions for Form 1040 If your annuity is fully taxable because you’ve already recovered all your after-tax contributions, you can skip Line 5a and enter the full amount on Line 5b. The distribution code in Box 7 and a few special situations can change that basic mapping, so check those before you file.
The Boxes That Actually Hit Your Return
Only a handful of the boxes on the CSA 1099-R produce numbers you type onto Form 1040. The rest are informational or drive how you treat the numbers you do enter.
- Box 1, Gross Distribution: the total OPM paid you during the year before any deductions. Goes on Line 5a.2Internal Revenue Service. Instructions for Forms 1099-R and 5498
- Box 2a, Taxable Amount: the portion of Box 1 that counts as taxable income. Goes on Line 5b. OPM calculates this using the Simplified Method, so most annuitants just use the number as-is.2Internal Revenue Service. Instructions for Forms 1099-R and 5498
- Box 2b, Taxable Amount Not Determined: if this is checked, OPM did not calculate Box 2a for you and you have to figure it yourself.2Internal Revenue Service. Instructions for Forms 1099-R and 5498
- Box 4, Federal Income Tax Withheld: what OPM already sent to the IRS on your behalf. Goes on Line 25a, where it joins your other withholding and gets credited against your tax bill.2Internal Revenue Service. Instructions for Forms 1099-R and 5498
- Box 5, Employee Contributions or Insurance Premiums: the after-tax amount you’re recovering tax-free this year, or insurance premiums deducted from your annuity. Informational; already reflected in the gap between Box 1 and Box 2a.3Internal Revenue Service. Instructions for Forms 1099-R and 5498
- Box 7, Distribution Code: a letter or number that tells the IRS what kind of payment this was. Certain codes send the money to a different line on Form 1040.2Internal Revenue Service. Instructions for Forms 1099-R and 5498
- Box 9b, Total Employee Contributions: your remaining after-tax basis in the plan, if OPM chose to report it. Useful for your records.3Internal Revenue Service. Instructions for Forms 1099-R and 5498
State income tax withheld and the state ID number sit further down the form. Those numbers go on your state return, not your federal one.
Trusting the Box 2a Number
For anyone who retired after November 18, 1996, OPM computes the taxable amount using the Simplified Method and prints the result in Box 2a.4Internal Revenue Service. Publication 721, Tax Guide to U.S. Civil Service Retirement Benefits That’s why Box 2a is almost always smaller than Box 1 in your early years of retirement: a slice of each monthly payment is treated as a return of the after-tax dollars you already paid in. Once you’ve recovered your entire cost in the plan, every dollar becomes taxable and Box 2a will match Box 1.
If Box 2b is checked with “Taxable amount not determined,” you have to run the Simplified Method yourself using the worksheet in IRS Publication 721.4Internal Revenue Service. Publication 721, Tax Guide to U.S. Civil Service Retirement Benefits The starting figure you need, your total after-tax contributions, is in the “Your Federal Retirement Benefits” booklet OPM sent when it finished processing your retirement application.5U.S. Office of Personnel Management. Annuity Payments
A narrow group of retirees whose annuities started before July 2, 1986 (or who elected it during a window that closed in November 1996) use the older General Rule with its actuarial life-expectancy tables. Publication 939 covers that calculation.6Internal Revenue Service. Publication 939, General Rule for Pensions and Annuities If your annuity started after November 18, 1996, the General Rule is not available to you.
When the Box 7 Code Changes Where You Report
Most federal retirees see Code 7, a normal distribution, and the basic Line 5a/5b treatment applies. Three other codes work differently.
Code 3, Disability
If you retired on disability and haven’t yet reached minimum retirement age, your annuity is taxed as wages rather than as pension income. Payments coded 3 go on Line 1 of Form 1040 (wages), not Line 5.4Internal Revenue Service. Publication 721, Tax Guide to U.S. Civil Service Retirement Benefits Once you hit minimum retirement age, the payments convert to a standard annuity and Simplified Method basis recovery begins.7U.S. Office of Personnel Management. Code 4, Survivor Annuity
Code 4 appears on a surviving spouse’s or other beneficiary’s CSA 1099-R. Survivor payments are generally fully taxable, but if the original retiree hadn’t finished recovering their after-tax contributions, the survivor continues that recovery using the same monthly exclusion, and Box 2a will reflect it.
Code G, Direct Rollover
Code G means the funds moved trustee-to-trustee into another eligible retirement plan, such as a traditional IRA. Report the total on Line 5a and enter zero on Line 5b. Nothing is taxable in the year of the rollover.
The Public Safety Officer Exclusion You Have to Claim Yourself
Retired law enforcement officers, firefighters, chaplains, and members of rescue squads and ambulance crews can exclude up to $3,000 per year of their annuity to pay for health or long-term care insurance premiums covering themselves, a spouse, or dependents.8Office of the Law Revision Counsel. 26 USC 402 – Taxability of Beneficiary of Employees Trust The premiums must be paid from the retirement plan of the employer you retired from as a public safety officer.9Internal Revenue Service. Publication 575, Pension and Annuity Income
OPM does not apply this exclusion for you. Box 2a on your CSA 1099-R will be the number without it. To claim the exclusion, reduce the taxable amount you enter on Line 5b by the qualifying premiums, up to $3,000. You can’t also deduct those same premiums as a medical expense on Schedule A.9Internal Revenue Service. Publication 575, Pension and Annuity Income
Your State Return
If OPM withheld state income tax, the amount and the state’s identification number appear on your CSA 1099-R and belong on your state return. State treatment of federal pensions varies. Some states fully exempt CSRS and FERS annuity income; others give a partial exemption that phases in at 60 or 65 or phases out above an income threshold; a few tax the annuity like any other income. Check your state’s current rules each filing season, and check them again if you moved during the year.

If the correction won’t arrive before the filing deadline, you have two choices. For a small discrepancy, file with the figures you know are correct and amend later on Form 1040-X once the corrected form is in hand.11Internal Revenue Service. Instructions for Form 1040-X For a large error, file Form 4868 for an automatic six-month extension so OPM has time to fix the form before you file.12Internal Revenue Service.
