Find Out Why Your Authority Was Revoked
Start with the FMCSA’s SAFER Company Snapshot or the Licensing & Insurance portal. Your operating status will read “Not Authorized,” and the record will point to whichever filing lapsed. Almost every revocation traces to one of two problems: your insurer stopped filing proof of coverage, or your Designation of Process Agents (Form BOC-3) terminated.2Federal Motor Carrier Safety Administration. While you’re looking at your snapshot, check the status of your USDOT number too. Your MC authority cannot be active on top of a deactivated USDOT number, and a missed biennial MCS-150 update deactivates it. Missing that update also exposes you to civil penalties of up to $1,000 per day, capped at $10,000.4Federal Motor Carrier Safety Administration. Do Not Operate While Revoked
Before you fix anything, park the trucks. Under 49 CFR 392.9a, any vehicle providing for-hire transportation without valid operating authority can be placed out of service at a roadside inspection.5Federal Motor Carrier Safety Administration. Reactivate Your USDOT Number If It’s Inactive
If your snapshot shows the USDOT number as “Not Active” or “Inactive,” handle that first. The FMCSA will not process an MC reinstatement while the underlying USDOT number is dead.
Reactivation means submitting the correct MCS-150 series form for your company type. Download the current form directly from fmcsa.dot.gov; the agency has warned that outdated versions pulled from third-party sites get rejected.7Federal Motor Carrier Safety Administration. Get Your Insurance Filing Back on Record
You cannot file proof of financial responsibility yourself. Your insurance company’s home office has to electronically file either Form BMC-91 (a surety bond) or Form BMC-91X (a certificate of insurance) with the FMCSA on your behalf, certifying that you carry at least the federally required minimum coverage.2Federal Motor Carrier Safety Administration. The minimum depends on what you haul and vehicle weight — general non-hazardous freight in heavier trucks is $750,000, and hazardous cargo requirements climb from there up to $5,000,000 for bulk explosives, certain poison gases, and radioactive materials. Call your insurer early. If your previous policy lapsed, you’ll need to bind new coverage and have the electronic filing transmitted, and it can take a few business days for the filing to post to the FMCSA’s system. The reinstatement submission won’t be accepted until that filing shows up.
Get Your BOC-3 Back on Record
The Designation of Process Agents names an agent in every state where you operate or travel who can accept legal service on your behalf. Most carriers pay a commercial process agent service — typically around $50 for all-state coverage — to prepare and file the BOC-3 with the FMCSA’s Office of Registration and Safety Information in Washington, D.C. If your previous designation terminated, contact a process agent service to file a new one, and give it a few days to appear in your record before the next step.
Submit the Reinstatement Request and Pay the $80 Fee
With both filings active, log in to the FMCSA Portal. As of January 2025 the agency consolidated its registration tools into the Portal, and it requires multi-factor authentication.10Federal Motor Carrier Safety Administration. You may need your USDOT PIN to link your account. Request one through SAFER with your EIN and USDOT number. The PIN can be delivered instantly to the email or phone number on file, or mailed to your physical address in 7 to 10 business days.11Federal Motor Carrier Safety Administration. The reinstatement fee is $80, paid by credit card or ACH at submission, and it is non-refundable regardless of outcome.12Federal Motor Carrier Safety Administration. What to Expect After You Submit

Watch your SAFER Company Snapshot. You’re waiting for the MC authority to switch from “Not Authorized” to “Active.” Do not book loads on the assumption that approval is imminent. Until the status changes, you are not legally authorized to operate as a for-hire interstate carrier.
Keep It From Happening Again
The same lapse that revoked you once will do it again. Confirm with your insurer that renewals are filed with the FMCSA before the existing filing expires, and set your own reminders well ahead of the renewal date. If you change process agent services, make sure the new BOC-3 is filed before the old one terminates.
File your MCS-150 biennial update on schedule, every two years, even if nothing has changed and even if you have stopped operating.13Federal Motor Carrier Safety Administration.
