How to Get Form SSA-2458: Verification Letter, SSA-3288, Fees

You cannot download or fill out SSA Form 2458 yourself. It’s an internal Social Security Administration document that field office staff generate when responding to benefit and payment inquiries, so it isn’t on the public forms page and isn’t something the public requests by name. If someone has told you to “get a 2458,” you need one of two things instead: a benefit verification letter from your my Social Security account, which takes about two minutes, or a completed Form SSA-3288 authorizing SSA to release your records directly to a third party.

What Form SSA-2458 Actually Is

SSA-2458, titled “Report of Confidential Social Security Benefit Information,” is a paper report that SSA staff produce internally. The agency’s procedures manual tells employees to “use the Inquiry Response Benefit Verification screen or the paper Form SSA-2458 (Report of Confidential Social Security Benefit Information) to manually generate a BEVE letter or respond to benefit and payment questions.”1Social Security Administration. POMS GN 03340.010 – Processing Access Requests for SSA Records It’s an output SSA sends, not an input you file, which is why it doesn’t appear on the public forms list.2Social Security Administration. You may see an SSA-2458 arrive in the mail if a state welfare office or other agency requested your benefit details from SSA directly. Minnesota’s health care programs manual describes it as “a report sent to the client, although a third party may have requested the information.”3Minnesota Health Care Programs Manual. 04.30ar3 – Verification of RSDI and SSI Benefits (Archive) If one is already in your hands, hand it to whoever asked. The Fast Path: Benefit Verification Letter Online

For nearly every situation where a lender, landlord, or agency wants proof of Social Security income, a benefit verification letter is what they’ll accept. When You Need Form SSA-3288 Instead

A benefit verification letter proves what you receive. It doesn’t get more detailed records into a third party’s hands. When a lender, attorney, or agency wants SSA to send them specific claim file information, medical records, or payment history, you sign Form SSA-3288, Consent for Release of Information, authorizing that release.5Social Security Administration. The Privacy Act requires SSA to have your written consent before disclosing records about you outside the agency, with limited exceptions for law enforcement, Congress, and a few other categories.6Office of the Law Revision Counsel. 5 U.S. Code 552a – Records Maintained on Individuals SSA-3288 is that written consent.

Filling Out SSA-3288

Download the form from ssa.gov/forms. Every field marked with an asterisk is required; a blank in any of those fields will cause SSA to reject the form.

  • Your full legal name, date of birth, and Social Security number.
  • The name and address of the third party you want SSA to send the information to. Be specific. A vague recipient can delay or block the release.
  • The types of records you want released, checked off, with date ranges where they apply. You cannot ask for “any and all records” or the “entire file.” SSA rejects blanket requests outright.5Social Security Administration. Consent for Release of Information – SSA-3288
  • The reason for the request, such as a mortgage application, eligibility determination, or litigation. A parent or guardian signing for someone else must also explain how the release benefits that person.
  • Your signature and date, made under penalty of perjury that the information you provided is true.

One boundary worth flagging: SSA-3288 covers benefit and claim records, not detailed earnings or employment history. For those, use Form SSA-7050-F4.5Social Security Administration. Consent for Release of Information – SSA-3288

How to Submit the Form

You have four routes:

If you mail it, certified mail gives you proof of the receipt date. That date matters because it starts the clock on how long the consent stays valid.

How Long the Consent Stays Valid

A signed SSA-3288 is good for one use unless you write otherwise on the form. The window for SSA to receive it depends on the type of records requested:

  • Non-medical records (benefit amounts, claim file information, payment history): SSA will honor the consent if received within one year of your signature date.10Social Security Administration. POMS GN 03305.003 – Consent Documents
  • Medical records: 90 days from your signature date. Arrive later and SSA returns the form and asks you to sign a new one.10Social Security Administration. POMS GN 03305.003 – Consent Documents

If a single form asks for both and reaches SSA after 90 days but inside a year, they release the non-medical records and return the medical portion. You can revoke consent at any time with a written statement to any Social Security office.

Signing for Someone Else

A parent or legal guardian of a minor child, or the legal guardian of a legally incompetent adult, can sign SSA-3288 on that person’s behalf.5Social Security Administration. Consent for Release of Information – SSA-3288 You’ll need to explain on the form how releasing the information serves that person’s best interest.

Power of attorney is not enough on its own. SSA distinguishes a representative payee, someone the agency formally appoints to manage benefits, from a person who simply holds a power of attorney or shares a bank account.11Social Security Administration. Frequently Asked Questions for Representative Payees To manage benefits for someone who can’t handle their own affairs, apply through SSA to become their representative payee.

Fees and Penalties

SSA does not charge for a release that relates to administering a Social Security Act program. If the request serves an unrelated purpose, SSA may charge for providing the records.5Social Security Administration. Consent for Release of Information – SSA-3288 Ordinary benefit verification and claim information requests rarely trigger a fee.

The form also carries a warning worth reading. Anyone who knowingly seeks access to another person’s records under false pretenses faces a fine of up to $5,000.5Social Security Administration. Consent for Release of Information – SSA-3288 Inaccurate identifying information, even by accident, can delay or block a legitimate request, so check every field before you sign.
