How to Get and Complete AF Form 422: Physical Profile Serial Report

To get an AF Form 422, you submit a digital request through the MyIMR portal on the Air Force Portal; a flight surgeon or other competent medical authority at your Base Operational Medicine Clinic then reviews your records and signs the completed form.1Joint Base Charleston. The Impact of Your IMR Status You don’t draft the form yourself. Your job is to trigger the request, make sure your medical readiness items are current, and specify the action you need clearance for.

What the Form Is and Who Signs It

AF Form 422, the Notification of Air Force Member’s Qualification Status, is how medical providers tell your commander whether you’re medically fit for a specific assignment, program, or duty. A flight surgeon or other competent medical authority generates it from your records and signs it so your personnel office or gaining unit can act on the result.228th Medical Group – Ellsworth Air Force Base. Base Operational Medicine The result reads as “cleared without limitations,” “cleared with limitations,” or “not qualified,” alongside your PULHES physical profile and any duty-limiting conditions with expiration dates. Without the signature of a competent medical authority, the form isn’t valid for administrative use.

When You Need One

Several career actions require a current AF Form 422. The most common triggers are overseas assignments, retraining into a new Air Force Specialty Code, special duty positions (military training instructor, recruiter, developmental special duty), commissioning programs, professional military education, and security clearance actions.228th Medical Group – Ellsworth Air Force Base. Base Operational Medicine For commissioning paths like Officer Training School or the Airman Education and Commissioning Program, the form must show you’re qualified for accession without a deployment limitation.

Overseas and remote assignments draw extra scrutiny because those installations often lack full medical facilities, so the medical standards office has to confirm the gaining location can support you before orders finalize. Members transitioning from active duty to the Guard or Reserve through Palace Chase or Palace Front also need an AF Form 422 as part of the application package, with its own routing described further down.

How to Submit the Request

The request starts in the Aerospace Medicine Information System, which you reach through the MyIMR portal on the Air Force Portal.1Joint Base Charleston. The Impact of Your IMR Status Log in with your CAC, go to the Medical Readiness tab, and submit a digital request. Specify the action you need clearance for: retraining, overseas assignment, commissioning, special duty, and so on. Being specific matters because the flight surgeon measures you against the medical standards for that action, not a generic fitness bar.

If the request involves a special duty or unique assignment, expect to provide a Report on Individual Person so the medical team can compare your profile against the demands of the role. Members transferring from active duty to the Air Reserve Component within twelve months of separation need to package a current AF Form 422 with PULHES codes, a DD Form 2697 (Report of Medical Assessment), and their last Periodic Health Assessment, per DAFMAN 48-123.3United States Air Force. What to Have Current Before You Request

Missing readiness items are the fastest way to have your request bounce back. Before you submit, confirm each of the following is current in MyIMR:

  • Your Periodic Health Assessment. If it’s expired, complete a new one before the medical standards office can generate the form.
  • Your dental readiness class. An outdated dental exam stalls the process.
  • Your most recent fitness assessment scores.
  • The medical standards for your target action. DAFMAN 48-123, which superseded AFI 48-123 in December 2020, lays out the examination and retention standards the flight surgeon will apply.3United States Air Force. DAFMAN 48-123 – Medical Examination and Standards

Some wings, particularly in the Guard and Reserve, also require recent blood work and vitals within the last six months before they’ll process the form.

How Long It Takes and How You Get It Back

Once submitted, the request routes to the Base Operational Medicine Clinic or Medical Standards office. Technicians pull your records, a flight surgeon reviews the file, and a signed form comes back in roughly seven to ten business days for a routine request.228th Medical Group – Ellsworth Air Force Base. Base Operational Medicine You’ll get an automated email when the form is signed. Log back into MyIMR or ASIMS to view and download the final PDF.

The form carries an expiration date. For commissioning-related actions, the form reportedly expires sixty months after the signature date and cannot be extended; if it lapses, you’ll need a new commissioning physical at the nearest military treatment facility or MEPS. For other actions the validity window may differ, so check with your medical standards office before assuming an older form still counts.

Guard and Reserve Members

If you’re in the Air National Guard or Air Force Reserve, the request process is less standardized than on active duty and varies by state and wing. Some units accept MyIMR submissions just like active duty; others require you to contact the Medical Group directly or submit a DD Form 2697 alongside the request.

There’s also a distinction between a “losing” and “gaining” AF Form 422. When you’re leaving a unit, you can usually initiate the losing 422 yourself through MyIMR. The gaining 422, needed by the unit you’re transferring into, is typically initiated by that gaining unit’s medical group rather than by you. Transfers between states or between components (Reserve to Guard or vice versa) add steps and time, so plan ahead.

Palace Chase and Palace Front Transitions

If your AF Form 422 is for a Palace Chase or Palace Front transition, the process changed in April 2024. The form is generated by the active-duty medical treatment facility’s BOMC office and must use standardized clearance language: “cleared without limitations,” “cleared with limitations,” or “not qualified.” Wording outside that template will get the application returned.4Air Reserve Personnel Center. What happens next depends on the outcome:

  • Cleared without limitations: upload the approved form to your AGR application in MyVector. No further medical review is required.
  • Cleared with limitations: the recruiter submits the form through AFRIS to AFRC/RSOM, which routes it to AFRC/SGP. If approved, a memorandum issues with waiver management instructions, and the gaining unit commander must sign and accept the member with the applicable limitations. If the commander declines, the application stops.
  • Not qualified: you’re referred to the Airmen Medical Readiness Optimization Board. If AMRO clears you, upload the board’s decision and the AF Form 422 to MyVector. If AMRO upholds the disqualification, notify the gaining unit so the position can be readvertised.4Air Reserve Personnel Center. The Separation History and Physical Exam is a separate requirement and can’t be processed until you’re within ninety days of your Date of Separation, so don’t wait for one before requesting the other.

If You’re Disqualified

A “not qualified” result isn’t necessarily final. DAFMAN 48-123 lets you appeal a waiver disposition in writing to the waiver authority, but the appeal must include information that wasn’t previously considered. Simply disagreeing with the outcome won’t move it.3United States Air Force. DAFMAN 48-123 – Medical Examination and Standards

If the initial waiver authority denies the appeal, they must consult the next higher waiver authority. The chain ultimately reaches AFMEDCOM/A3P Medical Standards, the final medical appeal authority for accession waivers and MAJCOM-level decisions. Beyond that, the only remaining path is an Exception to Policy, which sits outside the Surgeon General’s authority and is a Secretary of the Air Force decision to accept risk beyond what the medical community recommends. It’s only available after the formal appeal process is fully exhausted.3United States Air Force. DAFMAN 48-123 – Medical Examination and Standards Palace Chase and Palace Front disqualifications go through the AMRO Board instead of the standard waiver chain. Either way, gather new medical records, specialist opinions, or updated test results before filing. You only get one shot at presenting new evidence at each level.
