To get a Form W-9 for yourself, download the current blank PDF free from the IRS website at irs.gov, fill in your name, tax classification, and taxpayer identification number, sign it, and return it to the business that asked for it. The form is one page, and most people finish it in about five minutes. Where to Download the Blank Form

Get the form from the IRS directly. On irs.gov, open the “About Form W-9” page and the fillable PDF is linked at the top.2Internal Revenue Service. About Form W-9, Request for Taxpayer Identification Number and Certification The current version is dated March 2024 in the upper-left corner. If a client hands you an older revision, either confirm they will still accept it or pull the current one yourself; outdated versions can slow processing.

Be careful about where a W-9 request comes from. Scammers send fake requests by email and text to harvest Social Security Numbers. If a company you don’t do business with contacts you out of the blue, verify through a known phone number or website before sending anything. Legitimate requesters usually ask during onboarding, not cold.

Make Sure You Actually Need a W-9

If a company is hiring you as an employee with a regular paycheck, withheld taxes, and benefits, the correct form is a W-4, not a W-9. The W-9 is for independent contractors, freelancers, and vendors who handle their own taxes. Filling out a W-9 when you should be classified as an employee can cost you thousands in self-employment tax and lost benefits. When the arrangement looks like employment but the company insists on a W-9, the IRS has guidance for reporting the classification issue.3Internal Revenue Service. What to Have Ready Before You Start

Pull together a few things before opening the PDF. You need your full legal name exactly as it appears on your tax return, plus any DBA name if you use one, and your mailing address so future 1099s reach you.

You also need a Taxpayer Identification Number, which federal law requires on any return or statement filed with the IRS.4Office of the Law Revision Counsel. 26 USC 6109 – Identifying Numbers For most individuals, that is your Social Security Number. For a corporation, partnership, or multi-member LLC, use the Employer Identification Number.5Internal Revenue Service. Instructions for the Requester of Form W-9 (Rev. March 2024) Resident aliens who cannot get an SSN can use an Individual Taxpayer Identification Number, obtained by filing Form W-7.6Internal Revenue Service. Filling Out the Form Line by Line

Lines 1 and 2: Name and Business Name

Line 1 is your legal name. Don’t leave it blank and don’t use a nickname. The name here must match your tax return.7Internal Revenue Service. Form W-9 (Rev. March 2024) Instructions Line 2 is optional. If you use a separate business name, trade name, or DBA, put it there. Freelancing under your own name? Leave Line 2 empty.

Single-member LLCs treated as disregarded entities trip people up. Your personal name goes on Line 1 as the owner, the LLC’s name goes on Line 2, and the TIN in Part I is yours, not the LLC’s, because the IRS looks through the disregarded entity to its owner.5Internal Revenue Service. Instructions for the Requester of Form W-9 (Rev. March 2024)

Line 3: Federal Tax Classification

Check one box on Line 3a to tell the requester how the IRS classifies you.7Internal Revenue Service. Form W-9 (Rev. March 2024) Instructions Most freelancers, gig workers, and solo consultants tick “Individual/sole proprietor or single-member LLC.” Incorporated businesses pick C Corporation or S Corporation. Multi-member LLCs taxed as partnerships and formal partnerships pick Partnership.

Getting the classification wrong won’t change what you owe, but it can cause the payer to file the wrong information return or withhold when they shouldn’t. If you’re unsure, check your formation documents or the IRS determination letter you got when you applied for an EIN.

Line 4: Exemption Codes

Most individuals leave Line 4 blank, and that’s correct. The exempt payee codes are for tax-exempt organizations, government agencies, corporations, and registered securities dealers.7Internal Revenue Service. Form W-9 (Rev. March 2024) Instructions Sole proprietors and individual freelancers aren’t exempt from backup withholding, and entering a code you’re not entitled to can cause problems later. The FATCA exemption field on the same line applies only to accounts held outside the United States. Domestic account? Leave it blank.

Part I: Taxpayer Identification Number

Enter your SSN, ITIN, or EIN in the boxes. Check every digit twice. A mismatched number is one of the fastest ways to trigger backup withholding or delay payments, because the IRS flags the mismatch when the payer files their 1099. An SSN must correspond to the name on Line 1; an EIN must match the entity named on Lines 1 and 2.7Internal Revenue Service. Form W-9 (Rev. March 2024) Instructions

Part II: Certification and Signature

Sign and date the form. Your signature certifies, under penalty of perjury, that the TIN is correct, that you aren’t subject to backup withholding (or that you are, if the IRS has notified you), and that you are a U.S. person.7Internal Revenue Service. Form W-9 (Rev. March 2024) Instructions

Electronic signatures are fine. The IRS lets requesters accept W-9s electronically, including by fax, as long as their system authenticates the signer and includes the perjury statement.5Internal Revenue Service. Instructions for the Requester of Form W-9 (Rev. March 2024) Signing a fillable PDF, using DocuSign, or faxing a signed copy all work if the requester’s system supports them.

Where to Send the Completed Form

Send the finished W-9 to the business or person who requested it. You do not send it to the IRS. The requester keeps it on file and uses it when filing their information returns.8eCFR. 26 CFR 1.6041-1 – Return of Information as to Payments of $600 or More

Because the form carries your SSN, treat delivery the way you would any sensitive financial document. Encrypted email, a secure upload portal, or physical mail are all reasonable. Avoid plain email attachments. If a requester insists on an unprotected method, push back. Cleanup after a compromised SSN is far worse than an awkward conversation about file security.

When to Send an Updated W-9

A W-9 doesn’t expire on a set schedule, but certain changes mean you need to send a fresh one to every requester holding your old information:

  • Legal name change from marriage, divorce, or a court order, so the name on Line 1 still matches your tax return.
  • A new TIN, for example after incorporating or forming an LLC that gets its own EIN.
  • A change in entity type, such as converting from a sole proprietorship to an S Corporation, which alters both Line 3 and likely the TIN.
  • A new address. The instructions note that marking the address “NEW” tells the requester to update their records.5Internal Revenue Service. Instructions for the Requester of Form W-9 (Rev. March 2024)
  • A change in backup withholding status, so a payer can stop withholding once the IRS has lifted a prior requirement.

Don’t wait for the requester to ask. A mismatch between your W-9 and your actual tax return is exactly what triggers IRS notices and backup withholding.

Penalties for Mistakes or False Information

The consequences run from annoying to severe. If you fail to provide a correct TIN or don’t return the form, the payer must start backup withholding at 24% on every reportable payment.9Internal Revenue Service. Tax Withholding Types That money goes to the IRS. You can recover it when you file your return, but until then you’re lending the government a quarter of your income interest-free. Backup withholding also applies if the IRS has previously notified you of underreported interest or dividend income.10Internal Revenue Service. Backup Withholding

The IRS can also assess a $50 civil penalty for each failure to comply with the TIN reporting requirement, up to $100,000 in a single calendar year.11Office of the Law Revision Counsel. 26 USC 6723 – Failure to Comply With Other Information Reporting Requirements

Willfully signing a W-9 you know contains false information is a felony. Because the form is signed under penalties of perjury, a conviction under the fraud and false statements statute can carry a fine up to $100,000 ($500,000 for a corporation) and up to three years in prison.12Office of the Law Revision Counsel. 26 USC 7206 – Fraud and False Statements Providing a fake SSN or misrepresenting your identity moves the matter from paperwork into criminal territory.
