How to Get a Copy of Your 1095-B Form Online

To get a copy of your 1095-B form online, sign in to your health insurance provider’s member portal and download the PDF from its tax documents section. If the portal is unavailable or you cannot find the form there, you can pull the same coverage data the insurer reported to the IRS by viewing your wage and income transcript in your IRS Individual Online Account.1Internal Revenue Service. Figure Out Who Issued Your 1095-B

Before you can download the form, you need to know which portal to log into. The issuer depends on how you were covered during the tax year.3Office of the Law Revision Counsel. 26 USC 6055 – Reporting of Health Insurance Coverage

Two lookalike forms cause most of the confusion. Form 1095-A comes from a Health Insurance Marketplace like HealthCare.gov or a state exchange, and you retrieve it inside your Marketplace account, typically between mid-January and early February.7HealthCare.gov. Download the Form From Your Provider’s Portal

Once you know who issues your form, go to that entity’s member website. For private insurance, that is the carrier’s site. For Medicaid or CHIP, it is usually your state health agency’s portal.

To sign in, you will typically need your Social Security number or date of birth along with the member ID or policy number on your insurance card. First-time users should look for a “Register” or “Create Account” link; setting one up usually requires a valid email address and answers to identity verification questions.

After logging in, find the section labeled “Tax Documents,” “Tax Forms,” or “Member Resources.” Select the correct tax year and download the PDF. If it does not open on its own, check your browser’s downloads folder.

Not seeing the form in the portal? Call the customer service number on the back of your card and ask for a copy. Many providers can email a digital copy or mail a replacement within a few weeks.

When the Form Should Be Available

For the 2025 tax year, insurers that still mail the form must send it by March 2, 2026, an automatic extension from the original February 2 date. Insurers that use the alternative online method must post a notice explaining how to request a copy, and must furnish the form within 30 days of your request or by January 31, 2026, whichever is later.2Internal Revenue Service. Pull the Coverage Data From the IRS

If the portal is down, you cannot reach customer service, or your issuer no longer exists, the IRS offers a backup path. Every insurer that reports to the IRS files the same coverage data the form contains, and you can view that data through a wage and income transcript.1Internal Revenue Service. If you exceed that across all form types, the transcript will not generate online, and you would need to file Form 4506-T by mail.1Internal Revenue Service. Fixing Errors on the Form

If the downloaded form shows a wrong name, incorrect Social Security number, or missing months of coverage, contact the issuer, not the IRS. The issuer files a corrected form by checking the “CORRECTED” box on a new 1095-B and sending it to both you and the IRS.2Internal Revenue Service. Instructions for Forms 1094-B and 1095-B (2025)

Have supporting documentation ready. A Social Security card supports an SSN fix; enrollment records support a claim of additional covered months. For example, if the form shows January through September but you had coverage through December, the issuer needs proof before filing the correction.

State law is where the form still carries weight. California, the District of Columbia, Massachusetts, New Jersey, and Rhode Island charge residents a tax penalty for going without qualifying coverage as of 2025. Vermont requires coverage but does not impose a financial penalty. In those states, you generally will not attach the 1095-B to your return, but you use the covered months and covered individuals on it to fill out the relevant state schedule. If you live in one of these states and cannot find the form, retrieving it online through your provider or the IRS transcript should come before you file.
