How to Get a CDE Form for Your Driver’s Permit

The CDE form for a driver’s permit is Indiana’s Certification of Driver Education Enrollment, a one-page document a BMV-licensed driver training school signs to confirm you are enrolled in an approved behind-the-wheel course.1Indiana Bureau of Motor Vehicles. Certification of Driver Education Enrollment You get it from the school after you enroll, not from the BMV, and you bring it with the rest of your permit paperwork when you apply. It proves enrollment, not completion; you do not need to finish the course before the school issues the form.

Who Actually Needs a CDE Form

The form matters most if you are 15. In Indiana, a 15-year-old can only apply for a learner’s permit by enrolling in an approved behind-the-wheel course and submitting a completed CDE form with the application. At 16 or older, you can get a learner’s permit without driver education by passing a vision screening and the knowledge exam.2Indiana Bureau of Motor Vehicles. BMV Learner’s Permit

The trade-off is timing. With driver education, you can move up to a probationary license at 16 years and 90 days. Without it, you wait until 16 years and 270 days.3Indiana Bureau of Motor Vehicles. How to Get the Form

A representative of a BMV-licensed driver training school completes the CDE form on school letterhead once you enroll in their behind-the-wheel program.1Indiana Bureau of Motor Vehicles. Certification of Driver Education Enrollment You cannot download it yourself, and the BMV does not hand it out. The school is the only source.

The steps look like this:

  • Enroll in an approved course. Indiana’s driver training program is 30 hours of classroom instruction plus six hours of behind-the-wheel training with a BMV-licensed school. Online instruction can replace the classroom portion, but the six driving hours must be in person.4Indiana Bureau of Motor Vehicles. BMV Driver Education
  • Ask the school to complete the form. The representative fills in your information, adds the school’s details, and signs the certification. You do not have to finish the course first; enrollment is enough.
  • Expect a possible administrative fee on top of tuition. Amounts vary by provider.

Lost the form before you made it to the BMV? Call the school that issued it. They can reissue, though they may charge for a replacement, and the new copy will carry a new signature date.

The 180-Day Deadline

The CDE form is valid for 180 days from the date the school representative signs it.1Indiana Bureau of Motor Vehicles. Certification of Driver Education Enrollment If you wait past that window, the form expires and you have to go back to the school for a new one. Treat the signature date as a countdown and get to the BMV well before it runs out.

What to Bring to the BMV With the Form

The CDE form is one item in a larger application. For a minor under 18, a parent or guardian has to come along and bring the following originals, not photocopies:5Indiana Bureau of Motor Vehicles. Learner’s Permit Requirements

  • The completed CDE form from an approved behind-the-wheel training provider
  • One identity document, such as a U.S. passport, birth certificate, or certificate of citizenship
  • One lawful status document
  • One Social Security document
  • Two Indiana residency documents (if the minor cannot provide two, an adult 18 or older living with the minor can sign a residency affidavit)
  • Proof of financial liability, signed on the permit application by a parent, legal guardian, or another adult willing to assume the obligation

The parent or guardian has to be there in person to sign the financial liability portion. The BMV checks the CDE form against your other documents, so the name and details need to match exactly.

Stay Enrolled After the Form Is Signed

The CDE form certifies current enrollment. If you withdraw from the driver training course after the school signs the form but before you get your permit, the certification no longer reflects your status. School attendance can matter too: states with graduated licensing systems commonly tie driving eligibility to staying in school. Dropping either your academic program or your driver training course can delay or block the permit.

Do Not Falsify the Form

A forged or altered CDE form is treated as fraud in obtaining a license. Penalties depend on the state and are consistently harsh. Missouri, for example, makes fraud or false documentation in a permit or license application a class A misdemeanor, revokes the applicant’s driving privileges for one year after conviction, and revokes the license of anyone who helped for the same period.6Missouri Revisor of Statutes. Missouri Revised Statutes Section 302.233 – Fraud in Obtaining a License or Permit, Penalty A year off the road at 15 or 16 costs far more than sitting through the driver education course honestly.

If You Are Not in Indiana

The specific term “CDE form” is Indiana’s. Many other states require their own proof of driver education enrollment or completion before issuing a permit to a minor, but the form’s name, issuer, and rules differ. If someone told you to bring a CDE form and you are applying somewhere other than Indiana, check with your state’s motor vehicle agency or the driver training school directly to find out which document your state actually uses.
