How to Find and Fill Out Chase Bank Forms Online

Chase bank forms are spread across several places rather than kept in one central library: everyday banking forms live inside your account on chase.com and the Chase Mobile app, brokerage forms sit on a dedicated PDF page, mortgage paperwork has its own section, and a few documents — power of attorney and beneficiary designations among them — can only be processed at a branch. Which channel you use depends on the form, and using the wrong one is the main reason requests get delayed.

Where Chase Keeps Its Forms

There is no single forms center for personal banking. Brokerage and investment PDFs — account maintenance, retirement distributions, transfers, beneficiary designations — are at chase.com/personal/investments/brokerage-forms.1J.P. Morgan Wealth Management. Brokerage Forms Auto loan customers have a separate forms center. Mortgage documents live under the mortgage assistance section. Everyday banking forms, like a direct deposit setup, appear under your account’s service options once you sign in.

The mobile app is the fastest route for frequently used forms. After signing in, tap the account, swipe up, and look under “Account services.”2Chase. Direct Deposit For anything that needs notarization, identity verification, or in-person review — power of attorney is the common one — you’ll need a branch. A banker can also print the current version if you’re unsure which revision applies.

Direct Deposit

Setting up direct deposit takes about two minutes online. Sign in, choose the checking account where you want deposits to land, go to “Account services,” and tap “Set up direct deposit form.” Chase generates a prefilled PDF with your name, account number, and routing number. You can download it, print it, or email it to your employer.3Chase. Set Up Direct Deposit

Verify the prefilled information before sending it. A transposed account number routes your paycheck somewhere it shouldn’t go, and recovering a misdirected deposit takes considerably longer than checking nine digits.

Wire Transfers

You can send a domestic or international wire through the Chase Mobile app without a paper form. Sign in, tap “Pay & transfer,” choose “Wires and global transfers,” accept the terms, select your account, and enter the recipient’s details.4Chase. Wire Money Online

If you’d rather work with a banker in person, bring a valid photo ID and your mobile phone to a branch. Sending a wire through a branch or with banker assistance may cost more than using the app.4Chase. Wire Money Online

Disputing an Unauthorized Charge

Chase handles fraud reports online rather than through a paper affidavit. Sign in, find the charge, and review the detail screen, which shows whether the transaction was in person or online and whether you’ve bought from that merchant before. If it looks fraudulent, choose the option to dispute it. Filing a fraud claim may deactivate the card, and Chase will issue a replacement.5Chase. Fraud – Unauthorized Charges

Federal rules set the deadlines. Report an unauthorized electronic transfer within two business days of learning about it and your liability caps at $50. Report later but within 60 days of your statement and the cap rises to $500. Miss the 60-day window and you can be liable for the full amount of transfers after the deadline that the bank can prove it would have stopped had you spoken up.6Consumer Financial Protection Bureau. Regulation E 1005.6 Liability of Consumer for Unauthorized Transfers Banks may extend these deadlines when extenuating circumstances caused the delay, but that is not something to rely on.

Power of Attorney

A power of attorney lets your named agent manage Chase accounts for you. Chase accepts outside POA documents and also offers its own POA form tailored to Chase accounts, but either path requires an in-person branch visit, and the agent must be there in person for the bank to add them.7Chase. Power of Attorney

Bring the signed POA, government-issued photo ID for both the principal and the agent, and any supporting documents your situation calls for, such as a letter of incapacity, physician statement, or death certificate. Chase reviews the POA to confirm it grants appropriate authority before approving changes. A statutory POA, one that follows your state’s official format, tends to move through the review more smoothly because banks are familiar with the standardized language. Requirements vary by state, and Chase notes you may need more than one branch visit to finish the process.7Chase. Power of Attorney

Adding a Beneficiary

A Payable on Death (POD) designation sends the funds in a bank account directly to your named beneficiary at your death, bypassing probate. At Chase, adding a POD beneficiary requires a branch visit; it can’t be done online. You’ll need the beneficiary’s first and last name, phone number, and address. The beneficiary does not need to be present and does not need to be a Chase customer.8Chase. Beneficiary – Frequently Asked Questions If the beneficiary is a business or other entity, bring its name, address, phone number, NAICS code, and EIN.

A POD designation gives the beneficiary no access or control while you’re alive, and you can change or remove it any time. After your death, the beneficiary claims the funds by giving the bank a copy of the death certificate.

Transfer on Death for Brokerage Accounts

For non-retirement brokerage accounts at J.P. Morgan Securities, the equivalent is a Transfer on Death (TOD) Agreement. Download it from the brokerage forms page, open it in Adobe (it won’t work in Chrome), fill it out, and have it notarized. Primary and contingent beneficiary percentages must each total exactly 100%, with no more than two decimal places. Mail the completed form to J.P. Morgan Securities LLC, Attn: Account Processing, Mail Code IL1-0291, 10 South Dearborn St., Chicago, IL 60603-5506.9Chase. Transfer on Death Agreement The designation isn’t effective until J.P. Morgan Securities receives and accepts the form at that address. TOD accounts are not available to Louisiana residents or to residents of states that have not enacted TOD legislation.

Mortgage Payoff and Third-Party Authorization

You can request a mortgage payoff quote through your Chase online account. If a title company, new lender, or other third party is requesting the figure on your behalf, they must first submit the Authorization to Furnish and Release Information form.10Chase. Mortgage Payoff The form can be sent by regular mail to Chase, Mail Code OH4-7302, P.O. Box 24696, Columbus, OH 43224-0696; by overnight mail to Chase, Attn: Third Party Authorization Research, 710 South Ash Street, Suite 200, Glendale, CO 80246-1989; or by fax to 1-614-422-7575 (free from any Chase branch).11Chase. Authorization to Furnish and Release Information Form

Co-op property payoff requests take a different path: email tri.state@jpmchase.com or fax 1-318-402-4389, and include your name, property address, Chase loan number, the new lender’s information, and your attorney’s contact details.10Chase. Mortgage Payoff To use existing escrow funds toward a payoff, call 1-800-848-9136 or send a request through the Secure Message Center on chase.com.

Tax Forms

W-9

Chase needs a valid W-9 on file with your correct taxpayer identification number. If your TIN doesn’t match the account name, or the IRS has notified you that you failed to report all interest and dividends, the bank must withhold 24% of your reportable payments as backup withholding.12Internal Revenue Service. Publication 15 (2026), (Circular E), Employer’s Tax Guide An accurate W-9 prevents the deduction entirely.

W-8BEN

Foreign individuals holding a U.S. Chase account complete IRS Form W-8BEN to certify foreign status and, if applicable, claim a reduced withholding rate under a tax treaty. Without a valid W-8BEN, the default federal withholding on U.S.-source income is 30%. The form is generally valid from the date you sign it through the last day of the third succeeding calendar year, so a form signed in 2026 expires on December 31, 2029, unless a change in circumstances makes the information on it incorrect.13Internal Revenue Service. Instructions for Form W-8BEN

1099s

Chase mails 1099-R forms (IRA, SEP-IRA, SIMPLE IRA, Roth IRA, and qualified plan distributions) by January 31. Consolidated 1099s, which bundle 1099-INT, 1099-DIV, 1099-MISC, 1099-B, and 1099-OID, are expected by February 17.14Chase Bank. Investment Tax Forms and Mailing Dates If the IRS grants Chase an extension, those dates may slip. You can also pull tax documents online under the “Tax Documents” page in your account statements.15Chase. Deposit Tax Forms and Mailing Dates

Business Account Documents

Opening or updating a Chase business account takes more documentation than personal banking, and what you need depends on structure:

  • Sole proprietor: your personal Social Security number works if you haven’t yet obtained an EIN.
  • LLC: EIN and business registration; if you’re the sole owner, your name should be listed as such.
  • Partnership: a partnership agreement showing all partners and the business name.

Every owner holding 10% or more of the business needs a tax identification number (SSN, TIN, or EIN) on file. You’ll also need a digital copy of an official government document confirming the business exists — articles of organization, a certificate of formation, or a DBA certificate — plus two forms of ID for each signer, at least one of them a government-issued photo ID.16Chase. Business Bank Account Information

All authorized signers must be present when opening the account in person. Adding or removing authorized users later can be handled through “Access & Security Manager” in online banking, where you can set each user’s access level.

Updating Your Address or Profile

Simple profile updates don’t require a paper form. Sign in to chase.com or the Chase Mobile app, select the profile icon, choose “Personal details,” and follow the prompts.17Chase. How to Find and Update Credit Card Billing Address You can also call the number on the back of your card or stop by a branch. Chase auto lease customers must call 1-800-227-5151 to change an address, since the update may affect lease terms.

What You’ll Need on Almost Any Form

A handful of items come up on nearly every form. Your nine-digit routing number identifies Chase; your account number identifies the specific account. Both appear at the bottom of your checks and on your direct deposit form. Personal forms need your Social Security number; business forms need the business’s EIN.18Chase. What Do You Need to Open a Business Bank Account

For identity verification, bring a government-issued photo ID: a state driver’s license, state ID card, or U.S. passport. Chase may also ask for a digital scan of the ID captured with your phone’s camera to help prevent fraud.16Chase. Business Bank Account Information Complete every field, and on paper forms use blue or black ink so signatures reproduce clearly.

Penalties for False Statements

Signing a bank form is not a formality. Under 18 U.S.C. § 1014, anyone who knowingly makes a false statement or willfully overvalues property to influence a bank’s action on a loan, credit application, or related agreement faces a fine of up to $1,000,000, up to 30 years in prison, or both.19Office of the Law Revision Counsel. 18 U.S. Code 1014 – Loan and Credit Applications Generally The statute covers loan applications, purchase agreements, and insurance applications involving federally connected institutions.

Even on routine forms where prosecution isn’t a realistic risk, inaccurate information can trigger account freezes, rejected transactions, or compliance delays. Getting the details right the first time is quicker than fixing a flagged submission later.
