Air Force forms are the standard templates the Department of the Air Force uses to authorize leave, document evaluations, nominate members for awards, verify dependents, and process nearly every other administrative action in the service. The only authorized source for current versions is the Department of the Air Force e-Publishing site at e-publishing.af.mil, where forms are posted as fillable PDFs you complete in Adobe Acrobat and sign with your Common Access Card.1Department of the Air Force E-Publishing. Where to Download the Current Version
Go to e-publishing.af.mil and use the product index to search by form number or title. One quirk: drop the spaces when you search a number. Enter “AFTO53” rather than “AFTO 53” to get results.2Department of the Air Force E-Publishing. Third-party sites are a reliable way to end up with an outdated form. Form numbers stay the same across revisions, so an old copy can look identical to the current one until someone checks the date in the footer. Outdated forms get kicked back. Before you type anything into a downloaded PDF, confirm the version date matches what e-Publishing shows.
AF vs. DAF Prefix
Many forms still carry the traditional “AF” prefix, but the service has been transitioning to “DAF” (Department of the Air Force) branding to reflect the unified structure covering both the Air Force and Space Force. If a search under “AF” turns up nothing, try “DAF.” The content and purpose of the form are the same; the prefix change is administrative.
Form Hierarchy
Forms exist at three levels under Air Force Instruction 33-360: departmental forms used Air Force-wide, Major Command (MAJCOM) forms covering a specific command’s needs, and base-level forms handling tasks unique to one installation.3Department of the Air Force. What You Need Before You Open the PDF
Adobe Acrobat Reader is the baseline. The built-in PDF viewers in Windows and macOS do not support the digital signature functions Air Force forms require, so a form that looks fine on screen will fail to accept your signature. You also need your Common Access Card in a working card reader, because the CAC carries the Public Key Infrastructure certificates that authenticate you when you sign. Without a valid CAC and reader, you cannot apply a legally recognized digital signature to a DoD form.
Gather your data before you start. Most personnel forms ask for your Social Security Number, rank, unit identification code, and official duty title. Some fields validate input against a specific format, such as dates in YYYY-MM-DD or numeric-only phone fields. If a field rejects what you typed, check the expected format before assuming the form is broken. Long sessions in a fillable PDF can lose unsaved data, so having your unit address and any supporting documents open in front of you makes the difference.
Forms You’ll Encounter Most Often
Hundreds of forms exist across personnel, medical, logistics, and operations functions. A handful come up constantly.
- AF Form 910, Enlisted Performance Report (AB thru TSgt): the standard evaluation for enlisted airmen from Airman Basic through Technical Sergeant. Evaluations are now completed through the myEval system rather than the standalone PDF; the offline AF Forms 715 and 716 are used only by exception.4Air Force’s Personnel Center. - AF Form 1206, Nomination for Award: used at every level for quarterly and annual award packages. Nominations are written as performance statements, each a standalone sentence describing an action and its impact in plain language without uncommon acronyms.5Air Force’s Personnel Center. Awards
- AF Form 988, Leave Request/Authorization: covers ordinary leave, emergency leave, and permissive TDY. The Air Force processes leave through LeaveWeb at leave.af.mil, which generates the 988 digitally. You’ll need your name, rank, Social Security Number, leave dates, destination, and contact information.
- AF Form 594, Application and Authorization to Start, Stop, or Change Basic Allowance for Quarters: verifies dependents for BAH purposes. Protecting the Information on the Form
Most forms collect personally identifiable information: Social Security Numbers, home addresses, medical data, financial details. The Privacy Act of 1974 requires any federal agency requesting your Social Security Number to tell you whether disclosure is mandatory or voluntary, what legal authority requires it, and how the number will be used. That notice typically appears in a Privacy Act Statement printed on or attached to the form.8Department of Justice. Disclosure of Social Security Numbers
When you hand-carry a completed form containing personal information, cover it with DD Form 2923 (Privacy Act Data Cover Sheet). The cover sheet signals that the enclosed documents contain protected information and must be delivered directly to the intended recipient, not left on a desk or handed off to a third party.9U.S. Army Human Resources Command. Controlled Unclassified Information
Some forms and attachments contain Controlled Unclassified Information (CUI), unclassified data that still requires safeguarding under DoD Instruction 5200.48. At a minimum, documents with CUI must display the acronym “CUI” in the banner at the top and the footer at the bottom of every page. If portion markings are used, each section containing CUI is marked “(CUI)” and unclassified sections are marked “(U).” The first page must also carry a CUI designation indicator block identifying the controlling DoD component, the office making the determination, the CUI category, distribution controls, and a point of contact.10Department of Defense. How to Submit a Completed Form
Where a form goes depends on what it does. The Air Force has moved most recurring personnel workflows into dedicated portals, so paper routing is now the exception.
Digital Portals
Performance evaluations run through myEval, which drafts, routes, and digitally signs the report and auto-populates the ratee’s reporting period information.11Air Force’s Personnel Center. Career Management The virtual Military Personnel Flight (vMPF) handles self-service actions: viewing and printing awards and decorations, accessing your data verification brief, updating portions of your record, updating emergency contact data, and viewing your evaluation history.12162nd Wing. Routing Through Your Chain
Forms that need supervisory endorsement or commander approval, such as award nominations, follow an electronic routing chain. You send the signed PDF to your supervisor or first sergeant, who signs and forwards to the next level. For forms that still require a wet signature (some reserve component documents do), submit the physical copy to your Commander’s Support Staff or orderly room.
Always confirm receipt. Digital portals provide status tracking and automated email notifications. For manually routed documents, keep a personal copy of every signed form. System errors and processing backlogs happen, and your copy is what lets you resubmit quickly instead of starting over.
Fixing a Mistake in Your Record
If you find an error in your official military record, such as a wrong assignment date, an incorrect evaluation, or an erroneous discharge characterization, you can request a correction through the Air Force Board for Correction of Military Records (AFBCMR), the highest administrative appeal authority within the Air Force.13U.S. Department of War. To start, complete DD Form 149 (Application for Correction of Military Record) and submit it with evidence supporting the probable error or injustice. The Air Force’s online application portal at afrba-portal.cce.af.mil is the preferred method.14Department of Defense. What Happens If You Falsify a Form
Submitting false information on an official military form carries consequences under both federal criminal law and the Uniform Code of Military Justice. Investigators do catch discrepancies, especially on forms tied to pay entitlements and security clearances.
Under 18 U.S.C. ยง 1001, knowingly making a materially false statement or using a false document in any matter within federal jurisdiction is a felony punishable by a fine and up to five years in prison.17Office of the Law Revision Counsel. Anyone subject to the UCMJ who signs a false record or makes a false official statement with intent to deceive can be punished as a court-martial may direct.18Office of the Law Revision Counsel. United States Code Title 10 – Section 907 Art 107 False Official Statements and False Swearing Punishment is set by the court based on severity and can include confinement, reduction in rank, forfeiture of pay, and a punitive discharge. Falsifying even a routine form can end a military career.
