How to Fill Out VA Form 2346a: Eligibility, Submission, and Delivery

VA Form 2346a is the request form enrolled veterans use to order a free six-month supply of hearing aid batteries and accessories from the Department of Veterans Affairs. Fill in your identifiers, shipping address, and each hearing aid’s device and battery details, then send the form to the VA Denver Logistics Center by mail, phone, or through the online Medical Device Ordering Tool on VA.gov.¹

Confirm You Qualify First

Three conditions have to be true before the VA will fill your order: you are enrolled in VA health care, you are registered as a patient at a VA medical center, and a VA provider has prescribed your hearing aids or the supplies you’re requesting.¹ If the VA fit your hearing aids after a clinical evaluation, you already meet all three. If you’ve never ordered before, call your local VA audiology clinic to confirm your devices are on file before you send the form in.

What to Have in Front of You

Missing or wrong device details are the most common reason orders come back wrong or slow. Pull these together before you start:

Your full legal name, date of birth, and either your Social Security Number or VA claim number. The Denver Logistics Center uses these to match the order to your medical records.²
Your current mailing address. The form has separate fields for a new permanent address and a new temporary address if you’ve recently moved or are staying somewhere short-term.²
The brand, model, and serial number of each hearing aid. Check the device itself, the original packaging, or the paperwork your audiologist gave you at your fitting.
The battery size for each device, and the specific type if you’re ordering wax guards. If you can’t tell, the color-coded tab on your last shipment identifies the size.¹

The form also covers domes, cleaning supplies, and desiccant drying products for VA-issued hearing aids.¹ Accessories ship separately from batteries, so expect two packages if you order both.² CPAP supplies are not on this form; those go on VA Form 2346b.

Filling Out the Form

The paper form runs one page. A fresh copy came in the box with your last shipment, and you can download one from VA.gov.³ Print in ink. Sloppy handwriting causes data-entry errors that slow the order.

The top section takes your legal name, date of birth, and SSN or VA claim number. Underneath, confirm or update your shipping address. If it has changed since your last order, mark whether the new address is permanent or temporary.

The middle section is where most mistakes happen. For each hearing aid, write the brand, model, and serial number, then the battery type and size. If your two hearing aids use different battery sizes, list each one on its own line. One order covers a six-month supply.²

In the accessories section, select only items that fit your specific model. A dome or wax guard sized for a different hearing aid is useless and slows the logistics center down. If you aren’t sure what fits, ask your VA audiologist before submitting.

Three Ways to Send It In

Online is fastest. Mail is slowest. All three routes deliver the order to the same Denver Logistics Center.

Online

The Medical Device Ordering Tool on VA.gov is the digital version of the form.⁴ From VA.gov, choose “VA Benefits and Health Care,” then “Health care,” then “Order hearing aid batteries and accessories” under “Manage your health.” Sign in with Login.gov or ID.me; DS Logon and My HealtheVet credentials are no longer accepted on VA.gov.⁵

The tool pulls your device information from your VA records, so you won’t type serial numbers manually. Confirm your address, pick your devices and accessories, review, and submit. Save the confirmation number on the final screen.

One catch: if you haven’t ordered batteries or accessories from the VA in the past two years, the online tool won’t show any items for you to select.¹ Use the phone or mail route instead.

Phone

The Denver Logistics Center takes calls Monday through Friday, 8:15 a.m. to 7:30 p.m. ET (TTY: 711). Option 1 is the Automated Battery Reorder System for batteries only. Option 2 puts you through to a representative who can handle both batteries and accessories.¹ Phone is the practical route if the online tool won’t show your items or you’d rather talk to a person.

No postage-paid envelope comes with the form, so add a stamp. Put a return address on the envelope. Mail adds transit time both directions, so expect the longest total wait of the three routes.

Delivery and Reordering

Supplies usually arrive within seven to ten days of submission.¹ Mailed forms take longer because they have to reach Denver before processing starts. The VA suggests reordering about 30 days before your current supply runs out.

Because one order covers roughly six months, the system won’t let you reorder early. The online tool will display the date an item becomes available again if you try before then.⁴ If your batteries are draining faster than expected, contact your VA audiologist rather than trying to order more.
