TRADOC Form 350-18-2-R-E, the Unit Pre-Execution Checklist, is the document your commander signs to certify you meet every prerequisite for a Training and Doctrine Command course before you report. To complete it, pull your current records first — ACFT scorecard, medical readiness printout, body composition worksheet if applicable, physical profile, and security clearance data — then transcribe those figures into the fillable PDF and route it for your signature and the commander’s. The current edition is dated April 2018 and lives on TRADOC’s administrative publications site as a fillable PDF; some installations also mirror it on their own portals.2U.S. Army Fort Campbell. Pull These Records Before You Open the Form
The checklist is only as accurate as the source documents behind it. Assembling everything first is faster than filling and hunting at the same time, and it keeps mismatched numbers off the form.
ACFT Scorecard (DA Form 705)
You need a current DA Form 705 showing a passing Army Combat Fitness Test. The minimum is 60 points per event across all five events, for a combined minimum of 300.3GoArmy.com. Medical and Dental Readiness
Pull your Individual Medical Readiness printout from MEDPROS. It tracks immunizations, physical profiles, dental status, vision, hearing, lab work, and your Periodic Health Assessment.5MEDPROS. Dental Class 1 and Class 2 both count as deployable and mark as a “Go” on the checklist. Class 3, meaning you need urgent or emergent dental treatment, is a “No-Go” and will block enrollment.7United Concordia TRICARE. Dental Readiness Classification
Body Composition
If you exceed the screening height and weight standards in AR 600-9, you’ll need a completed DA Form 5500 (male) or DA Form 5501 (female) documenting your body fat percentage from the circumference tape test.8Department of the Army. Physical Profile
Bring a copy of your DA Form 3349 if you have a permanent profile. Soldiers with a permanent P3 or P4 must also bring the complete results of their MOS Administrative Retention Review (MAR2) when reporting to the school.1TRADOC. TRADOC Regulation 350-18 Reserve Component Soldiers with a P3 or P4 who haven’t completed the MAR2 must still bring the DA Form 3349 at a minimum. Compare the profile restrictions against the course prerequisites before the commander signs; a profile that prohibits activities central to the course will block enrollment.
Security Clearance
Courses that require a clearance list the level in ATRRS and the course catalog. Your Unit Security Manager can verify eligibility through the Defense Information System for Security (DISS), the current DoD-wide personnel security system. Note the clearance level and the investigation completion date. You’ll need both on the form.
Filling In the Fields
Open the fillable PDF and work top to bottom. Personal identifiers — name, rank, and DOD ID — must match your official records exactly, because a mismatched DOD ID is one of the fastest ways to create a processing problem at the schoolhouse.
The ACFT section wants the test date and your event scores. Transcribe those directly from the DA Form 705 rather than working from memory. The medical readiness section uses “Go” and “No-Go” markings for each category: vision, hearing, dental class, immunizations, and PHA currency. If you have a permanent profile, mark its existence on the form and attach the DA Form 3349 to the packet. The body composition section captures height, weight, and body fat percentage if a tape test was performed.
For clearance, enter your eligibility level and investigation completion date. Any course-specific prerequisites (language proficiency scores, prerequisite course completions, specialized certifications) go in the designated blocks. Cross-reference DA Pam 611-21, the program of instruction, and the ATRRS course listing to catch any course-specific requirement the generic checklist doesn’t spell out.1TRADOC. Signing the Form
Two signatures make the form official: yours and your unit commander’s (or an authorized representative’s). Without both, it’s an unofficial draft and the schoolhouse will not accept it.
Your signature certifies the information is accurate to the best of your knowledge. Treat that seriously. The commander’s signature certifies that the Soldier has met all unit-level requirements and is cleared to attend. TRADOC Regulation 350-18 places responsibility for ensuring prerequisites are met on the unit commander.1TRADOC. TRADOC Regulation 350-18
When You Don’t Meet a Prerequisite
If you fall short on one or more prerequisites, don’t leave the line blank and hope for the best. Pursue a waiver before departure. TRADOC Regulation 350-18 requires training waivers to route through the unit chain of command to the course proponent for final approval before the student arrives, and you must report with the approved waiver in hand.1TRADOC. TRADOC does allow a limited grace period for certain documents. Soldiers who report without documented evidence of a security clearance, physical profile, or other non-routine prerequisite have 72 hours from the class start date to produce the required documents. For Reserve Component Soldiers attending inactive duty training courses, the deadline extends to close of business Saturday of the second multiple unit training assembly. Turning It In
Once signed, the checklist goes into the physical training packet you hand-carry to the course location. You also need proper orders. TRADOC Regulation 350-18 prohibits students from traveling or reporting for institutional courses without them. For courses at or near your home station (within 50 miles), the ATRRS Automated Training Application plus a DA Form 4187 or a commander’s memorandum can serve as the order.1TRADOC. At day-zero in-processing, schoolhouse staff compare the checklist against the supporting documents, verifying that scores match, signatures are present, dates fall within valid windows, and any required waivers or profile paperwork are included. Missing non-routine documents trigger the 72-hour window described above. A missing checklist or missing signatures can result in immediate administrative return. A Soldier sent home picks up a “No-Show” code (status “N”) in ATRRS, and noncompliance can be annotated on a DA Form 1059, the Academic Evaluation Report. Starting early, pulling records before filling any fields, and getting the commander’s review at least a week before the report date is what keeps that from happening.
