How to Fill Out the SSD Student Eligibility Form: Sections and 2026 Deadlines

The SSD Student Eligibility Form is the College Board’s request document for testing accommodations on the SAT, PSAT, and AP exams. You complete the 17-section form, attach documentation of your disability, and submit it through your school’s SSD coordinator using the SSD Online portal at ssdonline.collegeboard.org. Independent submissions can go by fax or mail. What to Gather Before You Start

Incomplete submissions are the top reason for delays, and the College Board will not start reviewing until every required item has arrived.1College Board. Know Your Dates and Deadlines A doctor’s note or an existing IEP or 504 Plan by itself is rarely enough.2College Board. When a Request Is Denied What counts as sufficient depends on your disability category:

  • Learning disorders such as dyslexia or dyscalculia: a psychoeducational or neuropsychological evaluation covering cognitive and achievement testing. The educational portion should be no more than five years old. Cognitive testing can be older, but evaluations done before third grade may not reflect current abilities.3College Board.   • ADHD: a clinical evaluation with a specific diagnosis, evidence of symptoms in more than one setting, and records showing academic impact.

  • Physical or medical conditions: a physician letter describing the condition, how it affects testing under standard conditions, and any equipment needed.

One threshold question worth settling before you fill anything out: are you requesting accommodations that you actually use at school? The College Board cross-references your requests against your school records. Asking for extended time on the SAT when you do not receive extended time on classroom tests is the fastest route to a denial.6College Board. Accommodations

Filling Out the 17 Sections

Sections 1 Through 11: Personal and School Information

Sections 1 through 8 collect your full legal name, mailing address, school name, date of birth, gender, email, and country or postal code if you are outside the United States. Section 9 asks for your College Board High School Code, a six-digit number sometimes called the AI code. Your counselor or SSD coordinator can look this up. Sections 10 and 11 ask for the date of your next College Board test and your expected high school graduation date. The College Board uses these to determine which deadlines apply to you, so get them right.

Sections 12 Through 15: Accommodations, Disability, and Documentation

Section 12 is where you select what you need. Options include extended time at 50% or 100% additional duration, braille format, large print, small group testing, extra breaks, and assistive technology such as text-to-speech.10College Board. Section 13 asks whether you currently use those accommodations on school-based tests. Section 14 asks you to categorize your disability into the correct group: learning disability, ADHD, hearing impairment, psychiatric disorder, physical or medical condition, and so on. Section 15 covers what documentation you are attaching or uploading.

Sections 16 and 17: Signatures

Section 16 is signed by the school’s SSD coordinator or an official school representative. The coordinator verifies that the accommodations you listed are ones the school provides, that documentation meeting College Board guidelines is on file, and that the form is accurate.8College Board. How to Submit

The fastest path is through your school’s SSD coordinator, who submits the request and uploads documentation through the SSD Online portal.11College Board. 2026 Deadlines

AP Exams

For the May 4–8 and May 11–15, 2026 testing weeks, requests and documentation must be received by January 16, 2026.1College Board. After You Submit

Once the College Board has your complete request, review takes up to seven weeks. If the reviewer asks for additional records, the clock resets for another seven weeks from the date those new documents arrive.14College Board. Your SSD coordinator can track status through the SSD Online dashboard, where each student appears as Approved, Not Approved, or Pending. You can check your own status through the Accommodations and Supports Dashboard at ssdstudents.collegeboard.org. Approval is portable. Once granted, your accommodations apply to the SAT, PSAT/NMSQT, PSAT 10, and AP Exams without a new request each time, and they remain in effect until one year after high school graduation.16College Board. Ive Been Granted Exam Accommodations by College Board in the Past Do I Need to Apply Again Two wrinkles: accommodations can be subject-specific, so extended time approved only for math does not carry to AP English; and on AP tests, extra breaks are a separate accommodation from extended time, so request both if you need both.

The SAT and PSAT are now delivered digitally through the Bluebook application, and approved accommodations load automatically when you open the app on test day.17Bluebook. If You Are Denied or Partially Approved

Denials generally fall into two buckets: the College Board needs more information, or the documentation submitted does not demonstrate a functional impairment that supports the requested accommodations.2College Board. If the issue is missing documentation, gather the records the letter identifies and resubmit through your SSD coordinator. If the documentation was found insufficient, you need different or additional evidence, not another copy of what you already sent. Asking for a lesser accommodation such as 50% extended time instead of 100% without new supporting evidence is unlikely to change the outcome.2College Board. When a Request Is Denied

Partial approvals are common: the College Board may grant some accommodations while denying others if the documentation supports only part of what you asked for. Any resubmission triggers another seven-week window, so weigh timing against your upcoming test dates before deciding whether to press the denied portion.

Temporary Injuries Use a Different Form

A broken hand, recent surgery, or acute injury that will resolve before the next testing cycle does not go through the SSD Student Eligibility Form. The College Board calls these temporary supports, and they are available only to students already registered for a test who cannot postpone. Eligible tests include the weekend SAT, SAT School Day for seniors, AP Exams, and state-provided PSAT 8/9, PSAT 10, and SAT School Day.19College Board. Requesting Temporary Assistance

To request temporary supports, the SSD coordinator selects New Request and then Temporary Request in SSD Online. The school needs a signed parental consent form and a doctor’s letter describing the injury, degree of impairment, date it occurred, and expected recovery date. AP exams also require a Teacher’s Survey from each relevant AP teacher. Requests submitted less than two weeks before a scheduled test may not be approved in time.19College Board.
